Does this transit system maintain either a record that each employee has received a copy of<br />

the anti-drug policy, or written notice that the policy is available for review?<br />

Were the actual job duties at this transit system reviewed to decide who performed safetysensitive<br />

functions?<br />

Does this transit system have a company-wide testing program, including testing that goes<br />

beyond the requirements of the FTA regulations?<br />

Does this transit system assure that the <strong>Federal</strong> Drug Testing Custody and Control Form and<br />

the DOT Breath Testing Form are not used for testing of non-safety-sensitive employees?<br />

How is the employee notified of the FTA authority for each test conducted under FTA<br />

What arrangements have been made to conduct drug and alcohol tests after normal business<br />

hours and on weekends?<br />

Have all company officials who are qualified to make Reasonable Suspicion referrals received<br />
