就 個 人 資 料 之 聲 明
本 人 向 「 香 港 寬 頻 」 遞 交 本 表 格 , 並 同 意 以 下 安 排 :1. 以 下 提 供 的 所 有 資 料 均 正 確 無 訛 。2. 香 港
寬 頻 收 集 及 持 有 關 於 本 人 的 個 人 資 料 可 能 會 被 香 港 寬 頻 用 於 以 下 用 途 : (a) 為 本 處 理 下 述 服 務 的 申
請 ; (b) 為 本 人 提 供 下 述 服 務 ; (c) 在 本 人 申 請 使 用 下 述 服 務 時 核 實 身 分 及 / 或 作 信 貸 審 查 ; (d) 為 本 人 處
理 與 下 述 服 務 有 關 的 付 款 指 示 或 追 收 欠 款 ; (e) 推 廣 香 港 寬 頻 服 務 ; (f) 為 本 人 設 計 所 需 的 服 務 ; (g) 進
行 市 場 研 究 或 顧 客 意 見 調 查 ; (h) 為 提 供 客 戶 服 務 ; (i) 處 理 投 訴 及 賬 戶 查 詢 ; (j) 遵 守 適 用 的 法 例 、 規 則
或 規 例 ; 及 (k) 任 何 與 收 集 個 人 資 料 的 原 來 目 的 直 接 有 關 之 其 他 用 途 。3. 香 港 寬 頻 收 集 及 持 有 關 於
本 人 的 個 人 資 料 均 會 被 保 密 處 理 ; 但 若 需 要 按 法 例 的 規 定 或 要 求 而 履 行 法 律 責 任 , 或 為 本 人 提 供
下 述 服 務 , 或 執 行 收 集 個 人 資 料 的 原 來 目 的 或 直 接 有 關 之 目 的 , 香 港 寬 頻 可 能 會 向 下 述 人 士 披 露
該 等 資 料 ( 不 論 在 香 港 境 內 或 境 外 ): (a) 電 訊 管 理 局 或 其 他 政 府 部 門 、 法 定 機 構 或 組 織 ; (b) 香 港 寬 頻
之 聯 繫 公 司 、 參 與 該 服 務 銷 售 或 行 政 , 或 提 供 香 港 寬 頻 服 務 的 承 辦 商 、 代 理 或 其 他 電 訊 服 務 供 應
商 ; 及 (c) 銀 行 、 金 融 機 構 、 信 用 卡 發 行 公 司 或 追 收 欠 款 服 務 公 司 。4. 如 欲 查 閱 或 更 正 個 人 資 料 , 或
不 欲 收 到 關 於 香 港 寬 頻 服 務 的 宣 傳 函 件 或 資 訊 , 本 人 會 以 書 面 方 式 通 知 香 港 寬 頻 私 隱 資 料 主 任 ,

1. 客 戶 同 意 及 授 權 「 香 港 寬 頻 網 絡 有 限 公 司 」(「 香 港 寬 頻 」) 根 據 此 登 記 表 格 上 之 資 料 作 為 更 新 客 戶 之 「 個 人 賬 戶 」 資 料 之 用 。2. 客
戶 同 意 及 接 受 「 香 港 寬 頻 」 將 以 上 述 所 指 定 的 方 式 向 客 戶 發 放 月 結 單 及 其 他 服 務 有 關 之 通 信 而 該 等 函 件 將 視 作 正 式 通 知 。3. 「 香 港 寬 頻 」
保 留 不 時 調 整 郵 寄 月 結 單 ( 如 適 用 ) 行 政 費 之 金 額 。 4. 客 戶 同 意 及 接 受 以 上 述 所 指 定 的 方 式 收 取 任 何 由 「 香 港 寬 頻 」 或 其 聯 繫 公 司 所 提 供
之 推 廣 優 惠 資 料 。 5. 此 更 改 服 務 將 於 「 香 港 寬 頻 」 收 到 本 表 格 後 七 天 內 生 效 。

1. Customer agrees and authorizes Hong Kong Broadband Network Ltd. (“HKBN”) to update the information of his/her Personal Account
according to the information provided herein. 2. Customer agrees and accepts HKBN will send him/her the monthly statement via the above
assigned method and those correspondences are treated as official notice.3. HKBN reserves the right to change the amount of administrative fee
of the monthly statement by post (if applicable) from time to time. 4. Customer agrees to receive promotional information from HKBN or its
associated companies via the above assigned method. 5. This request for information amendment will be effective within 7 days upon HKBN’s
receipt of this form.
