Van de voorzitter From the Chairman 

Beste Interplast vrienden, 

Voor u ligt het jaaroverzicht van 2010 van Stichting 

Interplast Holland. Het afgelopen jaar zijn acht teams 

uitgezonden, te weten naar Nigeria, Guinee Bissau, 

Burundi, Zanzibar en Uganda. We hebben 619 patiënten 

kunnen opereren. 

Er is geprobeerd om veel aan opleiding te doen. Hoewel 

dit in de nieuwe landen nog moeizaam van de grond komt, 

ziet het er in Guinee-Bissau en Burundi hoopvol uit voor de 

toekomst. We worden in alle landen zeer gewaardeerd 

voor het goede en hooggekwalificeerde werk dat Interplast 

levert. 

In Oeganda starten we in 2011 met een Brandwondenpreventieprogramma 

in samenwerking met de Nederlandse 

Brandwondenstichting. Het blijkt dat de impact van een 

Burns Awareness Week te gering is. Dit jaar zijn de nodige 

voorbereidingen getroffen. Zo is in juni onder andere een 

nulmeting in de sloppenwijken rond Mulago Hospital uitgevoerd. 

In 2011 zullen professionals van de Neder landse 

Brand wonden stichting naar Kampala afreizen om mensen 

te trainen tot Burn Prevention Officers. 

In Burundi hebben we gezien dat de noodzaak er is voor 

een brandwondenafdeling en eventueel een Plastisch 

Chirurgische Afdeling. In 2011 gaan we inventariseren 

wat de mogelijkheden hiervoor zijn en willen we de nodige 

investeringen aanbieden, met het idee hetzelfde te kunnen 

bewerkstelligen als in Oeganda. 

Dit jaar was een teamuitzending naar Jemen niet mogelijk 

in verband met de onrustige politieke situatie. We hopen 

dat dit snel verandert. 

Ook dit jaar hebben we helaas weer een trieste boodschap 

ontvangen. Deo Mbonimpa is overleden tijdens zijn werk 

in het Kennemer Gasthuis. Deo’s ‘In Memoriam’ kunt u 

lezen op pagina 25. 

De Verpleegkundige Commissie is uitgebreid. Marie- 

Thérèse de By-Bakker, Paula Ellen en Elles Loenen 

hebben de taken van Els Gerritsen uitstekend overgenomen. 

Marjo Aerts neemt al jaren de anesthesie kant voor 

haar rekening. 

Dear Interplast friends, 

Stichting Interplast Holland is pleased to present the 

2010 annual review. This past year eight teams went 

on missions to Nigeria, Guinea-Bissau, Burundi, Zanzibar 

and Uganda. We operated on 619 patients. 

The endeavour this year was to put a great effort into 

training. And although this is a laborious process in our 

new countries, Guinea-Bissau and Burundi are showing 

promise for the future. Interplast is appreciated in all the 

countries we visit for our good and highly qualified 

work. 

In Uganda we are commencing the Burns Prevention 

Programme 2011 in cooperation with the Dutch Burns 

Foundation. The impact of the Burns Awareness Week 

is not sufficient. This year we made the necessary 

preparations for the new programme. In June zero 

measurements were carried out in the slums around 

Mulago Hospital. In 2011 professionals from the Dutch 

Burns Foundation will travel to Kampala to train locals 

to be burn prevention officers. 

In Burundi we have seen that there is also a need for a 

burns unit, and in due course, a plastic surgery section 

as well. We will be taking stock of the options for these 

in 2011 and we hope to offer the necessary funds for 

investment with the idea of achieving the same set-up 

as in Uganda. 

The team mission to Yemen was impossible because of 

the turbulent political situation. We hope this changes 

fast. 

This year, too, we have unfortunately received sad news. 

Deo Mbonimpa died at work at the Kennemer Gasthuis. 

You can read Deo’s In Memoriam on page 25. 

The nursing committee has new members. Marie-Thérèse 

de By-Bakker, Paula Ellen and Elles Loenen, have done 

well in taking over the tasks of Els Gerritsen. 

Marjo Aerts has been in charge of anaesthetics for many 

years now. 


Stichting Interplast Holland is nog steeds in staat om met 

beperkte middelen grote impact te hebben op het leven van 

arme mensen met een lichamelijke handicap. Steeds vaker 

wordt echter van ons verwacht dat we de lokale staf en het 

ziekenhuis betalen voor hun medewerking. Onze mening 

blijft dat het gastland en ziekenhuis iets voor hun eigen 

landgenoten over moet hebben. Dit is soms lastig vol te 

houden, want sommige organisaties uit andere landen 

betalen wel. 

Ondanks het feit dat de tijden economisch en ook politiek 

moeilijk zijn op dit moment, blijven we ons inzetten voor de 

armste mensen in de samenleving. 

Tot slot wil ik al onze teamleden, staf en vooral de 

sponsoren zeer bedanken voor hun inzet in het afgelopen 

jaar. Zonder jullie bijdrage en betrokkenheid was het niet 

gelukt.

Stichting Interplast Holland remains capable of making 

an enormous impact on the life of poor people with 

a physical handicap using limited resources. But 

increasingly we are being asked to pay local staff 

and the hospital for their cooperation. We remain 

convinced that the host country and the hospital 

must be prepared to do something for their fellow 

countrymen. This can be a problem at times because 

some organisations from other countries do pay. 

Despite the difficult times in the economic and political 

sense we will continue to do our best for the poorest 

people in the community. 

To close I should like to express my great gratitude to our 

team members, staff and above all the sponsors for their 

efforts in the past year. Without your contributions and 

commitment we would not succeed.

Inleiding 

Stichting Interplast Holland is een ideële organisatie die 

zich richt op reconstructieve chirurgie voor kinderen en 

(jong) volwassenen in ontwikkelingslanden. Jaarlijks wordt 

een aantal teams van ervaren Nederlandse plastisch en 

reconstructief chirurgen, anesthesiologen en operatieen 

anesthesieassistenten uitgezonden. 

De artsen en assistenten, die ter plekke worden bijgestaan 

door lokale artsen en verpleegkundigen, zetten zich hiervoor 

kosteloos tijdens hun vakanties in. 

Zij bieden hulp door: 

• het uitvoeren van kosteloze reconstructieve operaties 

bij kinderen en (jong) volwassenen met lip- en gehemeltespleten 

(schisis), brandwondverminkingen, tumoren en 

andere aangeboren en/of verworven (bijvoorbeeld oorlogsslachtoffers) 

afwijkingen 

• het opleiden van lokaal medisch personeel in de 

reconstructieve chirurgie, anesthesiologie en verpleegkundige 

technieken 

• te ondersteunen bij het opzetten van brandwondafdelingen 

en afdelingen voor reconstructieve chirurgie 

binnen de bestaande ziekenhuizen en ondersteuning te 

bieden bij het organiseren van nationale brandwondpreventie 

campagnes 

Om duurzame resultaten te bereiken, wordt intensief 

samengewerkt met lokale ziekenhuizen, artsen, verpleegkundigen, 

universiteiten en hulporganisaties. 

Om de lokale ziekenhuizen zoveel mogelijk te ontlasten 

worden alle materialen die nodig zijn voor circa 100 tot 120 

operaties meegenomen vanuit Nederland. 

Interplast is een internationale organisatie die in 1969 in de 

Verenigde Staten ontstond. In 1990 werd Stichting Interplast 

Holland opgericht. In vele andere landen is Interplast 

inmiddels vertegenwoordigd. Interplast Holland is als 

stichting volledig onafhankelijk van haar zusterorganisaties. 

Interplast Holland richt zich momenteel voornamelijk op 

Zanzibar, Burundi, Nigeria, Rwanda, Oeganda en Guinee- 

Bissau. Naar deze landen vinden jaarlijks missies plaats. 

In het verleden werden teams uitgezonden naar Indonesië, 

Vietnam, Pakistan, India, Bhutan, Tibet en Libanon. 

Introduction 

Stichting (Foundation) Interplast Holland is a charitable 

organisation that performs reconstructive surgery for 

children, young people and adults in developing 

countries. Each year the organisation sends teams 

of experienced plastic and reconstructive surgeons, 

anaesthesiologists and operating theatre assistants 

on a number of missions to developing countries. 

The doctors and assistants, who are assisted by local 

doctors and nurses, travel during their holidays and 

provide their services free of charge. Their assistance 

extends to: 

• performing reconstructive operations free of 

charge for children and adults with cleft lips or palates, 

mutilations caused by burns, tumours and other 

congenital skin defects 

• training local medical personnel in reconstructive 

surgery, anaesthesia and nursing techniques 

• helping to set up burns and reconstructive surgery 

units in hospitals 

To secure lasting results Interplast Holland works 

intensively with local hospitals, doctors, nurses, 

universities and NGOs. In order not to burden local 

hospitals unduly, all materials required to perform 

100-120 operations are flown in from The Netherlands. 

Interplast is an international organization which 

was founded in the United States in 1969. Stichting 

Interplast Holland was founded in 1990. By now 

Interplast is represented in many other countries. 

As foundation Interplast Holland is fully independent 

from its fellow Interplast organizations. They are 

regarded as sister organizations. 

Stichting Interplast Holland currently concentrates 

its activities in Zanzibar, Burundi, Nigeria, Rwanda, 

Uganda and Guinea-Bissau. Missions to these 

countries take place every year. 

In the past teams have also been sent to Vietnam, 

Pakistan, India, Bhutan, Tibet and Lebanon. 


Teamuitzendingen 

In 2010 vonden acht reguliere Interplast missies plaats: 

twee naar Nigeria, één naar Guinee-Bissau, één naar 

Zanzibar, twee naar Burundi en één naar Rwanda. 

Een achtste missie had een speciaal karakter; in samen - 

wer king met het Trust Fund for Victims (van het Interna tional 

Criminal Court) werd voor de derde keer een missie 

georga niseerd naar het noorden van Oeganda. Deze keer 

werd er in Kitgum gewerkt! 

De twee geplande missies naar Jemen werden wegens 

de verslechterde veiligheidssituatie geannuleerd. 

Uganda Burns and Plastic Surgery Institute (UBPSI) 

De Burns Unit van het UBPSI wordt nog steeds ondersteund 

door Interplast Holland. 

Na 6 jaar is veel bereikt en de zorg voor de brandwondpatiënten 

geoptimaliseerd. Nu ligt de focus met name op 

de preventie van brandwonden. In samenwerking met de 

Nederlandse Brandwonden Stichting wordt hiervoor een 

programma ontwikkeld. Dit preventieprogramma wordt in 

Oeganda gesteund door het Ministerie van Gezondheid. 

Ook werd in 2010 een nieuwe techniek (Meek) geïntroduceerd 

voor huidtransplantatie en is onderzoek gedaan 

naar de mogelijkheden een Huidbank (Amnion-bank) op 

te richten in Oeganda. De Euro Skin Bank in Beverwijk 

is bereid hierbij te helpen. Dit krijgt zijn vervolg in 2011. 

Visie 

Stichting Interplast Holland beoogt een wereld 

waarin ook gehandicapte kinderen en volwassenen 

in ontwikkelingslanden voldoende kansen krijgen 

fysiek en geestelijk volledig te participeren in een 

gezonde en rechtvaardige samenleving. 

Missie 

Stichting Interplast Holland is een ideële 

organisatie, die de ontwikkeling op het 

gebied van de reconstructieve chirurgie 

en de brandwondenzorg in ontwikkelingslanden 

bevordert en daar zelf in participeert. 

Inleiding 

Team missions 

Eight regular Interplast missions took place in 2010: two 

to Nigeria, one to Guinea-Bissau, one to Zanzibar, two to 

Burundi and one to Rwanda. 

The eighth was special in that it was carried out in conjunction 

with the Trust Fund for Victims of the International 

Criminal Court and was the third mission of its kind to be 

sent to the north of Uganda. This time the team worked in 

Kitgum. 

The two missions to Yemen were cancelled due to the 

deteriorating security situation. 

Uganda Burns and Plastic Surgery Institute (UBPSI) 

The Burns Unit of the UBPSI is still supported by Interplast 

Holland. 

After six years much has been achieved and the care of 

burns patients has been optimised. Now the focus lies 

particularly on preventing burns injuries. A programme for 

this is being developed together with the Dutch Burns 

Foundation. The prevention programme also has the 

support of the Health Ministry in Uganda. 

2010 saw the introduction of a new technique (Meek) for 

skin transplants and an investigation was carried out into 

the possibilities of setting up a tissue or amnion bank in 

Uganda. The Euro Skin Bank in Beverwijk is prepared to 

help with this. These plans will be followed up in 2011. 

Visionary goal 

Interplast Holland wants to see a world in which 

handicapped children and adults in developing 

countries have sufficient opportunities to 

participate physically and mentally in a healthy 

and just society.

GUINEE-BISSAU is een klein West-Afrikaans land met een 

geschat aantal inwoners van 1,5 miljoen. Guinee-Bissau 

is een van de minst ontwikkelde landen ter wereld, en 

behoort tevens tot de armste. Op de Human Development 

Index van de Verenigde Naties staat Guinee-Bissau 

op de 164ste plaats (van de 169. N.B. Nederland staat 

op nummer 7). 

De gemiddelde levens ver wach ting is 48 jaar, en bijna 

100 kinderen op de 1000 sterven in hun eerste levensjaar. 

Het land leeft voornamelijk van de landbouw en is de 

laatste jaren het grootste exportland ter wereld geworden 

voor cashewnoten.

Na twee eerdere missies in 2008 en 2009 volgde een 

derde uitzending in februari van 2010. Het lokale organisatie 

comité: Consul Jan van Maanen, zijn echtgenote 

Will, en Johannes Mooij hadden alles ook nu weer prima 

voorbereid. 

Op twee tafels opereren 

Uit evaluaties na de twee voorgaande missies bleek de 

nazorg van de door Interplast geopereerde patiënten 

onvoldoende.

GUINEA-BISSAU is a small West African country with 

an estimated population of 1.5 million. It is one of the 

least developed countries in the world and among the 

poorest, listed number 164 out of 169 on the United 

Nation’s Human Development Index. The Netherlands, 

for instance, is number 7. 

Average life expectancy is 48 years and infant mortality 

in the first year is almost 100 in every 1,000. 

The country lives primarily from agriculture and has 

recently become the biggest exporter of cashew nuts 

worldwide.

Visited hospital Hospital Nacional Simão Mendes, Bissau 

After two earlier missions in 2008 and 2009 a third 

mission was despatched in February 2010. The local 

organising committee, Consul Jan van Maanen, his wife 

Will, and Johannes Mooij, had again made excellent 

preparations for the team’s arrival. 

Operating on two tables 

Evaluations of two previous missions had revealed that 

the aftercare of patients operated by Interplast was 

insufficient. 


Guinee-Bissau 

Burn contracture elbow shortly after operation After one year 

Chirurg in opleiding Charlotte Lameijer werd speciaal daar- 

om aan het team toegevoegd en is twee weken langer in 

Guinee- Bissau gebleven. Charlotte had al enige chirurgische 

ervaring en bleek al snel een zeer waardevolle aanvulling 

te zijn voor plastisch chirurg Rein Zeeman. Zo kon 

er iedere dag op twee tafels geopereerd worden – dé 

Interplast manier! Charlotte heeft een boeiend verslag 

geschreven over haar verblijf, hier volgen enkele passages. 

Vrijdag 29 Januari, 9 uur 

Daar staat het doorgewinterde team van Interplast op 

Schiphol; Rein Zeeman, Gijs Witte, Paula Ellen en Elly 

Hofstee. Daar voeg ik, de nieuwe aanwinst van het team, 

me bij. Zonder veel problemen worden alle koffers ingecheckt 

en stappen we in het vliegtuig. Op Lissabon hebben 

we een halve dag wachttijd. De laatste vlucht naar Guinee- 

Bissau verloopt ook voorspoedig en eenmaal daar rond 

03.00 worden we opgewacht door Jan van Maanen, de 

Nederlandse consul, die ons via de VIP-ruimte naar ons 

hotel loodst zodat we van onze broodnodige slaap kunnen 

gaan genieten. 

Surgeon in training Charlotte Lameijer was therefore 

deliberately added to the team and stayed two weeks 

longer in Guinea-Bissau. Charlotte already had some 

surgery experience and rapidly proved to be a very 

valuable assistant to plastic surgeon Rein Zeeman. 

That meant that the team could operate on two tables 

every day, the Interplast way! Charlotte has written a 

fascinating account of her stay, which is quoted below. 

Friday 29th of January, 9 am 

There they are. The seasoned Interplast team; Rein 

Zeeman, Gijs Witte, Paula Ellen and Elly Hofstee. I am 

the new acquisition coming to join them. All the luggage 

is checked in fairly easily and we step on board. We have 

half a day’s wait in Lisbon. The last flight to Guinea- 

Bissau goes well and having arrived around 3 am we’re 

met by Jan van Maanen, the Netherlands consul, who 

pilots us through the VIP room to our hotel so we can 

enjoy some desperately needed sleep.

Bilateral cleft lip One year after operation 

Zaterdag 30 Januari, 10 uur 

Op naar het ziekenhuis Simão Mendes, hier zie ik erg naar 

uit, want ik ben benieuwd wat ons te wachten staat. Het 

ziekenhuisterrein bestaat uit verschillende gebouwen waarin 

alle verpleegafdelingen, de apotheek, de poliklinieken en de 

operatiekamers gevestigd zijn. Ook het kantoor van de welbekende 

Johannes Mooij is te vinden in één van de gebouwtjes. 

Hier begint onze dag waar al een heleboel patiënten 

op ons staan te wachten. We zijn de hele ochtend bezig met 

het screenen van de patiënten die na een gedegen voorselectie 

van Jan van Maanen en zijn vrouw Will zich hebben 

gemeld. We worden bijgestaan met de uitgebreide Creoolse 

woorden schat en grapjes van Samora, wereld beroemd in 

heel Nederland en Guinee-Bissau. ’s Middags wordt dan 

door Elly en Paula onder leiding van teamleader Rein het 

OK-programma voor de komende week gepland. 

Zondag 31 januari 

’s Ochtends ruimen we de OK in. Dit betekent voor de 

teamleden die hier vorig jaar waren (iedereen, behalve ik) 

een feest van herkenning met de werknemers; Fatima 

(OK-assi stent), Tenè (OK-assistent), Sana (schoonmaker) 

en dr. Maimuna Baldé (aangezichtschirurge). Het klaar - 

maken van de operatiekamers betekent voor elk van het 

team iets heel anders. Elly heeft de leiding over het ordenen 

van de anesthesiematerialen. Gijs zorgt onder andere voor 

het professioneel aansluiten van de O2-slangen met duck 

tape. 

Saturday 30th of January 2010, 10 am 

We’re on our way to the Simão Mendes hospital which I’m 

really looking forward to because I’m curious to know what’s 

in store for us. The hospital grounds comprise different 

buildings accommodating all the nursing wards, the 

pharmacy, the outpatients departments and the operating 

theatres. The office of the famous Johannes Mooij can be 

found in one of the small buildings as well. This is where our 

day begins and there are already a lot of patients waiting for 

us. We spend the entire morning screening the patients who 

have come to the hospital after a thorough pre-selection by 

Jan van Maanen and his wife Will. Samora, with his 

extensive Creole vocabulary and jokes, and world famous 

throughout the Netherlands and Guinea-Bissau, assists us. 

In the afternoon Elly and Paula plan the operating theatre 

programme for the coming week supervised by the team 

leader. 

Sunday 31 st of January 

In the morning we fit out the operating theatre. For the 

team members who were there the previous year (everyone, 

except me) there is the pleasure of seeing familiar faces 

again; Fatima and Tenè (operating theatre assistants), Sana 

( the cleaner) and Dr Maimuna Baldé (plastic surgeon). 

Getting the operating theatres ready means something 

different for each member of the team. Elly is in charge of 

sorting out the anaesthesia materials. Gijs is responsible for 

professionally connecting up the oxygen tubes with duck

Guinee-Bissau 

Paula zorgt voor orde in de chaos van alle dozen met 

materialen die nodig zijn voor de OK’s en Rein controleert 

de twee OK-tafels en de OK-lampen (waarvan maar 3 van 

de 12 lampen het lijken te doen) en gaat aan het klussen 

met een van de lokale medewerkers om alles optimaal 

functionerend te krijgen. Ik kijk mijn ogen uit en probeer 

te helpen waar nodig in het goed geoliede team. 

Maandag 1 februari t/m vrijdag 12 februari 

Maandagochtend gaan we dan echt aan de slag. Rein en 

ik beginnen samen te opereren, waarbij ik assisteer. Maar 

er staan niet voor niets twee operatietafels in één operatiekamer. 

Dus al gauw beginnen we de patiënten te verdelen. 

Tijdens de operaties zorgt Paula ongemerkt dat alles zo 

vlekkeloos mogelijk verloopt. Elly en Gijs zijn een team dat 

elkaar prima aanvult, waardoor de anesthesie over het 

algemeen fantastisch verloopt. 

Tijdens ons verblijf worden we uitgenodigd om een radio- 

presentatie te geven op een nationale radiozender over 

onze activiteiten in Guinee-Bissau. Hier ga ik samen met 

Rein en Samora heen; we vertellen dat we ontzettend 

goed geholpen worden, maar geven ook aan dat de zorg 

in Guinee-Bissau geoptimaliseerd kan en moet worden. 

In de krant van Guinee-Bissau verschijnt een artikel over 

Interplast en onze activiteiten. Ook zijn we op het nationale 

nieuws van Guinee-Bissau te zien, terwijl we in actie zijn in 

de operatiekamers en treedt Jan op als woordvoerder, 

waar bij hij aandacht vraagt voor (ouders van) patiëntjes die 

niet geopereerd willen worden, in de hoop dat ze een visa 

krijgen om naar Portugal te gaan om daar de benodigde 

chirurgische zorg te krijgen. Het vervelende van deze uitzendingen 

is dat het vaak gebeurt dat ouders/verzorgers 

van deze kinderen de visa zien als mogelijkheid op een 

beter leven en vaak verdwijnen bij aankomst in Portugal, 

de kinderen worden dan aan hun lot overgelaten. Het doel 

van Interplast, en het doel dat elke internationale medische 

missie naar mijn mening zou moeten hebben, is de lokale 

medische zorg te ondersteunen, maar voornamelijk te 

zorgen voor onderwijs zodat de zorg geboden kan worden 

door lokaal medisch personeel en de uitzendingen beschreven 

hierboven niet nodig zijn. 

In de twee weken dat het voltallige Interplast team aanwezig 

was hebben we 67 patiënten geopereerd waar nog een boel 

nazorg na deze twee weken nodig is. Dit is de reden dat bij 

deze missie ervoor is gekozen om iemand achter te laten, 

wat helemaal geen straf is in dit gezellige land. En diegene 

ben ik, Charlotte.

working, and sets to work doing repairs with one of the 

local members of staff to make sure that everything is 

functioning as it should. I’m all eyes and try to help 

where needed in the well-oiled team. 

Monday 1 st of February to Friday 12 th of February 

Monday morning we set to work proper. Rein and I begin 

to operate together with me assisting. But there’s a good 

reason that there are two operating tables in one 

operating theatre, so quite soon we begin to divide the 

patients. During the operations Paula unobtrusively 

makes sure that everything goes as smoothly as 

possible. Elly and Gijs are a team that complement one 

another really well so that the anaesthesia side of things 

proceeds without a hitch on the whole. 

During our stay we are asked to give a radio presentation 

about our activities in Guinea-Bissau on a national radio 

station. I go along with Rein and Samora; we explain how 

much help we get but also that care in Guinea-Bissau 

needs to be improved. The newspaper in Guinea-Bissau 

publishes an article about Interplast and the work we do. 

We’re also on the national news, showing us in action in 

the operating theatres. Jan acts as spokesperson and 

draws attention to (the parents of) young patients who 

don’t want to be operated on, in the hope that they will 

be given a visa to go to Portugal to receive the necessary 

surgery. The irritating thing is that often the parents or 

guardians of these children see the visa as a way of 

finding a better life and disappear as soon as they arrive 

in Portugal, leaving the children to their fate. Interplast’s 

aim, and that of every international medical mission, 

I think, should be to support local medical care but 

primarily to ensure that people are educated so that 

the care can be offered by local medical staff and the 

missions described above become superfluous. 

In the two weeks that the full team was present we 

operated on 67 patients for whom a great deal of after 

care would be needed in the following fortnight. This 

is why we decided on this mission to leave someone 

behind, which is no hardship in this hospitable country.

Zaterdag 13 februari t/m vrijdag 26 februari 

Deze twee weken heb ik de maandagen, woensdagen en 

vrijdagen gepland voor poliklinische controles van de 

patiënten die geopereerd zijn in de eerste twee weken. 

Omdat Samora weer aan het werk is bij Plan in Bufata, moet 

ik mezelf redden in het Creools. Dit gaat met handen en 

voeten toch redelijk. 

Tijdens deze laatste twee weken verwijder ik alle aangebrachte 

K-draden (metalen pennetjes voor fixatie van 

ledematen), instrueer ik ouders/verzorgers/patiënt en 

verpleging over de nazorg en de benodigde oefeningen 

ter bevordering van de functionaliteit. 

Conclusies 

De missie van Interplast in Guinee-Bissau 2010 lijkt weer 

een succes te zijn. Ook het feit dat coördinatie van de 

nazorg is bereikt doordat iemand langer blijft dan het gehele 

Interplast team en zo lokaal medisch personeel kan instrueren 

is een positieve ontwikkeling. Het lokale medische personeel 

heeft geuit dit als erg prettig te hebben ervaren. Door 

de laatste twee weken de wondproblemen te hebben gezien, 

geloof ik dat het belangrijk is laagdrempelig pre- en postoperatief 

te behandelen met antibiotica. 

Persoonlijk heb ik enorm genoten van deze missie; het team 

was fantastisch, ik heb erg veel geleerd op een constructieve 

manier. Ook ben ik ervan overtuigd dat de medische zorg die 

Interplast levert in deze ontwikkelings landen op een goede 

manier is opgebouwd; ondersteunend, maar vooral gericht 

op onderwijs van lokale medici, zodat deze in de toekomst 

zelf de chirurgische zorg kunnen leveren die Interplast nu 

levert. Ik zie alweer uit naar een volgende missie! 

Saturday 13th of February to Friday 26th 

of February 

I planned Mondays, Wednesdays and Fridays in this fortnight 

for outpatient check ups on the patients that were 

operated on in the first two weeks. Since Samora had 

gone back to work at Plan in Bufata, I had to mange as 

best I could in Creole. With a bit of gesticulating I coped 

reasonably well. 

In the last two weeks I removed all the Kirschner or 

K wires, provided parents/carers/patients and nursing 

staff with instructions about the aftercare and the requisite 

exercises needed to advance functionality. 

Conclusions 

The Interplast mission to GuineaBissau 2010 would again 

appear to have been a success. It was also a good thing 

that the aftercare could be coordinated by someone who 

stayed on longer than the whole team who was able to 

instruct local staff. The local medical staff said that they 

were very pleased with this arrangement. Having seen 

the problems with the wounds in the last two weeks I think 

it is important for there to be low threshold pre and post 

operative treatment with antibiotics. 

I personally thoroughly enjoyed the mission; the team 

was fantastic and I learned a lot in a constructive way. 

I am convinced as well that the medical care provided by 

Interplast in these developing countries is well structured; 

supportive but above all focussed on teaching local medics 

so that in the future they themselves will be able to provide 

the surgical care that Interplast now delivers. I’m looking 

forward to a next mission!

chirurgie. 

Voor de komst van Interplast Holland werden patiënten 

soms doorverwezen naar een kliniek voor reconstructieve 

chirurgie in Dar es Salaam op het vasteland: een kostbare 

zaak! De teams van Interplast zijn dan ook erg welkom op 

Zanzibar, waar ze sinds 2008 werken in het Mnazi Mmoja 

Hospital, een overheidsziekenhuis met 430 bedden en 

diverse specialismen.

Ook dit jaar lagen de voorbereidingen en organisatie in 

het Mnazi Mmoja Hospital in de capabele handen van 

‘Dr. Rama’ (Ramadhan). Hij houdt het hele jaar door 

contact met Interplast en stuurt plastisch chirurg Ed 

Hartman foto’s ter consultatie. 

Deze keer had Dr. Rama in totaal 97 patiënten geselecteerd, 

twee keer zoveel als het jaar daarvoor. Daarvan 

vielen er 19 af tijdens de screening door het team van 

Interplast; sommige van deze kinderen waren nu nog te 

jong voor een operatie, maar komen volgend jaar zeker 

in aanmerking.

The island of ZANZIBAR lies off the east coast of Africa 

and is a semi-autonomous part of Tanzania. The island 

of Pemba close by is often considered to be part of 

Zanzibar. 

In this part of the African continent there is no plastic 

reconstructive surgery. 

Before the arrival of Interplast Holland patients were 

sometimes referred to a clinic for reconstructive surgery in 

Dar es Salaam on the mainland, which was a very 

expensive exercise. So the Interplast teams are very 

welcome on Zanzibar, where they have been working since 

2008 in the Mnazi Mmoja Hospital, a government hospital 

with 430 beds and a range of specialists.

This year, too, the preparations and the organisational 

aspects in the Mnazi Mmoja Hospital were in the capable 

hands of Dr Rama (Ramadhan). He keeps in touch with 

Interplast throughout the year and sends plastic surgeon 

Ed Hartman photographs so he can consult with him on 

cases. 

This time Dr Rama has selected 97 patients altogether, 

twice as many as last year. Nineteen of them were taken 

off the list during the screening by the Interplast team. 

Some of the children are still too young for an operation 

but will certainly be eligible next year.

Het was goed dat Principal Secretary of the Minister 

of Health Dr. Mohamed Jiddawi meekeek tijdens het 

screenen van de patiënten. Als voor malig uroloog zag 

hij duidelijk dat er wederom vele jonge tjes met urethrale 

problemen bij waren. Daarvan had ongeveer de helft een 

echte hypospadie (aangeboren afwijking aan de plasbuis), 

maar de andere helft had vooral problemen (fistels) door 

complicaties na een verkeerd uitgevoerde circum cisie. 

De kans is nu groot dat er een speciale voorlichtingscampagne 

komt!

It was a good thing that the Principal Secretary of the 

Minister of Health Dr Mohamed Jiddawi looked on during 

the screening of the patients. As former urologist he saw 

quite clearly that again a lot of young boys presented 

with urethral problems. Approximately half of them had 

hypospadias, a congenital defect in which the urethra 

opens inferior to its normal location but the other half 

suffered primarily from fistulas arising from complications 

after badly performed circumcisions. There is now a big 

chance that there will be a publicity campaign!

Despite huge oil revenues there is great poverty in the 

West African country of NIGERIA. Seven hundred babies 

less than a month old die daily. The country is far behind 

others in terms of health care. The COCIN Hospital & 

Rehabilitation Centre is situated in Mangu on the Jos 

Plateau in Northern Nigeria. This hospital, formerly 

belonging to the Nederlandse Leprastichting or Netherlands 

Leprosy Relief (NLR), has been visited twice a year since 

2007 by a team mainly comprising plastic surgeons and 

assistents from the north of the Netherlands.

It was touch and go as to whether the mission could go 

ahead! Unrest among cattle farmers and landowners was 

leading to heavy fighting and dozens of fatalities only 80 

kilometres from Mangu. Interplast consulted closely with the 

Netherlands embassy in Abuja on the security situation;

eindelijk vertrekken! Inderdaad bereikte het team veilig 

Mangu, mede dankzij vaste begeleider Kefas, die is opgeleid 

tot verpleegkundige maar inmiddels alles doet: management, 

financiën, gids, zaalarts en operateur. 

Volgende keer zeker beter 

Grote vorderingen zijn mogelijk in Nigeria. Het is mooi om te 

zien dat een eens zo klein instituut, dat begon als leprazieken 

huis, is uitgegroeid tot een multidisciplinair ziekenhuis. 

Er is een oogcentrum gekomen, extra verpleegunits en een 

HIV- centrum. Assistent-plastisch chirurg Chantal Moues, 

voor het eerst mee, schrijft er beeldend over in haar verslag 

(zie www.interplastholland.nl). Wel is het OK-complex nodig 

aan vernieuwing toe; de plannen hiervoor zijn al verge vorderd. 

Ook zullen we de Nederlandse ambassade steun vragen 

voor de aanschaf van nieuwe sterilisatieapparatuur; dan 

hoeven we de volgende keer geen instrumenten meer te 

steriliseren in een pan water op een open vuur... 

Vele mensen geopereerd 

Tijdens de eerste screening, samen met Kefas, werden 59 

patiënten geselecteerd voor operatief ingrijpen. In de tweede 

week kwamen daar nog eens 18 patiënten bij, die bijvoorbeeld 

van ver kwamen en zich pas later konden melden. 

In totaal werden nu 77 patiënten geopereerd, waarvan 9 

met schisis, 44 met brandwondcontracturen en 24 met 

overige aandoeningen.

depart! The team did indeed reach Mangu safely, partly 

thanks to the regular guide Kefas who has been trained 

as a nurse but is meanwhile into management and finance 

and acts as guide, ward doctor and operator. 

Certainly better next time 

Major steps forward are possible in Nigeria. It is wonderful 

to see how a once small leprosy institute has expanded to 

become a multidisciplinary hospital. There is an eye clinic, 

additional nursing units and an HIV centre. Assistant plastic 

surgeon Chantal Moues, joining the team for the first time, 

wrote an evocative report on the mission that can be found 

on our site www.interplastholland.nl. An extract in English is 

included below. The operating theatre complex badly needs 

renovating but the plans for this are at an advanced stage. 

We will also be asking the Netherlands embassy for help to 

purchase new sterilising equipment that would save us having 

to sterilise instruments in a pan of water on an open fire...

During the first screening, together with Kefas, 59 patients 

were selected for an operation. In the second week another 

18 patients were added. They had come from a great distance 

and were only able to report later. Altogether 77 patients were 

operated, 9 with clefts, 44 with burn contractures and 24 with 

other disorders.

Een stukje uit Chantals verslag 

Chantal schrijft: ‘de dagen vlogen voorbij! Er werd hard 

gewerkt en geen moment onbenut gelaten om zoveel mogelijk 

patiënten te helpen. De mentaliteit van de Nigerianen is 

geweldig, ondanks al de armoede zijn ze dankbaar voor wat 

zij hebben. We voelden ons welkom en beschermd in deze 

roerige tijd. Er zit duidelijk een opwaartse lijn in de ontwikkelingen 

rondom het Cocin-ziekenhuis, dat gedragen wordt 

door lokale artsen en verpleegkundigen, die met internationale 

hulp veel voor de ernstig verbrande patiënten kunnen 

bereiken. Dit is hét antwoord op het algehele negatieve 

adagium dat heerst bij vele Nederlanders: “geen druppel op 

de gloeiende plaat, maar een hele golfbeweging!”.’

Chantal writes; ‘The days flew by! Everyone worked hard and 

not a moment was lost in trying to help as many patients as 

possible. The Nigerians’ attitude is tremendous, despite all 

the poverty they are grateful for what they have. We felt 

welcome and protected in these turbulent times. Clearly 

progress is being made in the developments regarding Cocin 

hospital that is manned by local doctors and nursing staff 

who, with international help, can achieve a lot for patients 

with serious burns. They are a perfect example that refutes 

the generally negative idea of a lot of Dutch people about 

giving help. Here, help is not just a drop in the ocean but a 

whole wave!’

De tweede missie 

Een relatief klein team reisde in oktober af naar Mangu om 

daar voor de tweede keer dit jaar te werken. In totaal werden 

200 patiënten gezien en 75 operaties uitgevoerd. Wederom 

was het grote aantal patiënten met brandwondcontracturen 

groot. Hier worden er 40 van geopereerd. Zoals in veel Afrikaanse 

landen is ook in Nigeria nauwelijks sprake van zorg 

voor brand wond patiënten. Gebrek aan kennis en de juiste 

middelen spelen hierbij zeker een rol. 

Tijdens de komende uitzendingen zullen onderwijs en lokale 

scholing meer op de voorgrond komen te staan. 

Naast de patiënten met brandwondcontracturen werden 

6 schisis-patientjes geopereerd, 7 urogenitale reconstructies 

uitgevoerd en 22 patiënten met overige aandoeningen. 

Dankzij de steun van de Nederlandse ambassade is de 

nieuwe sterilisatieapparatuur er gekomen! Het Interplastteam 

maakte daar direct dankbaar gebruik van. 

Nieuwe afdeling en oude bekende 

Na deze twee prima verlopen weken vertrok het team weer, 

met uitzondering van Cees en Neeltje Spronk. Zij bleven wat 

langer om de bouw van de nieuwe verpleegafdeling te begeleiden, 

evenals het bezoek van de nieuwe ambassadeur in 

Nigeria, Bert Ronhaar (voormalig ambassadeur in Jemen en 

daar al bekend met Interplast). 

Het ziekenhuis vraagt een bijdrage van de patiënten, maar 

omdat het hier vaak gaat om de armste laag van de bevolking 

draagt Stichting Faridpur (zie www.stichtingfaridpur.nl) 

50 euro per persoon bij. 

The second mission 

A relatively small team set off for Mangu in October to work 

there for the second time this year. Altogether the team saw 

200 patients and carried out 75 operations. Again there was 

a huge number of patients with burns contractures 40 of 

whom were operated on. As in many African countries there 

is no question of care for burns patients in Nigeria. Lack of 

knowledge and the right resources certainly play a role. 

Education and local training will be given greater priority in 

future missions. 

Besides the burns contracture patients, 6 young cleft 

patients were operated on, 7 urogenital reconstructions 

carried out and 22 patients with other disorders treated. 

Thanks to the support of the Netherlands embassy the new 

sterilisation equipment had arrived. The Interplast team 

made grateful use of this straightaway. 

New ward and familiar faces 

After two weeks that had gone smoothly the team left with 

the exception of Cees and Neeltje Spronk. They stayed on 

longer to supervise the building of the new nursing ward and 

to await the visit of the new ambassador in Nigeria, Bert 

Ronhaar, a former ambassador in Yemen so familiar with 

Interplast). 

The hospital asks patients to pay for their treatment but 

because in these cases they often come from the poorest 

populations group Stichting Faridpur helps by contributing 

50 euro per patient (http://www.stichtingfaridpur.nl/).

van Afrika. Door de burgeroorlog van 1994 tot 2005 is het

verbinding met zee. Medische zorg is maar voor een

BURUNDI is one of Africa’s most densely populated 

countries and the smallest. The civil war lasting from 

1994 tot 2005 has meant that it is also one of the world’s 

poorest. It is located in the great lakes region of Central 

Africa and is landlocked. Medical care is accessible to 

very few. Sixty-five out of 1,000 children die in their first 

year. There are fewer than 200 doctors for a population 

of around nine million.

Verbouwing! 

De derde missie naar Burundi stond in het teken van veel 

improviseren, en tóch veel opereren. Nog maar een week 

en het Interplast Holland-team zou in Burundi neerstrijken. 

Maar dan het bericht: in het Universiteitsziekenhuis (CHUK) 

in Bujumbura aldaar werden de operatiekamers nog verbouwd! 

Gelukkig kon uitgeweken worden naar Clinique 

Prince Louis Rwagasore. Voor Interplast toch een onbekend 

ziekenhuis, dus Rein Zeeman en André Nkeshimana 

vertrokken een dag eerder om alles voor te bereiden. De 

operatiekamer bleek wat klein en eenvoudig, maar er zou 

wel goed in te werken zijn. De volgende dag arriveerde de 

rest van het team en diezelfde middag nog was het door 

naar het ziekenhuis, om de patiënten te screenen. 

Uitstekende communicatie 

Minstens 200 mensen stonden er te wachten om gezien 

te worden door de plastisch chirurgen en anesthesioloog! 

De kinderen met schisis kregen voorrang en er werd een 

wachtlijst gemaakt voor een volgend bezoek. Goed dat het 

nieuwe teamlid Deo Mbonimpa erbij was: de communicatie 

met de familie en patiënten verliep nu veel beter dan voorheen. 

Bij het – samen met Els! – schoonmaken van de lipjes 

van de geopereerde kindjes gaf Deo de ouders meteen 

goede voorlichting en instructie. Het werd een prima week 

met vele, voornamelijk schisis-OK’s. 

Building renovation work 

The third mission to Burundi was all about improvising, and 

still managing to operate. Only a week to go before the 

Interplast Holland team were due to arrive in Burundi and 

a message arrived that building renovation work was still 

going on in the operating theatres in the University Hospital 

(CHUK) in Bujumbura. Fortunately the team was able to 

switch to the Clinique Prince Louis Rwagasore. For Interplast 

this was an unfamiliar hospital so Rein Zeeman and André 

Nkeshimana left a day earlier to prepare everything. The 

operating theatre proved to be small and rather rudimentary, 

but adequate for working in. The next day the rest of the 

team arrived and on the same afternoon went on to the 

hospital to screen patients. 

Excellent communication 

Two hundred people at least were waiting to be seen by the 

plastic surgeons and anaesthetist. Children with cleft lips and 

palates were given priority and a waiting list was made for a 

subsequent visit. It was a good thing that Deo Mbonimpa, the 

new team member, was there. Communication with the 

family and patients went much better than in the past. When 

Deo was cleaning the small lips of the children who had been 

operated on, together with Els, he was immediately able to 

tell the parents what they needed to do as well. It was a fine 

week, primarily operating on cleft lips and palates.

Tweede locatie: geleende anesthesiemachines 

Bij de tweede locatie, het Ngozi Hospital, stonden minstens 

400 mensen te wachten. Enkelen met een te gecom pliceer 

de problematiek voor Interplast tijdens dit bezoek. 

Ook hier kregen de kinderen met schisis voorrang en werd 

een wachtlijst gemaakt. Klaar voor de start, maar tijdens 

het voorbereiden van de OK bleken de anesthesie machines 

niet te werken, want bepaalde onderdelen ontbraken 

gewoon! Gelukkig wist de directeur van het ziekenhuis een 

oplossing: in een nabij gelegen health centre stonden twee 

machines die niet of maar weinig werden gebruikt. 

De lokale anesthesiste vond het prima om ze uit te lenen 

en kwam later in de week nog in de leer bij het anesthesieteam 

Rutger en Leo. Na wat improvisatie aan de machines 

kon het overvolle operatieprogramma toch nog op tijd 

beginnen. 

Enthousiaste en leergierige staf 

Zeker het vermelden waard is de goede sfeer waarin werd 

gewerkt. Deze was beslist ook te danken aan de lokale 

staf – zowel chirurgie als anesthesie – die enthousiast 

meewerkte en graag wilde leren. En zo werden in twee 

intensieve weken uiteindelijk 113 patiënten geopereerd, 

waarvan 84 met schisis. 

Transporting anaesthesia machines 

Second location: borrowed anaesthetic equipment 

At the second location, the Ngozi Hospital, at least 400 

people were waiting; a few cases were even too complicated 

for Interplast to handle during the visit. Here again 

children with clefts had priority and a waiting list was 

made. The team was ready to start but while preparing 

the operating theatre the anaesthetic equipment proved 

to be non-functioning. Some parts were just simply 

missing. Luckily the hospital director had a solution. 

There were two small, virtually unused, machines in a 

health centre close by. The local anaesthetist was quite 

happy to lend them and later in the week he joined the 

anaesthesia team of Rutger and Leo to brush up his 

skills. After improvising somewhat with the equipment 

the overloaded operating programme was able to start 

reasonably on time after all. 

Enthusiastic staff eager to learn 

The good working atmosphere is certainly worth 

mentioning. This was definitely attributable to the local 

staff, both surgeons and anaesthetists who worked 

enthusiastically with the team and were eager to learn. 

So in two extremely busy weeks 113 patients were 

ultimately operated on, 84 of them with clefts.

The morning after the operation at Ngozi

Najaarsmissie; voortaan meer ruchtbaarheid 

Helaas. Was de eerdere missie dit jaar in de Burundese 

hoofdstad Bujumbura zo’n succes, bij de tweede missie 

in september/oktober waren de zaken matig voorbereid. 

Zo stonden de materialen nog op het vliegveld en waren er 

bij de screening slechts tien mensen; wel wat anders dan de 

200 de vorige keer… Aan de komst van het Interplast-team 

was duidelijk te weinig ruchtbaarheid gegeven. Daarom 

werden in het weekend diverse journalisten en radiostations 

te woord gestaan en zo zagen we op maandag en dinsdag 

gelukkig veel meer patiënten bij de screening. 

Een dag eerder 

Overigens was het ook goed dat een kleine Interplast/Izeredelegatie 

al een dag eerder in Burundi was aangekomen. 

Deze kon nu een groot deel van de dag besteden aan het 

loskrijgen van de materialen bij de douane. Voor de bedoelde 

ontmoeting met de nieuwe minister van Gezondheid was 

hierdoor geen tijd, maar daarvoor bleek ook niet echt een 

vaste afspraak te staan… Sowieso was de lokale staf minder 

enthousiast en belangstellend dan tijdens onze eerste 

missie. Het team werkte dan ook veel alleen op de OK, met 

een licht ongemakkelijk gevoel. (Zie ook het verslag van

Meer energie op de tweede locatie: Ngozi 

Zo anders was het gelukkig in de tweede week op de tweede 

locatie, Ngozi! Het team werd hier uiterst vriendelijk ontvangen 

door de lokale chirurg dr. Augustin. Bij de screening 

stonden net zoveel mensen als tijdens de vorige keer, waarbij 

de mensen op de wachtlijst uiteraard voorrang kregen. 

Werkelijk een verademing voor het team, dat was de opvang 

en medewerking door de lokale staf in Ngozi! 

Autumn mission; more publicity in future 

What a pity. While the earlier mission this year to the 

Burundi capital Bujumbura was such a success, now 

preparations were poor. The medical materials were still 

at the airport and only ten people turned up for screening, 

a big difference with the 200 on the previous visit. 

Clearly little publicity had been given to the arrival of the 

Interplast team. So during the weekend various journalists 

and radio stations were spoken to and on Monday and 

Tuesday many more patients were fortunately there for 

the screening. 

A day earlier 

It was also a good thing that a small delegation had 

arrived a day earlier in Burundi because they were able 

to spend most of the day getting the medical materials 

released by customs. There was no time for the intended 

meeting with the new health Minister but it also turned 

out that no actual appointment had been made. In any 

case the local staff were less enthusiastic and interested 

than during our first mission. The team consequently 

worked alone in the operating theatre with a slightly 

uncomfortable feeling. 

More energy at the second location: Ngozi 

Happily things were different in the second week in the 

second location, Ngozi. The team were warmly welcomed 

by the local surgeon Dr Augustin. Just as many people 

turned up for the screening as the last time and, of course, 

those on the waiting list had priority. The welcome reception 

and the cooperation of the local staff in Ngozi were a relief 

for the team. Dr Augustin had drawn up his own programme 

for the week in the operating theatre next to Interplast’s.

Dr. Augustin had op de OK naast die van Interplast zijn 

eigen programma gemaakt voor de week. Zo kon hij 

gemakkelijk ook de assistentie van Rein Zeeman vragen 

bij een paar nogal ingewikkelde ingrepen. 

Enthousiaste reacties op presentatie 

Voor een enthousiaste groep van 80 verpleegkundigen, 

artsen en studenten gaf Rein Zeeman een presentatie over 

het werk van Interplast Holland, met name over het soort 

ingrepen en de resultaten daarvan. Men reageerde zeer 

enthousiast op de presentatie, die veel foto’s voor en na een 

operatie bevatte, waardoor resultaten zichtbaar zijn. 

Nazorg én belangrijk gesprek in Bujumbura 

Na een goede week in Ngozi reisde het team op vrijdag 

terug naar Bujumbura, waar in het ziekenhuis nog visite 

werd gelopen. Ook stonden veel patiënten te wachten op 

hun verbandwisseling. Wederom was er weinig ondersteuning 

door het lokale personeel. Het is daarom maar 

goed dat Deo twee weken langer bleef voor de nazorg! 

Bovendien kon het gesprek met de nieuwe minister van 

Gezondheid, Dr. Sabine Ntakarutimana, toch nog plaatsvinden, 

in het ministerie tegenover het ziekenhuis. In dit 

hartelijke onderhoud werd meer ondersteuning bij volgende 

missies toegezegd. 

So he could easily ask Rein Zeeman for assistance in the 

case of a few rather complicated procedures. 

Enthusiastic reactions to the presentation 

Rein Zeeman gave a presentation to an enthusiastic group 

of 80 nursing staff, doctors and students on the work of 

Interplast Holland, and in particular on the kind of operations 

and the results. The audience was extremely appreciative of 

the presentation that contained many before and after 

photos, making results immediately visible. 

After care and an important talk in Bujumbura 

After a good week in Ngozi the team returned to Bujumbura 

on Friday, where they did their rounds in the hospital. A lot of 

patients were waiting for their wounds to be dressed again. 

Once more there was little support from the local staff. It was 

a good thing that Deo stayed on two weeks longer for the 

aftercare. What’s more the talk with the new Health Minister 

Dr Sabine Ntakarutimana took place after all in the ministry 

opposite the hospital. During this friendly interview more 

support was promised for subsequent missions.

Deo was in 2010 nauw betrokken bij de twee Interplastmissies 

naar Burundi. Als Burundese Nederlander bleek hij 

van onschatbare waarde voor het team te zijn. Wij hebben 

Deo leren kennen als een humoristisch en toegewijd lid van 

het Interplast-team! Hij was een geduldig en voorkomend arts 

die graag een bijdrage leverde aan zijn Burundese landgenoten. 

Hij begreep als geen ander onze cultuur en die van 

zijn landgenoten. 

Als vluchteling uit Burundi, waar hij als arts en ziekenhuisdirecteur 

in het noorden werkzaam was, kwam hij in Zwitserland 

terecht. Hier werkte hij als assistent van een chirurg. 

Zijn familie vluchtte naar Nederland alwaar uiteinde lijk 

gezinshereniging plaats vond. 

Hij heeft nog getracht zijn artsenbul in Nederland te halen, 

maar met gezin en baan was dat niet haalbaar. 

Hij vond als operatieassistent in het Kennemer Gasthuis 

toch zijn draai. 

In Burundi was hij voor ons 

de dokter die hij daar altijd 

was geweest, en die daar ook 

zijn collega’s en vrienden van 

school weer tegen kwam. 

Voor de nazorg van de door 

ons geopereerde patiënten 

bleef hij twee weken langer. 

Hij gaf instructies en begeleidde 

de patiënten verder in 

Bujumbura en Ngozi. 

Een paar weken na terug- 

komst in Nederland werd Deo 

onwel tijdens zijn werk in het 

Kennemer Gasthuis. Hij kon 

niet meer gereanimeerd 

worden. 

We zullen Deo erg missen, 

als teamlid was hij warm 

en vol humor en ‘niets moet’ 

was een gevleugelde uitspraak 

van hem. 

Deo, we zullen je niet vergeten en we gaan door met ons 

werk in Burundi zonder jou. Je had het niet anders gewild!

Deo was closely involved in 2010 in two Interplast 

missions to Burundi. As Burundian Dutchman he was 

invaluable to the team. We got to know Deo as a 

humorous and devoted member of the Interplast team. 

He was a patient and considerate doctor who was 

happy to contribute to helping his fellow Burundi 

countrymen. He understood as no other our culture 

and that of his fellow Burundians. 

As a refugee from Burundi, where he had worked as 

hospital director and doctor in the north he ended up 

in Switzerland. There he worked as an assistant to a 

surgeon. His family sought refuge in the Netherlands 

where ultimately they were reunited. 

He tried to obtain his doctor’s diploma in the Nether lands 

but with a job and a family to support that was not a 

feasible option. In the end he found his niche working as 

an operating theatre assistant in the Kennemer Gasthuis. 

In Burundi he was the doctor 

who he had always been. 

There he met his colleagues 

and friends from school. 

He stayed on two weeks 

longer for the aftercare of 

the patients we had 

operated on. He gave 

instruction and guidance to 

the patients in Bujumbura 

and Ngozi. 

A few weeks after his return 

to the Netherlands Deo was 

taken ill while at work at the 

Kennemer Gasthuis. He 

could not be reanimated. 

We will badly miss Deo, as 

team member was he warm 

and full of humour. One of 

his favourite expressions 

was ‘there’s no such thing 

as must’. 

Deo, we will not forget you and we will go on with our work in 

Burundi despite your absence! You wouldn’t have wanted it 

otherwise!

Rwanda 

RWANDA, ‘land van de duizend heuvels’, is met 10 miljoen 

inwoners het meest dichtbevolkte land van Afrika. Door een 

groot tekort aan artsen – na de genocide van begin jaren 

negentig – was gezondheidszorg slechts voor een enkeling 

bereikbaar. Gelukkig brachten ingrijpende hervormingen en 

maatregelen de afgelopen jaren positieve ontwikkelingen. 

Zo is er een soort basisverzekering voor iedereen, hebben 

vele NGO’s (Non Governmental Organisation) nu hun weg 

naar Rwanda gevonden en worden veel artsen in Amerika 

opgeleid. 

Eerste locatie: plussen en min 

Bij deze derde missie naar Rwanda waren de zaken in het 

universiteitsziekenhuis in Kigali (CHUK) goed geregeld! 

Echt ieder jaar zijn duidelijke verbeteringen te zien; zo zijn 

de operatiekamers modern en volop in gebruik, met name 

op het gebied van orthopedie, urologie, gynaecologie, pediatrie 

en traumatologie. Tegenvallend voor dit doorgewinterde 

team, deze keer vergezeld door voorzitter en plastisch 

chirurg Rein Zeeman, was het aantal patiënten tijdens 

de screening. Dit ondanks doorverwijzing uit een ziekenhuis 

in Oost-Rwanda (via contacten met andere NGO’s). 

Weinig schisis, dankzij Operation Smile 

We zagen hier voornamelijk patiënten met brandwondcontracturen 

en andere defecten, en maar weinig patiënten 

met een lip- en/of gehemeltespleet (schisis).

‘The Land of the Thousand Hills’ as RWANDA is called 

is the most densely populated country in Africa with 

10 million inhabitants. The desperate shortage of doctors 

in the wake of the genocide in the early nineties meant 

that health care was available only to the happy few. 

Fortunately radial reforms and measures in past years 

have brought about positive developments. Now there 

is a kind of basic insurance for all and many NGOs have 

found their way to Rwanda and many doctors are being 

trained in America.

First location: pros and cons 

For the third mission to Rwanda things had again been well 

organised at the university hospital in Kigali (CHUK). Every 

year in fact there are clear improvements; the operating 

theatres are modern and in full use, notably in the field of 

orthopaedics, urology, gynaecology, paediatrics and traumato 

logy. A disappointment for the seasoned team, this time 

accompanied by the chairman and plastic surgeon Rein 

Zeeman, was the number of patients during the screening, 

despite referrals from a hospital in East Rwanda through 

contacts with other NGOs. 

Few cleft lips and palates thanks to Operation Smile 

Here we saw mainly patients with burns contractures 

and other defects and few patients with cleft lips and/or 

palates. 


Team in Kabgayi 

Rwanda 

Medeoorzaak hiervan is dat NGO Operation Smile (Zuid 

Afrika) zich sinds dit jaar ook in Rwanda geheel richt op 

schisis-patiëntjes. Goed werk van deze van oorsprong 

Amerikaanse en nu internationale organisatie! Zij opereerden 

in 2 weken 200 schisis-patiënten, maar vergaten lokale staf 

op te leiden. 

Huidtransplantaten aangeslagen 

Tijdens de eerste week werden in het CHUK 36 patiënten 

geopereerd. Veel brandwondcontracturen werden behandeld, 

evenals al maanden bestaande huiddefecten (meestal posttraumatisch); 

er werd een huidtransplantaat op aange bracht. 

Tijdens de verbandwissels drie dagen later bleek alles te zijn 

aangeslagen.Sommige patiënten die al maandenlang waren 

opgenomen konden binnen twee weken ineens het ziekenhuis 

verlaten… (Zie ook Adriaan de Blécourts verslag op 

www.interplastholland.nl.) 

Maanden van opname voorkomen 

Een probleem in het CHUK was het ontbreken van apparatuur 

om huidtransplanten af te nemen. Maar aan het eind van 

de missie kreeg CHUK-directeur dr. Theobald Hategekimana 

een mesh graft-apparaat en twee dermatomen (huid schaven) 

(in bruikleen) overhandigd. Voortaan kan men dus zelf 

huid trans plantaten afnemen en patiënten met brandwonden 

en andere huiddefecten sneller en beter behandelen, en zo 

dus maanden (soms zelfs jaren) van ziekenhuisopname 

voorkomen. 

Miscommunicatie op tweede locatie 

Op de tweede locatie, het Kabgayi-ziekenhuis in Gitarama, 

was eigenlijk niet veel voorbereid. 

This is certainly because of the NGO Operation Smile 

which has been totally concentrating on such patients since 

the beginning of the year. Good work from this international 

organisation that was originally an American initiative. They 

operated on 200 patients with cleft lips and/or palates in a 

fortnight, but forgot to train local staff. 

Skin grafts take 

During the first week at CHUK 36 patients were operated on. 

Many burns contractures were treated along with month-long 

skin defects, mostly post traumatic. Skin grafts were carried 

out all of which proved to have taken by the time the wounds 

were redressed three days later. Some patients who had 

been in hospital for months could suddenly go home within 

a fortnight. 

Preventing months of hospital stays 

CHUK’s problem was that it had no equipment to prepare 

skin transplants. But at the end of the mission CHUK 

director Dr Theobald Hategekimana was given a mesh 

grafting machine and two dermatomes on loan. Henceforth 

the hospital itself can harvest the skin for transplanting and 

thus help patients with burns and other skin defects faster 

and better, saving months and sometimes even years of 

stays in the hospital. 

Miscommunication at the second site 

At the second site, the Kabgayi hospital in Gitarama, 

very little had been prepared. Unfortunately there had been 

some miscommunication and people had not been informed 

in good time of the arrival of the Interplast team. In the end 

the team operated for only two days.

Er was sprake van miscommunicatie zodat men blijkbaar 

niet tijdig op de hoogte van de komst van het Interplast-team 

was… Uiteinde lijk heeft het team daar maar twee dagen 

geopereerd. 

Toch een succesvolle missie… 

In totaal zijn er 45 operaties verricht en vonden er 15 verband 

wisselingen onder narcose plaats. Ondanks het lagere 

aanbod van patiënten was het toch een geslaagde missie! 

…en afspraken gemaakt! 

Met Rwanda’s minister van Gezondheid, dr. Richard 

Sezibera, is onder meer gesproken over het – net als in 

Oeganda – eventueel helpen opzetten van een brand wonden 

unit in het CHUK. In Oeganda zouden artsen en verpleeg 

kundigen uit Rwanda goed opgeleid kunnen worden. 

Uiteraard zijn er ook nieuwe afspraken gemaakt over 

heldere communicatielijnen voor toekomstige missies!

Nevertheless, a successful mission… 

Altogether 45 operations were performed and 15 wound 

dressings under anaesthetic carried out. A successful 

mission after all despite there being fewer patients. 

… and new agreements reached! 

Interplast also talked to Rwanda’s Health Minister 

Dr Richard Sezibera, for one thing about the possibility 

of setting up a burns centre in the CHUK hospital like 

the one in Uganda. Doctors and nurses from Rwanda 

could be trained in Uganda. Naturally new agreements 

were made about clear lines of communication for future 

missions!

At 8:00 am the journey to Kitgum. The members of the team 

included Dr. Rein Zeeman (plastic surgeon and team leader), 

Dr. Floris de Graaf (plastic surgeon), Dr. Henriëtte Willigers 

(anesthesiologist), Michael Angel Longoleyang (theatre 

nurse) and Tert van Alphen (medical student from The 

Netherlands) bringing the number to five from the previous 

one of four, set off at the above mentioned time, and arrived 

in Gulu at 1 pm proceeded to Acholi Inn for lunch, then took 

off for Kitgum, arrived at 6 pm. 

Taken to the residential place where we were accommo dated. 

We had a shower, supper, and sleep. In the morning of 

the 7th of June we started with screening the patients at St. 

Joseph’s Hospital. After lunch in the afternoon we took our 

medical equipment and prepared the operation theatre. 

Tuesday, June the 8th till Saturday June the 12th 

They were five intensive working days. Together with the 

local team everyone had a role to play thanks to the team 

with the equipment we had. Sunday the 13th of June was 

our day off, but due to the bad road we couldn’t access our 

patients, included post burn contractures mostly (that had 

almost 90% of our surgeries: releases, Z-plasties, K-wires 

and so on). Speed, accuracy and dedication was the motto 

of the team. Everyone worked hard, but in between it was 

relaxed and evenings we had dinner together. 

In summary therefore 

The programme was a success. The number of patients seen 

was over 100 people in two weeks. The number of operations 

carried out was 67 in 8 days. Where our patients came from: 

Kitgum, Lira, Pader, Lamwo, and other neighbouring districts 

of Acholi. Team members who sourced the patients included 

Mr Jackson, and Mrs Judith from Aynet, Joyce and her 

colleague from AVSI. There was one patient for the max 

facial surgeons to review. 

We were privileged to be visited by Mr Scott Bartell and 

Maureen from the ICC Trust Fund (The Hague), who are 

responsible of field monitoring. Also in our visit was Astrid, 

Board Member of Uganda Burns and Plastic Surgery Institute 

and in charge of Norwegian Refugee Council in Kitgum. The 

St. Joseph’s team included Dr Moses Elaju in charge of 

Surgical Ward (Dept), Dr Matthew from Canada, Mrs Irene

Margaret Aciro in charge of the operation theatre. 

The reason of putting down this summary is to affirm how 

much dedication Stichting Interplast Holland is committed to 

improving the lives of the vulnerable people of Uganda, 

bringing Hope that may have been lost due to trauma, 

stigmatization, isolation, desolation to the community. 

Challenges 

This is shortage of sundries; sometimes essentials like 

K-wires of different sizes, lack of a motivated and a 

dedicated team within the local community. I propose they 

should be trained on how to handle post operative surgeries. 

Our surgeries included post ward round after theater, the 

patients who were operated within the first 5 days, showed a 

very great improvement. The grafts were very splendid and 

that gave us hope and encouragement. 

We could have done much more than the 67 patients, but the 

challenges remain: who will monitor these patients since we 

find that the hospital had only three medical officers, and a 

physiotherapist. Follow-up makes it difficult to monitor each 

patient who may need his or her physiotherapist. Despite all 

the above we encourage our friends to continue with the 

mission they started. I was very surprised to notice that none 

of the patients we operated could say ‘thank you’, but maybe 

they did in their heart.

It has been to me a very pleasurable experience, never seen 

how people can have such a huge heart for the impoverished 

people of the world. Hope this will prove those who may have 

never seen the other side of the world how important it is to

UBPSI in 2010

Na een succesvolle missie in Kitgum in Noord-Oeganda, 

verbleven wij nog 5 weken in Kampala om op de Burns 

Unit te werken. 

Allereerst willen we de verpleegkundigen van de Burns 

Unit complimenteren voor hun toewijding: al zijn ze met 

te weinig, ze geven de patiënten toch altijd de zorg die 

nodig is en zijn trots op hun werk en hun afdeling. 

Hieronder een uitweiding over de specifieke doelen van 

deze trip: 

1 Een begin maken van een brandwondenpreventieprogramma. 

2 Onderzoeken of oprichting van een Amnion-bank 

(huidbank) mogelijk is. 

3 Introductie van een unieke nieuwe mesh graft-techniek 

(MEEK mesher). 

4 Observeren van het functioneren van de Burns Unit. 

1 Brandwondenpreventieprogramma 

Het brandwondenpreventieprogramma wordt opgezet in 

samenwerking met de Nederlandse Brandwonden Stichting, 

die meer dan financiële ondersteuning wil geven. Zij wil 

namelijk ook trainingen verzorgen, voor belangstellende 

verpleegkundigen en voor wie streeft naar meer kennis en 

bewustzijn op het gebied van brandwonden (van vrouwengroepen 

tot brandweer en leraren). In het verleden 

organiseerden het UBPSI en Stichting Interplast Holland 

zogeheten Burn Awareness Week en symposia, maar of 

dit nou het gewenste effect op de doelgroep had… 

Het merendeel van de slachtoffers van brandwonden woont 

nu eenmaal in de sloppenwijken rondom Kampala (volgens 

de statistieken van de Burns Unit). 

In een gesprek over het brandwondenpreventieprogramma 

gaf de minister van Gezondheid Dr. Jacinto Amandua 

(Commissioner Clinical Services) aan het eens te zijn met 

de opzet van dat programma. Ook zegde hij zijn mede werking 

toe, inclusief de inzet van personeel van zijn ministerie. 

Het is de bedoeling een commissie samen te stellen, met 

daarin het ministerie van Gezondheid, het UBPSI en 

Stichting Interplast Holland (Nederlandse Brandwonden 

Stichting). 

Een van de grote eerste beginvragen is: hoe meet je de 

effectiviteit van een preventieprogramma? Immers, de 

Nederlandse Brandwonden Stichting wil eerst een nulmeting 

zien alvorens het programma goed te keuren. 

UBPSI in 2010

After a successful mission in Kitgum in North Uganda, we 

stayed five weeks together in Kampala to work in the Burns 

Unit. 

First of all we would like to compliment the nursing staff of 

the Burns Unit for their devotion: although there are too few 

of them, they always provide the patients with the care they 

need and they are proud of their work and ward. 

Below is a more detailed account of the specific aims of this 

trip: 

1 To make a start on a burns prevention programme. 

2 To investigate the possibility of setting up a Amnion bank. 

3 To introduce a unique new mesh graft technique. 

4 To observe how the Burns Unit functions. 

1 Burns prevention programme 

The burns prevention programme had been set up in 

collaboration with the Dutch Burns Foundation which 

wanted to give more than financial support alone. The 

idea is to provide training courses for nursing staff who 

are interested and for all those who are endeavouring to 

know more and become more aware in the domain of 

burns, ranging from women’s groups to fire service officers 

and teachers. In the past UBPSI and Stichting Interplast 

Holland organised Burn Awareness Weeks and symposia, 

but it is questionable whether these have had the desired 

effect on the target group. Most of the victims who suffer 

burns live in the slums around Kampala according to 

Burns Unit statistics. 

In a talk about the burns prevention programme the Health 

Minister and Commissioner of Clinical Services Dr Jacinto 

Amandua said that he agreed with the setting up of the 

programme. He also promised his cooperation, including 

the deployment of staff from his ministry. Now the job is to 

get together a commission combining the Health Ministry, 

the UBPSI and Stichting Interplast Holland (Dutch Burns 

Foundation). 

One of the first major questions was working out how to 

measure the effectiveness of a prevention programme. 

The fact is that the Dutch Burns Foundation wanted a 

zero measurement before it approved the programme. 

That inquiry had to be completed by the end of the visit. 

The first step was to have a questionnaire filled in by 

200 people living in the slums around Mulago. Thankfully 

the local leaders gave their full cooperation. What’s more

Dat onderzoek moest tijdens dit bezoek zijn afgerond. 

De eerste stap daartoe was het laten invullen van een 

vragenlijst door 200 mensen uit de sloppenwijken rondom 

Mulago. Het was fijn dat plaatselijke leiders daaraan goed 

meewerkten. Bovendien werden de vragen afgenomen 

door gemotiveerde mensen die voldoende kennis hebben 

over brandwonden en oorzaken van brandwonden. 

Wie eenmaal zo’n training heeft gekregen, moet informatie 

gaan geven op scholen (aan de leraren, die het weer doorgeven) 

en aan gemeenschappen ter plaatse. Meer details 

en precieze financiële zaken zijn nog te bespreken met 

de Nederlandse Brandwonden Stichting. Voor Stichting 

Interplast Holland zullen Rein Zeeman en Marjo Aerts dit 

brand wonden preventieprogramma leiden. Inmiddels heeft 

het UBPSI een aparte budgetlijn voor dit programma 

gemaakt. 

2 Amnion-bank 

De Euro Tissue Bank in Beverwijk is bereid mee te helpen 

aan de oprichting van een Amnion-bank in Oeganda. De 

praktische zaken hiervan zijn goed doorgesproken met 

Sr. Monica Walusimbi en Dr. R. Ssentongo, evenals met 

de minister van Gezondheid Dr. Jacinto Amandua (Com missioner 

Clinical Services). Hij zei dat er ook een cornea-bank 

(voor transplantatie van hoornvliezen) op stapel staat. Twee 

banken die te combineren zouden moeten zijn?

the basics for doing a training course on how to inform 

and advise people on burns prevention. 

People who have received such training go on to provide 

information to teachers in schools who can in turn pass 

this on, and to local communities. Further details and the 

exact financial matters still need to be discussed with 

the Dutch Burns Foundation. Marjo Aerts will be heading 

the burns prevention programme on behalf of Stichting 

Interplast Holland. Meanwhile UBPSI has earmarked a 

separate budget for this. 

2 Tissue bank 

The Euro Tissue Bank in Beverwijk is prepared to help in 

setting up a tissue bank in Uganda. The practical aspects 

of this have been thoroughly discussed with Sr Monica 

Walusimbi and Dr R. Ssentongo, as well as with the 

Health Minister and Commissioner of Clinical Services 

Dr Jacinto Amandua. He said that a cornea bank was 

also planned. 

The two banks could possibly be combined. We will be 

sending more information about amnion membranes and 

how these are used for burns patients. The project can 

start as soon as the Health Ministry gives the go ahead.

3 Nieuwe Mesh Graft-techniek 

Er werd een nieuw MEEK mesh graft-apparaat 

gedemonstreerd aan het betreffende medisch personeel 

(chirurgen, verpleegkundigen, assistenten). Vooral bij 

uitgebreide brandwonden is dit apparaat uitermate nuttig 

wanneer er te weinig donorplekken zijn. 

4 Jaarverslag van het UBPSI 

UBPSI 

Een compleet jaarverslag, inclusief financieel verslag, is van 

groot belang richting de donoren (die immers deze afdeling 

bekostigen). De gegevens voor het jaarverslag over

Brandwondentraining voor verpleegkundigen 

In de bestuursvergadering vorig jaar kreeg de heer 

dr. R. Ssentongo de vraag om samen met de Makerere 

University en het ministerie van Gezondheid een voorstel 

te schrijven voor een brandwondentraining voor verpleegkundigen, 

op basis van het al enkele jaren bestaande 

lesprogramma. Maar helaas zijn daar momenteel geen 

duidelijke vorde ringen te zien. 

3 New Mesh Graft technique 

A new MEEK mesh graft machine was demonstrated 

to the relevant medical staff (surgeons, nurses and 

assistants). The equipment is extremely useful in the 

case of extensive burns when patients have too few 

donor sites. 

4 UBPSI Annual Report 

A complete annual report, including a financial report 

is important for the donors, who after all fund this unit.

Burns training for nursing staff 

At the board meeting last year Dr R. Ssentongo was 

asked to gather data with Makerere University and the 

Health Ministry for a draft proposal for burns training 

for nursing staff on the basis of the present course 

which has been running for several years now. 

Unfortunately little progress has been made on this 

to date.

Stichting Interplast Holland 

[ Foundation] 

Interplast consists of volunteer medical personnel (plastic 

surgeons, anaesthesiologists, theatre and anaesthetic 

nurses and other specialists), who work free of charge during 

their holidays. Travel expenses, medical supplies and 

instruments are funded by donations raised in Holland from 

companies as well as the general public. 

Interplast teams provide reconstructive operations that 

transform the lives of children and (young) adults with 

physical disabilities and thereby improve the future of the 

whole family of those children as well. Interplast has no 

financial, political, racial or religious interest. 

The aim is to provide (and teach) reconstructive surgery to 

improve function, not to perform cosmetic surgery. 

Cooperation with local medical staff and working at existing 

hospitals close to the patients’ home is efficient and offers 

education in a specialist field for all involved. Apart from 

medical staff, local volunteers with social commitment are 

essential for the preparation and the smooth running of 

a successful Interplast mission. 

If you, your town, your hospital, or a charitable organisation 

you know think about hosting an Interplast team, here are 

some important issues to consider: 

• Advance notice for the team should be given at least 

6 months before the intended date. 

• Duration of the mission is usually 2 weeks, i.e. 

10 operating days. Depending on the severity of cases, 

about 100 patients can be operated during that time. 

• Size of the team varies depending on the number of 

operating tables, anaesthetic facilities and local staff 

available. On average, a team will consist of 6 people, 

2 surgeons, 1 anaesthesiologist, 2 theatre nurses and 

1 anaesthetic nurse, thus being able to run 2 operating 

tables (smaller or larger teams possible on request).

Local staff, i.e. doctors, nurses, interested volunteers 

are essential for the smooth running of a mission. 

Local doctors should perform the patient pre-selection 

during the months before the arrival of an Interplast team. If 

possible, they should inform the team about the type of 

surgery and special cases beforehand to allow appropriate 

planning of instruments and supplies for the trip. 

• The first day consists of screening and selecting the 

patients for the operating lists, unpacking equipment and 

setting up the operating room(s). 

• Types of operation: burns contractures, congenital 

deformities like cleft lip and palate, functional deficits or 

disfiguration from injury, infection (polio, leprosy, Noma etc.) 

tumours – in children and (young) adults. 

• Long working hours have to be anticipated by all involved 

to make an Interplast mission effective. 

• Apart from operating, ward rounds and change of dressing 

sessions take place every day. 

• The hospital should offer: two operating tables / 

anaesthetic machines, a recovery room, enough beds, 

electricity supply, water, normal saline for infusion, sheets 

and gowns, if possible oxygen, nitrous oxide, halothane, 

some dressing material and plaster of Paris. 

The Interplast team will provide: special instruments and 

medical equipment, suture material and special drugs and 

dressings. 

• You are requested to provide: (if possible, but if you don’t 

have the means, other arrangements can be discussed) 

basic, clean accommodation, food and transport for the team. 

Government / Ministry of Health permission for the mission; 

assistance with customs, excess baggage clearance etc. 

Many years of experience and thousands of 

grateful patients are proof of the success 

of Interplast activities. YOU can be part of it.
