You are hereby notified that because of such record, this Department proposes to suspend or revoke your driving privilege or to place your privilege on probation as provided in Sections 12809, 13359, 13950-52, V. C. You are entitled to a hearing to present any evidence, oral or written, as to why the Department should not take the proposed action against your driver’s license. You have the choice of a formal or informal hearing. In either type of hearing, you may present any evidence on your behalf. You are not required to be represented by legal counsel, but your attorney may be present if you wish. In the formal hearing a complete written record is made of the entire proceedings and is available for review of the courts. An informal hearing has been scheduled by the Department of Motor Vehicles, to be held at: A Driver Improvement Analyst will act as referee at the informal hearing. A formal hearing, if requested, will be scheduled in place of the informal hearing.
