POSITION TITLE: <strong>Transition</strong> <strong>Coordinator</strong><br />
REPORTS TO: Director of Special Education and Executive Director<br />
FUNDAMENTAL OBJECTIVES:<br />
Maintain a high quality transition program throughout the member school districts served<br />
by <strong>NW</strong><strong>BOCES</strong>.<br />
Prepare students with disabilities and their families for a successful transition into postschool<br />
activities, including post-secondary education, independent living, employment<br />
and/or community participation.<br />
PROFESSIONAL RESPONSIBILITIES:<br />
Provide leadership for implementing transition-focused planning in the high schools and<br />
middle schools served by <strong>NW</strong> <strong>BOCES</strong>.<br />
Develop and maintain a working relationship with businesses, agencies and organizations<br />
which provide post-secondary services for students with disabilities.<br />
Communicate effectively with students, families, community agencies, and school<br />
personnel.<br />
Educate transition personnel on current legal requirements for transition programs.<br />
Research and provide up-to-date information on assessments and curriculum materials<br />
available for transition programs.<br />
Attend IEP meetings when requested by parents and/or transition teachers to give<br />
definition to the <strong>Transition</strong> Planning.<br />
Develop and maintain a <strong>Transition</strong> Steering Committee composed of teachers, parents,<br />
SWAP coordinators and representatives from outside agencies.<br />
Develop and maintain a Handbook for <strong>Transition</strong> Teachers with guidelines, forms,<br />
resources, etc. pertinent to transition.<br />
Develop and maintain a Parent Resource book to be used by teachers when helping<br />
parents contact outside agencies for postsecondary support and services.<br />
Encourage the development of self-advocacy skills and participation in the IEP process<br />
for students with disabilities.<br />
Serve as a liaison between <strong>Colorado</strong> Department of Education and <strong>NW</strong><strong>BOCES</strong> regarding<br />
<strong>Transition</strong> issues.<br />
Evaluate IEP and Summary of Performance documents in each district for compliance<br />
with the requirements of the <strong>Colorado</strong> Department of Education and the Federal Office of<br />
Special Education<br />
Serve as a resource for transition teachers and provide professional development in<br />
current transition issues.
QUALIFICATIONS:<br />
Education:<br />
Bachelor’s Degree in Education; Master’s Degree preferred<br />
Valid <strong>Colorado</strong> teaching certification<br />
Valid Special Education certification<br />
Special Knowledge/Skills:<br />
Knowledge of Special Education policies and procedures, as well as state and<br />
federal guidelines for the educational requirements for transition and special<br />
education students<br />
Excellent organizational, communication, and interpersonal skills<br />
Ability to travel to multiple districts throughout the <strong>BOCES</strong> area<br />
Experience:<br />
Three years of secondary teaching experience preferred
