You may obtain an application form and<br />
information on <strong>th</strong>e Self-Exclusion Program by<br />
requesting an application form from <strong>th</strong>e Security<br />

<strong>th</strong>e attached response card to:<br />

You will be required to provide identification <strong>th</strong>at<br />

<strong>th</strong>e form you will be photographed. That photo,<br />
and o<strong>th</strong>er identifying information will be<br />
distributed to <strong>th</strong>e casinos.<br />

No. You must return <strong>th</strong>e application form<br />
in person to a Security Representative.<br />
The requirement to file in person is for<br />
your protection. Wi<strong>th</strong> few exceptions only<br />
you can put yourself on <strong>th</strong>e self-exclusion list.<br />
what will happen if i go to<br />
a casino and try to gamble?<br />
After you are placed on <strong>th</strong>e self-exclusion list,<br />
casino personnel will refuse to accept your<br />
wagers or ask you to leave <strong>th</strong>e gaming area.<br />
If you do gamble, you would be unable to collect<br />
any winnings or recover any loses. Once you are<br />
placed into <strong>th</strong>e program, you will not be able to<br />
receive complimentary goods or services, credit<br />
or check cashing privileges or <strong>of</strong>fered any o<strong>th</strong>er<br />
amenities by <strong>th</strong>e casino.<br />
how long will i be on<br />
<strong>th</strong>e self-exclusion list?<br />
That’s up to you. When you request self-exclusion,<br />
you will choose whe<strong>th</strong>er you want to be excluded<br />
for a minimum <strong>of</strong> one year, five years or for life.<br />
If you sign up for life, you cannot be removed<br />
from <strong>th</strong>e list. If you choose <strong>th</strong>e one-year or<br />
five-year option, you must remain on <strong>th</strong>e list<br />
for at least <strong>th</strong>at leng<strong>th</strong> <strong>of</strong> time. After <strong>th</strong>at time<br />
expires, you may ask to be removed by writing<br />
a letter to <strong>th</strong>e <strong>Seminole</strong> Gaming Commission.<br />
