conditions. Testing<br />

should not be delayed because the<br />

employee indicates that they don’t<br />

believe they can provide a specimen.<br />

If this is the case, the collector<br />

should follow the “insufficient<br />

volume” procedures. Likewise the<br />

test should not be delayed to await<br />

union representation.<br />

3. In the event both drug and alcohol<br />

tests are required, the alcohol test<br />

should be conducted first, if possible.<br />

4. The collector must inspect the<br />

collection room before and after each<br />

specimen collection. Any<br />

unauthorized persons and materials<br />

that could be used to adulterate the<br />

specimen must be removed. Access<br />

to the room must be restricted while<br />

the collection is taking place. The<br />

collector must make sure all<br />

collection supplies are available, the<br />

area is properly secured, water<br />

sources are secured, and bluing agent<br />

is placed in all toilets as described<br />

previously.<br />

5. The collector must verify the identity<br />

of the employee through the use of<br />

an official photo identification card<br />

issued by the employer or a federal,<br />

state or local government (e.g.,<br />

driver’s license). Faxes or<br />

photocopies of identification are not<br />

acceptable. If photo identification is<br />

not available, verification by a transit<br />

agency representative is acceptable,<br />

but cannot be made by a coworker or<br />

other employee being tested. If the<br />

identity of the employee cannot be<br />

verified, the collector must contact<br />

the DER immediately to verify the<br />

employee’s identity.<br />

6. The collector must explain the basic<br />

collection procedures to the<br />

employee and show the employee<br />

the written instructions provided on<br />

the back of the <strong>Federal</strong> Drug Testing<br />

Custody and Control form.<br />

7. Only <strong>Federal</strong> Drug Testing Custody<br />

and Control forms can be used. If<br />

non-<strong>Federal</strong> forms are used for a<br />

federal DOT test, the test will be<br />

cancelled unless corrective<br />

procedures are followed. The<br />

preprinted specimen ID number must<br />

be checked to be sure it matches the<br />

specimen ID number on the<br />

preprinted specimen bottle seals.<br />

The collector should make sure that<br />

the laboratory’s, employer’s, and<br />

MRO’s name and address are<br />

correctly printed on the form. Some<br />

collection sites do testing for a<br />

number of clients, and problems with<br />

billing and reporting can occur if the<br />

wrong forms are used.<br />

The collector must complete Step 1<br />

of the form by entering the<br />

employee’s identification number,<br />

reason for the test, drug test to be<br />

performed, and the collection site<br />

information. Refusal to provide a<br />

social security number is not a test<br />

refusal, but the collector must make<br />

note of such in the remarks section<br />

of the form.<br />

8. The collector must direct the<br />

employee to remove outer clothing<br />

(coveralls, sweaters, jackets, vests,<br />

hats, etc.) and to leave these<br />

garments along with other personal<br />

belongings (purses, briefcases,<br />

backpacks) outside the privacy<br />

enclosure. If requested, the<br />

employee should be provided a<br />

receipt for items left. Employees are<br />

allowed to keep their wallets.<br />
