− Documents related to the random selection process<br />

− Documents generated in connection with decisions to administer<br />

reasonable suspicion alcohol tests Record Keeping<br />

− Documents generated in connection with decisions on postaccident<br />

tests<br />

− Documents showing existence of medical explanation of inability<br />

of safety-sensitive employee to provide enough breath for test<br />

− Records of inspection, maintenance, and calibration of EBT<br />

♦ Education and Training Records:<br />

− Materials on alcohol misuse awareness, including a copy of the<br />

employer’s policy on alcohol misuse<br />

− Educational materials that explain the regulatory requirements<br />

− The employer’s policy and procedures with respect to<br />

implementing the regulatory requirements<br />

− Names of safety-sensitive employees attending training on<br />

prohibited drug use and the dates and times of such training<br />

− Documentation of training provided to supervisors to qualify<br />

them to make reasonable suspicion determinations<br />

− Certification that training complies with the regulatory<br />

requirements<br />

− The employer’s copy of the chain-of-custody form<br />

♦ Documents related to the refusal of any safety-sensitive employee to<br />

submit to an alcohol test required by 49 CFR Part 654<br />
