Supplies. The following supplies and<br />

documents will be needed at each collection<br />

site you use. The laboratory with which you<br />

contract usually provides these supplies.<br />

The collection kit has been standardized and<br />

the specifications are defined in Appendix A<br />

of Part 40.<br />

• Single-Use Collection Cups – The<br />

cups must be plastic and large<br />

enough to easily catch and hold at<br />

least 55 mL of urine. The cups must<br />

have graduated volume markings<br />

clearly noting levels of 45 mL and<br />

above. The cups must have a<br />

temperature strip attached that can<br />

provide temperature readings<br />

between 90°F and 100°F. The cups<br />

must be individually wrapped or<br />

have a peelable, sealed lid (much<br />

like an orange juice container) or<br />

other visible tamper-evident system.<br />

• Plastic Specimen Bottles – The<br />

plastic specimen bottles must be<br />

large enough to hold at least 35 mL<br />

of urine and must have a screw-on or<br />

snap-on cap that prevents seepage<br />

during shipment. The bottles must<br />

have clear markings indicating the<br />

30 mL level for the primary<br />

specimen, and the 15 mL level for<br />

the split specimen. The bottles must<br />

be designed so that the required<br />

tamper-evident tape bottle seals fit<br />

with no damage to the seal when<br />

applied or initialed. The bottles must<br />

be wrapped in a sealed plastic bag,<br />

shrink wrapped, have a sealed lid, or<br />

be packaged by some other easily<br />

visible tamper-evident system. The<br />

plastic material must be leak<br />

resistant.<br />

• Leak-Resistant Plastic Bag – The<br />

plastic bag must have two sealable<br />

compartments or pouches that are<br />

leak resistant. One pouch must be<br />

large enough to hold two specimen<br />

bottles and the other large enough to<br />

hold the CCF paper work. Once<br />

closed, the bag must be tamperevident.<br />

• Absorbent Material – Each kit must<br />

contain enough absorbent material to<br />

absorb the contents of both specimen<br />

bottles, and the material must be<br />

provided in the plastic bag pouch<br />

that holds the specimen bottles.<br />

• Shipping Container – The shipping<br />

container must be designed to<br />

adequately protect the specimen<br />

bottles from damage during<br />

shipping.<br />

• <strong>Federal</strong> Drug Testing Custody and<br />

Control Form (CCF)– This form<br />

must be used to document every<br />

urine collection required under the<br />

DOT testing program. The five-part<br />

form must include the names,<br />

addresses, telephone numbers, and<br />

fax numbers of the employer and the<br />

MRO. The CCF documents the<br />

chain of custody and is legal<br />

evidence that the reported test results<br />

apply to the donor. The <strong>Federal</strong><br />

CCFs cannot be used for non-DOT<br />

urine collections. Furthermore, nonfederal<br />

CCFs cannot be used for<br />

DOT urine collections. The CCF<br />

form was revised on June 23, 2000.<br />
