<strong>AUGMENTIN</strong> 625 毫 克 錠 劑 : 每 一 錠 劑 含 有 500 毫 克 amoxycillin ( 以 amoxycillin<br />
trihydrate 的 形 式 ) 與 125 毫 克 clavulanic acid ( 以 potassium clavulanate 的 形 式 )。<br />
<strong>AUGMENTIN</strong> 625 毫 克 錠 劑 : 白 / 灰 白 色 卵 圓 型 、 膜 包 衣 、 無 刻 字 之 錠 劑 , 錠 劑<br />
的 一 面 有 劃 線 , 一 面 無 紋 路 。<br />
臨 床 特 性<br />
【 適 應 症 】<br />
葡 萄 球 菌 , 鏈 球 菌 , 肺 炎 雙 球 菌 , 腦 膜 炎 球 菌 及 其 他 具 有 感 受 性 細 菌 引 起 之 感 染<br />
症 。<br />
【 說 明 】<br />
<strong>AUGMENTIN</strong> 是 一 種 廣 效 性 的 抗 生 素 製 劑 , 它 對 於 一 般 診 所 及 醫 院 中 所 常 見 的<br />
細 菌 病 原 體 都 有 效 。Clavulanate 的 β-lactamase 抑 制 作 用 將 amoxycillin 的 抗 菌 譜<br />
擴 展 至 更 大 範 圍 的 微 生 物 , 包 括 許 多 對 其 他 β-lactam 抗 生 素 具 抗 藥 性 的 微 生 物 。<br />
<strong>AUGMENTIN</strong> 口 服 製 劑 為 每 日 服 用 二 次 的 給 藥 設 計 , 它 可 使 用 於 下 列 部 位 細 菌<br />
感 染 的 短 期 治 療 :<br />
上 呼 吸 道 感 染 ( 包 括 耳 鼻 喉 ), 例 如 扁 桃 腺 炎 、 鼻 竇 炎 、 中 耳 炎 等 。<br />
下 呼 吸 道 感 染 , 例 如 急 性 與 慢 性 支 氣 管 炎 , 肺 葉 性 及 支 氣 管 肺 炎 。<br />
生 殖 - 泌 尿 道 感 染 , 例 如 膀 胱 炎 、 尿 道 炎 及 腎 盂 腎 炎 。<br />
皮 膚 及 軟 組 織 感 染 , 例 如 癤 、 膿 瘍 、 蜂 窩 組 織 炎 及 創 傷 感 染 。<br />
骨 及 關 節 感 染 , 例 如 骨 髓 炎 。<br />
牙 科 感 染 , 例 如 牙 槽 膿 腫 。<br />

<strong>AUGMENTIN</strong> 是 一 種 廣 效 性 的 殺 菌 劑 , 其 有 效 的 微 生 物 包 括 :<br />

【 劑 量 與 用 法 】<br />
治 療 感 染 的 一 般 劑 量<br />
成 人 與 12 歲 以 上 的 兒 童<br />
輕 度 至 中 度 的 感 染<br />
<strong>AUGMENTIN</strong> 625 毫 克 錠 劑 一 顆 , 每 日 服 用 二 次 。<br />
治 療 的 方 式 可 先 以 注 射 的 方 式 開 始 給 藥 , 再 改 以 口 服 的 方 式 繼 續 給 藥 。<br />
牙 科 感 染 的 劑 量 ( 例 如 牙 槽 膿 腫 )<br />
成 人 與 12 歲 以 上 的 兒 童 :<strong>AUGMENTIN</strong> 625 毫 克 錠 劑 一 顆 , 每 日 服 用 二 次 , 連<br />
續 使 用 5 天 。<br />
<strong>AUGMENTIN</strong> 625 毫 克 錠 劑 並 不 建 議 使 用 於 12 歲 以 下 的 兒 童 。<br />

為 了 將 此 藥 物 對 腸 胃 所 造 成 的 不 適 降 至 最 低 , 應 於 用 餐 開 始 時 服 用 。 此 藥 在 用 餐<br />
開 始 時 服 用 可 得 到 最 佳 吸 收 效 果 。<br />
如 未 重 作 評 估 , 此 藥 之 使 用 不 可 持 續 超 過 14 天 。<br />
【 禁 忌 症 】<br />
對 β-lactams(penicillins, cephalosporins) 有 過 敏 病 史 之 病 人 。<br />
出 現 過 與 <strong>AUGMENTIN</strong> 或 penicillin 有 關 的 黃 疸 / 肝 功 能 失 常 的 病 史 。<br />

【 警 語 及 注 意 事 項 】<br />
在 開 始 <strong>AUGMENTIN</strong> 治 療 之 前 , 應 仔 細 詢 問 是 否 曾 經 發 生 penicillins、<br />
cephalosporins 以 及 其 他 過 敏 原 相 關 之 過 敏 反 應 。<br />
以 penicillins 治 療 的 病 患 曾 有 嚴 重 且 偶 爾 會 致 死 之 過 敏 性 ( 類 過 敏 性 ) 反 應 的 報<br />
告 。 這 些 反 應 較 易 發 生 於 有 penicillins 過 敏 病 史 的 患 者 ( 見 “ 禁 忌 症 ”)。<br />
疑 似 有 感 染 性 單 核 白 血 球 增 多 症 時 , 應 避 免 使 用 <strong>AUGMENTIN</strong>, 因 為 在 這 個 狀<br />
況 下 使 用 amoxycillin 可 能 出 現 麻 疹 樣 疹 。<br />
延 長 使 用 偶 而 可 能 會 造 成 非 感 受 性 微 生 物 的 過 度 生 長 。<br />
曾 有 罕 見 的 報 告 顯 示 病 人 在 接 受 <strong>AUGMENTIN</strong> 及 口 服 抗 凝 血 治 療 後 會 出 現 不<br />
正 常 的 凝 血 時 間 (prothrombin time) 延 長 (INR 上 升 )。 對 於 同 時 正 在 接 受 抗 凝 血 療<br />
法 的 病 人 , 應 小 心 監 測 。 必 要 時 應 調 整 口 服 抗 凝 血 製 劑 以 維 持 適 當 的 抗 凝 血 功 能 。<br />
某 些 接 受 <strong>AUGMENTIN</strong> 治 療 的 病 人 中 , 曾 發 現 肝 功 能 試 驗 結 果 產 生 變 化 的 例<br />
子 。 這 些 變 化 的 臨 床 意 義 尚 不 清 楚 , 但 肝 功 能 不 正 常 的 病 人 在 使 用 <strong>AUGMENTIN</strong><br />
時 應 特 別 小 心 。<br />
極 少 數 報 告 會 有 膽 汁 鬱 滯 性 黃 疸 的 出 現 , 可 能 會 很 嚴 重 但 為 可 逆 。 在 停 止 給 藥 後<br />
六 週 內 , 其 徵 候 及 症 狀 可 能 漸 不 明 顯 。<br />
對 於 患 有 腎 功 能 損 傷 的 病 人 ,<strong>AUGMENTIN</strong> 的 劑 量 應 依 照 劑 量 及 用 法 一 節 所 建<br />
議 的 內 容 作 適 當 調 整 。<br />
在 排 尿 量 減 少 的 病 人 , 曾 經 發 生 結 晶 尿 , 但 非 常 罕 見 , 大 多 數 發 生 在 接 受 注 射 治<br />
療 的 病 人 。 高 劑 量 amoxycillin 給 藥 期 間 , 建 議 維 持 適 當 的 水 份 攝 取 和 排 尿 , 以<br />
減 少 發 生 結 晶 尿 的 可 能 性 ( 見 “ 過 量 ”)。<br />
【 藥 物 交 互 作 用 】<br />
不 建 議 同 時 使 用 probenecid, 因 其 會 減 少 腎 小 管 對 <strong>AUGMENTIN</strong> 的 排 除 作 用 。<br />
<strong>AUGMENTIN</strong> 與 probenecid 併 用 , 可 能 會 提 高 並 延 長 amoxycillin 的 血 中 濃 度 ,<br />
不 過 不 影 響 clavulanate。<br />
Amoxycillin 與 allopurinol 併 用 會 增 加 皮 膚 過 敏 反 應 的 可 能 性 ; 至 於<br />
<strong>AUGMENTIN</strong> 與 allopurinol 併 用 , 目 前 無 相 關 數 據 。<br />
和 其 他 抗 生 素 相 同 ,<strong>AUGMENTIN</strong> 可 能 會 影 響 腸 內 菌 株 , 造 成 雌 激 素 再 吸 收 降<br />
低 並 使 得 口 服 避 孕 藥 效 果 下 降 。<br />
曾 有 罕 見 的 案 例 見 於 文 獻 報 告 中 , 關 於 持 續 使 用 acenocoumarol 或 warfarin 且<br />
併 用 amoxycillin 的 病 患 出 現 INR (International normalized ratio) 上 升 的 情 形 。<br />
如 需 併 用 此 類 藥 物 , 應 小 心 監 測 凝 血 時 間 或 International normalized ratio 且 適<br />
時 增 加 或 停 用 <strong>AUGMENTIN</strong>。<br />
【 懷 孕 與 授 乳 】<br />
利 用 小 老 鼠 及 大 白 鼠 所 進 行 的 動 物 試 驗 中 , 在 經 口 及 注 射 的 方 式 投 予<br />
<strong>AUGMENTIN</strong> 後 顯 示 此 藥 並 不 會 有 畸 胎 的 情 形 發 生 。 一 針 對 孕 婦 早 產 和 胚 胎 膜<br />
早 熟 破 裂 (premature rupture of the foetal membrane, pPROM) 進 行 之 單 盲 試 驗<br />
指 出 ,<strong>AUGMENTIN</strong> 之 預 防 療 法 可 能 與 新 生 兒 壞 死 性 腸 炎 的 危 險 性 增 加 有 關 。<br />
如 同 所 有 其 他 藥 品 一 樣 , 若 非 醫 師 認 為 有 必 要 , 在 懷 孕 期 間 應 避 免 使 用 , 尤 其 是<br />

在 懷 孕 的 前 三 個 月 。<br />
在 授 乳 的 期 間 仍 可 使 用 <strong>AUGMENTIN</strong>, 本 藥 用 於 母 體 時 , 對 於 嬰 兒 並 不 會 有 所<br />
傷 害 , 唯 一 的 例 外 是 當 微 量 的 藥 物 隨 著 母 乳 進 入 嬰 兒 體 內 時 , 可 能 會 有 造 成 過 敏<br />
的 危 險 。<br />
【 對 駕 駛 及 機 械 操 作 能 力 的 影 響 】<br />
目 前 未 發 現 有 相 關 副 作 用 。<br />
【 不 良 反 應 】<br />
根 據 大 型 臨 床 試 驗 的 數 據 , 將 副 作 用 的 發 生 率 歸 類 為 極 常 見 (very common)、 常<br />
見 (common)、 不 常 見 (uncommon) 或 罕 見 (rare)。 至 於 歸 類 為 極 罕 見 (very rare)<br />
的 副 作 用 , 則 是 使 用 上 市 後 的 數 據 , 因 此 其 較 接 近 通 報 率 而 非 實 際 的 發 生 率 。<br />

極 罕 見 : 肝 炎 及 膽 汁 鬱 滯 性 黃 膽 。 其 他 penicillins、cephalosporins 亦 有 相 同<br />
之 報 告 。<br />
肝 臟 反 應 主 要 發 生 於 男 性 及 高 齡 患 者 , 並 可 能 與 治 療 時 間 延 長 有 關 。 在 孩 童 則 極<br />
罕 見 。<br />
徵 候 及 症 候 可 通 常 在 治 療 期 間 或 治 療 後 不 久 即 出 現 , 但 也 有 些 病 例 是 在 治 療 結 束<br />
數 週 後 才 發 生 。 這 些 反 應 通 常 是 可 逆 轉 的 。 至 於 肝 臟 方 面 的 反 應 但 可 能 會 很 嚴<br />
重 , 也 曾 報 告 有 極 罕 見 的 死 亡 病 例 。 這 些 大 部 分 發 生 在 帶 有 嚴 重 疾 病 的 病 人 或 同<br />

【 過 量 】<br />
可 能 有 明 顯 的 腸 胃 道 症 狀 、 體 液 與 電 解 質 的 失 衡 。 採 取 症 狀 療 法 , 並 注 意 水 與 電<br />
解 質 的 平 衡 , 可 緩 解 症 狀 。 曾 經 發 現 amoxycillin 結 晶 尿 在 某 些 情 況 下 引 起 腎 衰<br />
竭 ( 見 警 語 及 注 意 事 項 )。<br />
可 以 血 液 透 析 移 除 循 環 中 的 <strong>AUGMENTIN</strong>。<br />
藥 理 學 特 性<br />
【 藥 效 學 】<br />
細 菌 的 抗 藥 性 來 自 於 細 菌 產 生 的 酵 素 , 使 抗 生 素 作 用 在 病 原 體 前 就 已 被 破 壞 。<br />
<strong>AUGMENTIN</strong> 中 的 clavulanate 就 是 針 對 此 一 防 衛 機 制 抑 制 β-lactamase, 增 加 微<br />
生 物 對 抗 生 素 的 感 受 度 , 使 amoxycillin 在 一 般 血 中 濃 度 時 就 能 對 微 生 物 形 成 快<br />
速 的 殺 菌 效 果 。Clavulanate 本 身 略 具 抗 菌 效 果 , 但 與 amoxycillin 配 方 成<br />
<strong>AUGMENTIN</strong> 後 , 則 形 成 一 廣 效 性 抗 生 素 , 可 廣 泛 使 用 於 醫 院 與 診 所 。<br />
【 藥 物 動 力 學 】<br />
<strong>AUGMENTIN</strong> 的 兩 個 主 成 分 其 藥 物 動 力 學 參 數 緊 密 配 合 , 兩 者 的 最 高 血 中 濃 度<br />
都 大 約 在 口 服 後 一 小 時 左 右 。<strong>AUGMENTIN</strong> 的 口 服 吸 收 在 用 餐 開 始 時 達 最 佳 狀<br />
態 。<br />
將 <strong>AUGMENTIN</strong> 劑 量 調 為 兩 倍 時 , 血 中 濃 度 也 大 約 到 達 兩 倍 。<br />
Clavulanate 與 amoxycillin 兩 者 與 血 漿 結 合 的 比 例 都 不 高 ; 血 漿 中 約 有 70% 以 游<br />
離 態 存 在 。<br />

QUALITATIVE AND QUANTITATIVE COMPOSITION<br />
<strong>AUGMENTIN</strong> 625 mg tablets: Each tablet contains 500 mg amoxycillin (as amoxycillin<br />
trihydrate) and 125 mg clavulanic acid (as potassium clavulanate).<br />
PHARMACEUTICAL FORM<br />
<strong>AUGMENTIN</strong> 625 mg tablets: A white to off-white oval- shaped film- coated debossed<br />
tablet, with a score line on one side and plain on the other side.<br />
CLINICAL PARTICULARS<br />
Indications<br />
<strong>AUGMENTIN</strong> is an antibiotic agent with a notably broad spectrum of activity against the<br />
commonly occurring bacterial pathogens in general practice and hospital. The β-<br />
lactamase inhibitory action of clavulanate extends the spectrum of amoxycillin to<br />
embrace a wider range of organisms, including many resistant to other β-lactam<br />
antibiotics.<br />
<strong>AUGMENTIN</strong> oral presentations for twice daily dosing, are indicated for short-term<br />
treatment of bacterial infections at the following sites:<br />
Upper respiratory tract infections (including ENT) e.g. tonsillitis, sinusitis, otitis media.<br />
Lower respiratory tract infections e.g. acute exacerbation of chronic bronchitis, lobar and<br />
bronchopneumonia.<br />
Genito-urinary tract infections e.g. cystitis, urethritis, pyelonephritis.<br />
Skin and soft tissue infections, e.g. boils, abscesses, cellulitis, wound infections.<br />
Bone and joint infections e.g. osteomyelitis.<br />
Dental infections e.g. dentoalveolar abscess<br />
Other infections e.g. septic abortion, puerperal sepsis, intra-abdominal sepsis.<br />
<strong>AUGMENTIN</strong> is bactericidal to a wide range of organisms including:<br />

Therapy can be started parenterally and continued with an oral preparation.<br />

Administration<br />
<strong>Tablets</strong> should be swallowed whole without chewing. If required, tablets may be broken<br />
in half and swallowed without chewing.<br />
To minimise potential gastrointestinal intolerance, administer at the start of a meal. The<br />
absorption of <strong>AUGMENTIN</strong> is optimised when taken at the start of a meal.<br />
Treatment should not be extended beyond 14 days without review.<br />
Contraindications<br />
<strong>AUGMENTIN</strong> is contraindicated in patients with a history of hypersensitivity to betalactams,<br />
e.g. penicillins and cephalosporins.<br />
<strong>AUGMENTIN</strong> is contraindicated in patients with a previous history of <strong>AUGMENTIN</strong>associated<br />
jaundice/hepatic dysfunction.<br />
Warnings and Precautions<br />
Before initiating therapy with <strong>AUGMENTIN</strong> careful enquiry should be made concerning<br />
previous hypersensitivity reactions to penicillins, cephalosporins, or other allergens.<br />
Serious and occasionally fatal hypersensitivity (anaphylactoid) reactions have been<br />
reported in patients on penicillin therapy. These reactions are more likely to occur in<br />
individuals with a history of penicillin hypersensitivity (see Contra-indications).<br />
<strong>AUGMENTIN</strong> should be avoided if infectious mononucleosis is suspected since the<br />
occurrence of a morbilliform rash has been associated with this condition following the<br />
use of amoxycillin.<br />
Prolonged use may also occasionally result in overgrowth of non-susceptible organisms.<br />
Abnormal prolongation of prothrombin time (increased INR) has been reported rarely in<br />
patients receiving <strong>AUGMENTIN</strong> and oral anticoagulants. Appropriate monitoring should<br />
be undertaken when anticoagulants are prescribed concurrently. Adjustments in the dose<br />
of oral anticoagulants may be necessary to maintain the desired level of anticoagulation.<br />
Changes in liver function tests have been observed in some patients receiving<br />
<strong>AUGMENTIN</strong>. The clinical significance of these changes is uncertain. <strong>AUGMENTIN</strong><br />
should be used with caution in patients with evidence of hepatic dysfunction.<br />
Cholestatic jaundice, which may be severe, but is usually reversible, has been reported<br />
rarely. Signs and symptoms may not become apparent for up to six weeks after treatment<br />
has ceased.<br />
In patients with renal impairment <strong>AUGMENTIN</strong> dosage should be adjusted as<br />
recommended in the Dosage and Administration section.
CONFIDENTIAL<br />
In patients with reduced urine output, crystalluria has been observed very rarely,<br />
predominantly with parenteral therapy. During the administration of high doses of<br />
amoxycillin, it is advisable to maintain adequate fluid intake and urinary output in order<br />
to reduce the possibility of amoxycillin crystalluria (see Overdose).<br />
Interactions<br />
Concomitant use of probenecid is not recommended. Probenecid decreases the renal<br />
tubular secretion of amoxycillin. Concomitant use with <strong>AUGMENTIN</strong> may result in<br />
increased and prolonged blood levels of amoxycillin but not of clavulanate.<br />
Concomitant use of allopurinol during treatment with amoxycillin can increase the<br />
likelihood of allergic skin reactions. There are no data on the concomitant use of<br />
<strong>AUGMENTIN</strong> and allopurinol.<br />
In common with other antibiotics, <strong>AUGMENTIN</strong> may affect the gut flora, leading to<br />
lower oestrogen reabsorption and reduced efficacy of combined oral contraceptives.<br />
In the literature there are rare cases of increased international normalised ratio in patients<br />
maintained on acenocoumarol or warfarin and prescribed a course of amoxycillin. If coadministration<br />
is necessary, the prothrombin time or international normalised ratio should<br />
be carefully monitored with the addition or withdrawal of <strong>AUGMENTIN</strong>.<br />
Pregnancy and Lactation<br />
Reproduction studies in animals (mice and rats) with orally and parenterally administered<br />
<strong>AUGMENTIN</strong> have shown no teratogenic effects. In a single study in women with<br />
preterm, premature rupture of the foetal membrane (pPROM), it was reported that<br />
prophylactic treatment with <strong>AUGMENTIN</strong> may be associated with an increased risk of<br />
necrotising enterocolitis in neonates. As with all medicines, use should be avoided in<br />
pregnancy, especially during the first trimester, unless considered essential by the<br />
physician.<br />
<strong>AUGMENTIN</strong> may be administered during the period of lactation. With the exception of<br />
the risk of sensitisation, associated with the excretion of trace quantities in breast milk,<br />
there are no detrimental effects for the infant.<br />
Effects on Ability to Drive and Use Machines<br />
Adverse effects on the ability to drive or operate machinery have not been observed.<br />
Adverse Reactions<br />
Data from large clinical trials were used to determine the frequency of very common to<br />
rare undesirable effects. The frequencies assigned to all other undesirable effects (i.e.,<br />

A moderate rise in AST and/or ALT has been noted in patients treated<br />
with beta-lactam class antibiotics, but the significance of these findings is<br />
unknown.<br />

Hepatic events have been reported predominantly in males and elderly patients and may<br />
be associated with prolonged treatment. These events have been very rarely reported in<br />
children.<br />
Signs and symptoms usually occur during or shortly after treatment but in some cases<br />
may not become apparent until several weeks after treatment has ceased. These are<br />
usually reversible. Hepatic events may be severe and in extremely rare circumstances,<br />
deaths have been reported. These have almost always occurred in patients with serious<br />
underlying disease or taking concomitant medications known to have the potential for<br />
hepatic effects.<br />

Gastrointestinal symptoms and disturbance of the fluid and electrolyte balances may be<br />
evident. Gastrointestinal symptoms may be treated symptomatically with attention to the<br />
water electrolyte balance.<br />
Amoxycillin crystalluria, in some cases leading to renal failure, has been observed (see<br />
Warnings and Precautions).<br />
<strong>AUGMENTIN</strong> can be removed from the circulation by haemodialysis.<br />
PHARMACOLOGICAL PROPERTIES<br />
Pharmacodynamics<br />
Resistance to many antibiotics is caused by bacterial enzymes which destroy the<br />
antibiotic before it can act on the pathogen. The clavulanate in <strong>AUGMENTIN</strong> anticipates<br />
this defence mechanism by blocking the β-lactamase enzymes, thus rendering the<br />
organisms susceptible to amoxycillin’s rapid bactericidal effect at concentrations readily<br />
attainable in the body. Clavulanate by itself has little antibacterial activity; however, in<br />
association with amoxycillin as <strong>AUGMENTIN</strong> it produces an antibiotic agent of broad<br />
spectrum with wide application in hospital and general practice.<br />
Pharmacokinetics<br />
The pharmacokinetics of the two components of <strong>AUGMENTIN</strong> are closely matched. Peak<br />
serum levels of both occur about 1 hour after oral administration. Absorption of<br />
<strong>AUGMENTIN</strong> is optimised at the start of a meal.<br />
Doubling the dosage of <strong>AUGMENTIN</strong> approximately doubles the serum levels achieved.<br />
Both clavulanate and amoxycillin have low levels of serum binding; about 70% remains<br />
free in the serum.<br />
