lished data suggests a 21% rate of local<br />

recurrence overall, with a 17%, 25% and<br />

27% rate in benign, borderline and<br />

malignant PTs, respectively, and a 10%<br />

rate of metastases overall, with a 0%, 4%<br />

and 22% rate in benign, borderline and<br />

malignant PTs, respectively {1887}. Local<br />

re c u r rence after surgery is stro n g l y<br />

dependent on the width of the excision<br />

margins {186}.<br />

Mammary hamartomas<br />

Definition<br />

A well demarcated, generally encapsulated<br />

mass, composed of all components<br />

of <strong>breast</strong> tissue.<br />

Epidemiology<br />

Hamartomas occur predominantly in the<br />

peri-menopausal age group, but may be<br />

found at any age, including teenagers<br />

and post-menopausal women.<br />

Clinical features<br />

H a m a rtomas are frequently asymptomatic<br />

and only revealed by mammography<br />

{3042}. They are detected in 0.16%<br />

of mammograms {1204}. Ve ry large<br />

lesions can deform the <strong>breast</strong>. Due to<br />

their well defined borders they are easily<br />

enucleated.<br />

Macroscopy<br />

Hamartomas are round, oval, or discoid,<br />

ranging in size from 1 cm to more than 20<br />

cm. Depending on the composition of the<br />

lesion the cut surface may resemble normal<br />

<strong>breast</strong> tissue, a lipoma or may be<br />

rubbery and reminiscent of a FA.<br />

Histopathology<br />

Generally encapsulated, this circ u m-<br />

scribed mass of <strong>breast</strong> tissue may show<br />

f i b rocystic or atrophic changes;<br />

pseudoangiomatous hyperplasia (PASH)<br />

is frequent {446}. The lesion gives the<br />

impression of “<strong>breast</strong> within <strong>breast</strong>”. In<br />

adolescents, differentiation between the<br />

appearance of the normal adolescent<br />

<strong>breast</strong> and FAs or asymmetric virginal<br />

hypertrophy can be difficult. Rare examples<br />

resembling phyllodes tumours, have<br />

been observed {2876}.<br />

Variants of hamartoma<br />

Adenolipoma {867}, adenohibern o m a<br />

{618}, and myoid hamartoma {624} could<br />

all be considered variants of mammary<br />

hamartoma.<br />

Prognosis<br />

The lesion is benign with no tendency to<br />

recur.<br />
