children, if they survive infancy, are at high<br />

risk <strong>of</strong> dying later in life from another<br />

diarrheal episode or from other infectious<br />

diseases. Thus a vicious cycle is <strong>of</strong>ten<br />

established and it is difficult to know which<br />

<strong>of</strong> the conditions should be considered as<br />

underlying cause (diarrheal disease or nutritional<br />

deficiency) because <strong>of</strong> their reciprocal<br />

implications,<br />

Another difficulty is frequently encountered<br />

when the duration <strong>of</strong> the illness is to<br />

be determined in cases where diarrheal disease<br />

recurred at various times within short<br />

intervals: for one informant the onset could<br />

have been that <strong>of</strong> the first episode, while for<br />

another it could have been the last. Likewise,<br />

the treating physician could state that<br />

the child was suffering from an acute or<br />

from a chronic gastroenteritis, which have<br />

different categories and fall in different seetions<br />

<strong>of</strong> the InternationalClassification.Finally,<br />

the assignment by etiologic agent<br />

becomes complicated because <strong>of</strong>ten more<br />

than one pathogenic agent is isolated in bacteriologic<br />

culture; also, if cultures are performed<br />

and are positive they may have been<br />

taken during a previous episode and the<br />

current manifestations may not be due to<br />

the same agent. It is highly possible that<br />

as frequency and quality <strong>of</strong> diagnostic<br />

methods are improved an increasing number<br />

<strong>of</strong> deaths now included in category 009<br />

will be assigned to more specific categories.<br />

Mortality from diarrheal disease by<br />

month <strong>of</strong> age in the first year <strong>of</strong> life differed<br />

markedly in Latin America, as can be seen<br />

in Table 73 and in Figure 71, where 13 projects<br />
