Procedures<br />
With <strong>The</strong> <strong>University</strong> <strong>of</strong> Alabama IRB approval and school district approval, individual<br />
schools were provided with information packets explaining the purpose and procedures <strong>of</strong> the<br />
study. Each packet contained copies <strong>of</strong> the following documents that bore the dated IRB stamp:<br />
1. <strong>The</strong> IRB approval letter;<br />
2. School district approval letter;<br />
3. Teacher consent form;<br />
4. Parent cover letter;<br />
5. Parent consent form; and<br />
6. Child assent form.<br />
Twelve school systems were invited to participate in the study. Of the twelve school<br />
systems invited to participate, five school systems elected to participate in the study. <strong>The</strong><br />
investigator met with school administrators representing each <strong>of</strong> the five participating school<br />
systems. At this meeting, school administrators recommended specific schools within their<br />
system to invite into the study. Of these five school systems participating in this study, three<br />
were individual private schools and two were public school systems, representing a total <strong>of</strong> 35<br />
intermediate and elementary schools. One public system administrator recommended four<br />
schools, <strong>of</strong> which one school agreed to participate in the study. <strong>The</strong> other public school system<br />
