esolving program deficiencies should be presented in descending order of importance. Detailed<br />

support for the findings, if required, should be placed in an appendix to the Spot Report.<br />

Examples of the discussion of some findings and resultant recommendations are:<br />

1) The recipient does not have a functioning Safety and Security Working Group. The<br />

SSMP identifies a Safety and Security Working Group (SSWG) that will be established<br />

prior to start of PE to assure that safety and security requirements, including police and<br />

fire regulations are incorporated into all phases of the design. The project is requesting<br />

entrance into PE and the PMOC has found no evidence that a SSWG exists. The<br />

Director of Safety, who would normally be either chair or co-chair of a SSWG was<br />

unable to state when a SSWG would become functional. The PMOC recommends that<br />

the recipient create a SSWG, as identified in the SSMP, and set a regular schedule for<br />

meetings. The SSWG should include participation from city, transit agency, and<br />

county agencies that the right of way traverses. Entry into PE should not be granted<br />

until the SSWG is formed and functional.<br />

2) The recipient has not addressed egress and overcrowding on platforms during<br />

periods of heavy system use. Overcrowding and lack of adequate egress is hazardous<br />

and introduces security vulnerabilities; neither the PHA nor TVA has addressed this<br />

issue at the stations serving the college and the high school and the design criteria are<br />

silent on maximum platform loads. These issues must be resolved with the local<br />

academic institutions, which generate increased ridership during those months that<br />

classes are in session. The PMOC recommends assessments of maximum passenger<br />

loads on these platforms, and the rate of flow through egress points, through formal<br />

hazard analyses and TVAs.<br />

Issues and Analysis<br />

In the course of the review, the PMOC may encounter safety and security issues that can affect or<br />

be affected by the project but do not constitute Findings, as discussed, above. These should be<br />

presented as the last subsection in Section 5.0.<br />

An example of the kind of issues that might be included is:<br />

The PMOC identified three schedule changes that relate to project safety and security:<br />

1) The tunnel TVA originally planned for September 2006 is now forecast to be completed in<br />

January 2007. This will delay review of the TVA by the city, delay issuance of tunnel bid<br />

package, and reduce schedule float by at least two months<br />

2) Changes in personnel in local police/fire departments have delayed formation of the Safety<br />

and Security Working Group originally planned for April 2006. It is currently planned that<br />

the new police and fire commissioners, named in December 2006, will select their<br />
