d. The GEH committee must document on the form (GEH # 2) the committee’s decision<br />

regarding the type(s) and amount of instruction to be provided to the student. This must<br />

include the designated amount of time per week that instruction will be provided and for<br />

what subjects.<br />

e. Documentation of the day(s) homebound instruction started and stopped.<br />

Meeting to Return the Student to <strong>School</strong><br />

1. When the physician provides a medical release date to return to school, the GEH Committee<br />

meets and completes the GEH form # 2 to dismiss the student from Homebound Services. The<br />

parent could participate by phone if they wish.<br />

2. File the General Education Homebound Summary Form # 3 and the General Education<br />

Homebound <strong>Teacher</strong> Contact Log From # 5 along with the other General Education<br />

Homebound Forms from the first meeting in the student’s cumulative folder.<br />

Attendance/Calculating eligible days present<br />

Amount of time served per week: Eligible days present/earned per week:<br />

One hour One day present<br />

Two hours Two days present<br />

Three hours Three days present<br />

Four or more hours Four days present (4-day week)<br />

Five days present (5-day week)<br />

1. Excused absences<br />

a. Student is too ill to complete school work.<br />

b. Student is contagious<br />

c. Family member is contagious<br />

d. Student has a doctor’s appointment<br />

2. Unexcused absences<br />

a. Unexcused absences may result in no grade (NG) and the need to reconsider<br />

Homebound Services.<br />

<strong>School</strong>work<br />

1. Textbooks<br />

a. Textbooks should be provided to the student from his/her campus.<br />

<strong>School</strong> should check them all out to the student the day of the GEH meeting.<br />

b. Parents are responsible for checking out and returning textbooks. Neither homebound<br />

instructors nor school personnel will be able to check out textbooks.<br />

2. Make-up work<br />

a. If the student has been out of school for more than five days prior to the initial General<br />

Education Homebound meeting, the family should request make-up work from the<br />

student’s campus.<br />

3. Extracurricular or employment<br />

a. Students who are too ill to attend school are considered too ill to participate in<br />

extracurricular activities or to hold a job.<br />

b. If the student exhibits the ability for such activities, the student would not continue with<br />

Homebound Services.<br />
