Attached is the <strong>Special</strong> <strong>Investigation</strong> <strong>Report</strong> for the above referenced facility. Due to<br />
the violations identified in the report, a written corrective action plan is required. The<br />
corrective action plan is due 15 days from the date of this letter and must include the<br />
following:<br />
• How compliance with each rule will be achieved.<br />
• Who is directly responsible for implementing the corrective action for each<br />
violation.<br />
• Specific time frames for each violation as to when the correction will be<br />
completed or implemented.<br />
• How continuing compliance will be maintained once compliance is<br />
achieved.<br />
• The signature of the responsible party and a date.<br />
If you desire technical assistance in addressing these issues, please feel free to contact<br />
me. In any event, the corrective action plan is due within 15 days. Failure to submit an<br />
acceptable corrective action plan will result in disciplinary action.<br />

ALLEGATION(S)<br />
• Caregivers are not washing their hands after changing diapers. Caregivers are<br />
changing children on the carpet instead of on the changing table. A child’s diaper<br />
was not changed all day.<br />
• Milk sits out for over an hour before being put back into the refrigerator and is<br />
used the following day. Food that has been in the refrigerator for days is being<br />
served for lunch.<br />

ALLEGATION:<br />
Caregivers are not washing their hands after changing diapers. Caregivers are<br />
changing children on the carpet instead of on the changing table. A child’s diaper<br />
was not changed all day.<br />
INVESTIGATION:<br />
On April 29, 2011 I talked to Joni Sluys, Licensee and Program Director for The Looking<br />
Glass Child Care Center. Joni Sluys said that the allegations are not true and believes<br />
a disgruntled employee who is leaving the center made these allegations. Joni Sluys<br />
said that they have regular staff meetings with all caregivers and diaper procedures and<br />
how to record information in the diaper log are reviewed several times during the year.<br />
The complaint was made by an anonymous person and no specific child was identified.<br />
On April 29, 2011, I completed an observation of diaper change procedures in the infant<br />
room and the toddler room. I also completed interviews with Caregiver 1 and Caregiver<br />
2 in the infant room and Caregiver 3 and Caregiver 4 in the toddler room. I observed<br />
Caregiver 1 changing a diaper in the infant room and observed Caregiver 3 and<br />
Caregiver 4 changing a diaper in the toddler room. All the caregivers used the<br />
designated changing areas in each room. All the caregivers washed their hands after<br />

changing diapers. The designated diaper change areas meet the child care licensing<br />
rules.<br />
After observing diaper changing in each room, I interviewed all four caregivers. All four<br />
caregivers said that they always use the designated changing area for changing<br />
diapers. All four caregivers said they always wash their hands after changing diapers.<br />
Caregiver 3 and Caregiver 4 in the toddler room said that they have one toddler who is<br />
big and if they need to change his clothes, they will change his clothes on the floor<br />
rather than on the changing table.<br />
I reviewed all the diaper change and feeding forms for each enrolled infant and toddler.<br />
Each infant and toddler has their own separate notebook located on a shelf outside the<br />
toddler room. The form for the current day is kept in the room and then transferred to<br />
the notebook at the end of the day. Each parent can review current and previous logs<br />
of their child’s diaper change and feeding without having to ask a caregiver to give it to<br />
them. I did not see anything on any of the forms in the notebooks that would indicate a<br />
child was not changed all day or infrequently.<br />
APPLICABLE RULE<br />
R 400.5209 Diapering; toileting.<br />
(1) Diapering shall occur in a designated diapering area that<br />
shall be all of the following:<br />
(c) Have non-absorbent smooth, easily sanitized surfaces in<br />
good repair and maintained in a safe and sanitary manner.<br />
ANALYSIS:<br />
CONCLUSION:<br />
Diapering is done on a non-absorbent, easily sanitized surface.<br />
VIOLATION NOT ESTABLISHED<br />
APPLICABLE RULE<br />
R 400.5209 Diapering; toileting.<br />
(3) The caregiver shall frequently check diapers/training pants<br />
and change diapers or training pants that are wet or soiled.<br />
ANALYSIS:<br />
CONCLUSION:<br />
There is no information to substantiate that children are not<br />
being changed as needed.<br />
VIOLATION NOT ESTABLISHED<br />

(6) The caregiver shall thoroughly wash his or her hands after<br />
each diapering and after cleaning up bodily fluids.<br />
ANALYSIS:<br />
CONCLUSION:<br />
There is no information to substantiate that caregivers are not<br />
washing their hands after changing diapers.<br />
VIOLATION NOT ESTABLISHED<br />
ALLEGATION:<br />
Milk sits out for over an hour before being put back into the refrigerator and is<br />
used the following day. Food that has been in the refrigerator for days is being<br />
served for lunch.<br />
INVESTIGATION:<br />
On April 29, 2011, Joni Sluys said that they do not serve left over food to children and<br />
they do not save food from one day to the next. There are a lot of strict health<br />
department regulations that have to be met in order to save food from one day to the<br />
next. She said It is time consuming and not worth their time to save food from one day<br />
to the next.<br />
The complaint form was submitted by an anonymous person and does not indicate any<br />
specific dates of incidents or children being affected.<br />
I inspected the refrigerator for the child care center. I reviewed two menus from the<br />
previous two days. I did not observe any leftover food served on the two previous days<br />
in the refrigerator. There was no indication that any of the food in the refrigerator was<br />
previously cooked, served or being saved.<br />
I inspected the gallon milk containers. I observed one gallon container that was opened.<br />
This container did not have an opening date and time marked on it. Joni Sluys said that<br />
it was probably opened that morning at breakfast. A half an hour later, I observed a<br />
milk container on the food cart. This container had the date and time on it. The<br />
caregiver standing with the food-cart said that once snack or meal time is done, the food<br />
cart goes back to the kitchen and the milk is put back into the refrigerator. The<br />
caregiver did not know how long it takes to complete snack or meals but believes it is<br />
not longer than 1 hour.<br />

APPLICABLE RULE<br />
R 400.5902c Food preparation.<br />
(12) All of the following shall apply to milk:<br />
(a) Containers shall be labeled with the date and time<br />
opened.<br />
ANALYSIS:<br />
CONCLUSION:<br />
One container of opened milk did not have a date or time the<br />
container of milk was opened.<br />
VIOLATION ESTABLISHED<br />
APPLICABLE RULE<br />
R 400.5902c Food preparation.<br />
(12) All of the following shall apply to milk:<br />
(c) Milk shall not be served if the contents appear to be<br />
unsanitary, or have been unrefrigerated for a period exceeding 1<br />
hour.<br />
ANALYSIS:<br />
CONCLUSION:<br />
There is no information to substantiate that milk is being left out<br />
longer than 1 hour.<br />
VIOLATION NOT ESTABLISHED<br />
APPLICABLE RULE<br />
R400.5902c Food Preparation<br />
(8) Potentially hazardous foods that have been cooked and then<br />
refrigerated shall be reheated rapidly to 165 degrees Fahrenheit<br />
or higher throughout before being served or before being placed<br />
in a hot food storage facility.<br />
ANALYSIS:<br />
CONCLUSION:<br />
There is no indication that previously cooked or prepared food is<br />
being reserved or reheated to be served again.<br />

RECOMMENDATION<br />
Upon receipt of an acceptable corrective action plan, it is recommended that the<br />
license remain in effect.<br />
