The Trust has developed a Clinical Services Strategy to drive the objectives of the<br />

Momentum programme and w hich is in alignment w ith the Clear and Credible Plans<br />

developed by the CCGs and this is pictured above. The overriding philosophy is to ensure<br />

care is provided closer to home w here possible and is of a high quality and safe standard<br />

with the aim to improve health outcomes for patients; as a result of w hich the 97% of patient<br />

contacts with healthcare services provided by the Trust w hich remain in <strong>Hartlepool</strong> w ill<br />

continue to expand, w ith acute hospital care provision being for the sickest patients who<br />

require the expertise contained w ithin.<br />

Follow ing the implementation of the proposed changes, the follow ing additional service<br />

developments are planned to take place in the <strong>Hartlepool</strong> area:<br />

Long Term Conditions:<br />

• Facilitate patient empow erment through development of personal care plans w orking<br />

with the patient, w ith improved co-ordination of services based on the patient’s<br />

preferences, enabling them to become experts in managing their ow n health<br />

conditions.<br />

• Maximise the contribution of state of the art technology to the management of long<br />

term conditions.<br />

• Consultant led teams focussed on long ter m conditions providing a comprehensive<br />

outreach and in-reach service based on patient needs.<br />

Planned Care:<br />

• Work w ith the patient to agree a mutually acceptable and transparent pathw ay.<br />

• Utilise innovative technology and proven best practice.<br />

• Provide services in an appropriate location and by the appropriate healthcare<br />

professional(s) to achieve the best quality outcomes this could include alternative<br />

facilities in <strong>Hartlepool</strong>.<br />
