(a) the name, address and any registration number of the intermediary;<br />
(b) the Member State or States in which the intermediary intends to operate;<br />
(c) the category of intermediary and, if applicable, the name of any insurance or<br />
reinsurance undertaking represented;<br />
(d) the relevant classes of insurance, if applicable;<br />
(e) demonstration of professional knowledge and ability.<br />
2. The competent authority of the home Member State shall, within one month of<br />
receiving the information referred to in paragraph 1, forward it to the competent<br />
authority of the host Member State, which shall acknowledge the receipt without<br />
delay. The home Member State shall inform the insurance or reinsurance<br />
intermediary in writing that the information has been received by the host Member<br />
State and that the insurance or reinsurance undertaking can commence its business in<br />
the host Member State.<br />
When receiving the information referred to in paragraph 1, the host Member State<br />
shall accept previous experience in insurance or reinsurance mediation activity, as<br />
demonstrated by proof of registration or declaration in the home Member State, as<br />
evidence of the required knowledge and ability.<br />
3. The proof of the previous registration or declaration shall be established by evidence<br />
of registration issued or declaration received by the competent authority or body of<br />
the home Member State of the applicant, which the latter shall submit in support of<br />
his application presented to the host Member State.<br />
4. In the event of a change in any of the particulars communicated in accordance with<br />
paragraph 1, the insurance or reinsurance intermediary shall give written notice of<br />
that change to the competent authority of the home Member State at least one month<br />
before implementing the change. The competent authority of the host Member State<br />
shall also be informed of that change by the competent authority of the home<br />
Member State as soon as is practicable and no later than one month from the date of<br />
receipt of the information by the competent authority of the home Member State.<br />
Article 6<br />
Exercise of the freedom of establishment<br />
1. Member States shall require any insurance or reinsurance intermediary who intends<br />
to exercise his freedom of establishment to establish a branch within the territory of<br />
another Member State first to notify the competent authority of his home Member<br />
State and to provide it with the following information:<br />
(a) the name, address and registration number (where applicable) of the<br />
intermediary;<br />
