The Grand Junction "Tiger Pride" Benefit Disc Golf Tournament
will be held Saturday October 13th, 2012 at Riverbend Park
in Palisade from 8am-1pm. Participant check-in begins at 8:00am.

Waiver of Liability: In consideration of the foregoing, I myself, my executors administrators
and assignees, do hereby release and discharge all sponsors, coordination groups, volunteers,
disc golf courses, parks organizations, host golf clubs, and any individuals associated with this
event, for all claims of damages, demands and actions whatsoever, arising or growing out of
my participation in this event. I recognize that participating in a competitive sport carries with
it risk of injury or death and hereby accept this risk. I also give any right to photos or likenesses
being used for media promotion or advertising from this sporting event.

sponsorship) and/or donations for drawings/prizes. Your business name/logo/flyer will be
displayed on a banner and at the hole that you sponsor. Proceeds will support music student
fees, competition expenses, instrument purchases, and uniform replacements.

Pre-Registration: The first 100 people to sign up and pay the $20.00 registration
fee are guaranteed a custom designed 1st Annual GJHS Discraft Disk (Z-plastic
driver or midrange.) Three ways to register and pay (cash or check only):
