Executive Summary<br />

<strong>Health</strong> Goals <strong>and</strong> Population Outcomes<br />

Four health goals have been identified within the 2013-2018 Strategic Plan, <strong>and</strong><br />

they will provide direction for all program work done within the <strong>Health</strong> <strong>Unit</strong>.<br />

For each goal, specific population outcomes have been identified which will be<br />

the focus for <strong>Health</strong> <strong>Unit</strong> programs <strong>and</strong> services:<br />

Goal #1 <strong>Health</strong>y Baby/Child/Youth Growth <strong>and</strong> Development.<br />

A baby/child/youth friendly community exists in <strong>Leeds</strong>,<strong>Grenville</strong> & <strong>Lanark</strong>.<br />

Children have effective parenting.<br />

Children <strong>and</strong> Youth have Developmental Assets®.<br />

Goal #2 <strong>Health</strong>y Living for all Residents.<br />

Residents participate in regular outdoor physical activity & play.<br />

Residents eat healthy foods.<br />

ff<br />

Residents maintain oral health.<br />

ff<br />

Residents manage stress <strong>and</strong> have positive mental health.<br />

ff<br />

Residents keep immunizations up-to-date.<br />

ff<br />

Residents make healthy, active choices re: sexuality, tobacco, alcohol, drugs,<br />

injury, infection prevention.<br />

Goal #3 <strong>Health</strong> Equity within the Population.<br />

Residents have food security.<br />

ff<br />

Residents have strong social connections.<br />

ff<br />

Residents have access to healthy living opportunities.<br />

ff<br />

Residents have access to public health services.<br />

Goal #4 <strong>Health</strong>y Environments.<br />

Residents live in an environment with healthy air, water <strong>and</strong> food.<br />

ff<br />

Childcare <strong>and</strong> residential care settings are hygienic.<br />

ff<br />

Residents live in a healthy built environment.<br />

The goals are interconnected <strong>and</strong> mutually dependent. Children with healthy<br />

development are more likely to make choices for healthy living. <strong>Health</strong>y<br />

environments support healthy living choices <strong>and</strong> healthy child development <strong>and</strong><br />

improve health equity, so that there is less disparity within the population. More<br />

residents having access to good housing, adequate income <strong>and</strong> clothing, healthy<br />

food, <strong>and</strong> recreation opportunities will contribute to healthy child development<br />

<strong>and</strong> healthy living.<br />
