SPECIAL PROVISIONS Project No. C-28(492) labor, equipment, tools and incidentals required to place the Type III barricade. The minimum length of a Type III barricade shall be eight (8) feet. B. The furnishing, erecting and maintaining of all other traffic control and protection devices shown on the plans or as ordered by the engineer shall not be measured and paid for directly, but they shall be considered subsidiary to the items for which the contract provides that direct payment will be made. CONTRACT START DATE Work under this contract will begin on or about August 26, 2013. Contractor’s request for an earlier start date will be considered. The contract days for this project will be strictly enforced. CONTRACT PAYMENT Payment for work under this contract will be made for only those items listed in the proposal. The cost of all other work, materials and miscellaneous items necessary to construct this project in accordance with the plans and specifications, shall be considered incidental to the unit bid prices of the items listed in the proposals. NPDES PERMIT This project has been authorized to discharge storm water under NPDES General Permit NER110000. The authorization number is NER113798. The contractor is responsible for compliance with the terms and conditions of this permit and the Storm Water Pollution Prevention Plan (SWPPP) which are housed at the Douglas County Engineer’s Office and available for review upon request. STATUS OF UTILITIES At this point, all known utility companies in the area have been contacted and are shown on the plans for the convenience of the contractor. It is the responsibility of the contractor to properly locate all utilities in the field, both shown and not shown in the plans. Below is a list of known local utilities and their current status: Omaha Public Power District OPPD has reviewed the 90% plans for the project listed above. There is a three phase overhead line on the south side in conflict and will be relocated to new right of way. OPPD has notified Douglas County that the conflicting poles will be relocated concurrently with the project and after removal of the trees on the south side of Fort Street. Contact: Joe Ostblom - (402) 636-3513 CenturyLink CenturyLink has a 9 way duct structure running east and west on the south side of Fort Street. This is running around 22' to 24' Rt and will need to be moved /adjusted for the grade cuts in this area. At first glance I would like to lower this in place concurrently with the project, if the general contractor would remove grade from the northside first then allow our contractor to move in to drop SP - 4
SPECIAL PROVISIONS Project No. C-28(492) the duct to its new elevation. The time it would take depends on the material of the ductline. I won't have that confirmed until the potholing is completed. More recently Centurylink has notified Douglas County that they may be able to lower the line prior to project construction. Contact: Jack Dodendorf - (402) 572-5814 Cox Communications Cox Communications was sent plans on February 26th, 2013. Cox Communications notified Douglas County that a fiber optics line that feeds the cellular tower at the SE corner of the intersection is located in the project limits and will be impacted. The line runs along the south side of Fort Street from 177th Street to 180th Street. It then runs along the east side of 180th Street, south of Fort Street, before turning east to the tower. Cox intends to lower the line at its current location and anticipates that this will be completed prior to project construction. Contact: Ken Porter - (402) 510-1617 MCI Telecommunications Digger's Hotline records indicate that MCI Telecommunications has facilities in the project area. Verizon Business was sent plans on February 26th, 2013. No information has been received Contact: Steve Bonczkowski - (630)395-6701 TEMPORARY TRAFFIC CONTROL It is anticipated that Fort Street will be closed to through traffic from the East side of 180th Street to 177th Street for the duration of the project. This closure will require three Type III barricades with one Road Closed sign at both ends of Fort Street. One Road Closed Ahead sign and One Road Closed 500' shall be located on Type III barricades East and West of the project on Fort Street. A Road Closed to Through Traffic sign shall be located on a Type III barricade along Fort Street, immediately West of 168th Street. The closure of the 180th & Fort Street intersection shall be kept to a minimum duration. It is anticipated that the intersection closure will occur near the completion of grading operations on Fort Street. During the intersection closure, Fort Street barricading shall be maintained, as described above, with the relocation of three Type III barricades and Road Closed sign from East side of the intersection to the West. Additionally, Road Closed to Through Traffic Signs shall be mounted on Type III barricades and be placed along Fort Street, East of 204th Street, East of 195th Street, and East of 192nd Street. Along 180th Street, north and south of the project the contractor shall place a Road Closed with three Type III barricades, a Road Closed Ahead sign on a Type II barricade, and a Road Closed 500' sign on a Type III barricade. The contractor shall also place a Road Closed to Through Traffic sign on a Type III barricade north of West Maple Road and South of Ida Street. SP - 5
