Documentation required from sponsors of stepchildren:<br />
1. Housing letter that shows child is authorized to reside in<br />
household.<br />
2. Legal documents showing that the spouse has legal primary<br />
custody of the child (this may be a separate custody agreement or<br />
part of a divorce decree-we only need to see information relating to<br />
custody)<br />
3. If the spouse's last name is different from the child's, birth<br />
certificate (this could occur if spouse has remarried more than once)<br />
4. In loco parentis affidavit executed by sponsor and notarized.<br />
Health Requirements for registration<br />
1. School physical completed within the past 6 months to a year or<br />
proof of an appointment for a physical.<br />
2. Each student must have his/her own social security number.<br />
Immunization Requirements are provided after school<br />
zoning.<br />

IAW Department of Defense Education Activity DoDEA guidelines and<br />
mandates, as well as to comply with Kentucky law, starting in 2004-05 school year, there<br />
will be a policy change in how Ft. <strong>Campbell</strong> Schools handle student immunizations<br />
against communicable diseases. As you know all students must have a Kentucky<br />
Immunization Certificate showing that all their immunizations are up to date and a<br />
physical when they are new to our schools. All sixth graders must also have a physical<br />
and their shot record will be reviewed as well. The following immunizations are required<br />
by DoDEA:<br />
• Hepatitus B (Hep B)<br />
• Diphtheria and tetanus toxoids and pertussis vaccine [acellular (DyaP) is<br />
preferred over whole cell (DPT) for all doses in the series]<br />
• Haemophilis influenzae Type B (Hib)<br />
• Poliovirus [combination of injectable (IPV) and oral (OPV) is preferred]<br />
• Measles-mumps-rubella (MMR)<br />
• Varicella (Var) (Chicken pox)<br />
(TB tests are no longer required.)<br />
Students will no longer be allowed to attend classes without an up-to-date<br />
immunization record and/or evidence of a doctor's appointment to complete a physical if<br />
required. This also pertains to students who need an additional immunization during the<br />
course of the school year. As you know immunizations are recurrent events. Our school<br />
nurses are very diligent in tracking individual student immunization deadlines over the<br />
course of a year. It is therefore very important that you take the time and insure that your<br />
student's immunizations are always up to date.<br />
Please know that the safety of all students is the driving force behind any policy<br />
change.
