4. Functional outcome measures on discharge from intensive care: An evaluation of
an extended seven day working pilot

Abstract: INTRODUCTION. Evidence suggests that on discharge from intensive care units
(ICU)patients can have persistent functional impairment. Functional activity on ICU can be
limitedby various factors, including early access to a rehabilitative culture (1). Extended 7
dayservices have been shown to enable early physiotherapy intervention in the acute care
setting(2).OBJECTIVES. To compare functional outcome measures and rehabilitation
treatments(Rehab Rxs) between a usual Monday to Friday, reduced weekend, (MF)
physiotherapy serviceand a 7DW pilot.METHODS. Over 5 weeks during the 7DW pilot, all
patients admitted to ICU who wereinvasively ventilated >48 h, survived ICU and transferred
to a ward, were functionally testedon discharge from ICU. Outcome measures used were
the Barthel Index score, Elderlymobility scale and dynamometer hand grip strength. All
data collected was compared with thatpreviously collected for a different project, with the
same inclusion criteria, over 9 weeksduring the usual MF service. Rehab Rxs were
obtained from patient records.RESULTS. ICU mortality for patients ventilated[48 h, was 27
and 34%, during the usualMFservice and 7DW pilot periods, respectively. Participants
characteristics are shown below. (Table presented) Compared with the usual MF service,
during the 7DW pilot a 54% and 362% increase in theaverage number of Rehab Rxs was
seen on weekdays and weekends, respectively. The averagenumber of Rehab Rxs per day
(range) for the7DWpilot was 11 (8.6-12) compared with 6 (1.8-8.4) for the usualMFservice.
All functional outcome measures improved during the7DWpilotwhen compared with the
usual MF service (Figure). (Graph presented) CONCLUSIONS. A 7DW service afforded
delivery of more consistent and increased rehabilitativeinterventions to patients on ICU. It
is not possible to suggest that this increase wasdirectly responsible for improved outcome
measures seen during the7DWpilot. However, thisis a positive outcome with respect to the
impact of extended physiotherapy services.

Abstract: Aim: A Wales wide strategy recommended the instigation f 24/7 working by all
specialist teams across all areas - both rural and urban. Costing of the service was high for
specialist nurse working a 24/7 rota, although 24/7 consultant on call was feasible. Method:
All specialist nurses were asked to explore 7 day working. A Ministerial letter to all services
requested change to 7 day working, to involve senior nurses at all levels in palliative care
services. Change involved discussion groups, consultation with unions over terms and
conditions of employment, patient feedback that the service was needed and awareness of
the Ministerial directive. Results: Initial estimates for change suggested costs of around
>=34,000 per service across a population of about 750,000, but careful planning resulted in
change for around >=8,000 - >=12,000 per annum per service. Concern about service
safety was unfounded. Clinical problems emerged that previously would have been
neglected, patient feedback was positive, nurses drew on consultant support, reported
finding the work rewarding and that it highlighted the need to maintain a wide range of
nursing skills. Opportunistic teaching occurred during weekend working, particularly of
hospital junior medical and nursing staff. The rotas in different parts of Wales were variably
organised. Some covered hospital and community separately; others worked across these
boundaries in a defined geographic area. Conclusion: In a 7 day society the delivery of 7
day care is feasible and practical without incurring great expenditure.
