Probation Services<br />

(Includes the Community Based, Specialized Units and Youth Development Services)<br />

Placement Permanency Unit<br />

This unit receives and assesses all cases referred by<br />

DCFS, delinquency court, Probation Officers,<br />

Attorneys, Child Advocates and the Placement<br />

Quality Assurance process. All cases are<br />

investigated for parents’ whereabouts,<br />

relatives/non-relative extended family and life-long<br />

connections. Each permanency officer carries a<br />

caseload of 6-8 and meets with the<br />

DCFS/Probation Collaborative Permanency<br />

Committee monthly to discuss all cases destined<br />

for legal guardianship and adoption. Currently, the<br />

committee is working on 3 potential adoptions and<br />

3 potential legal guardianships.<br />

Mentoring Model This is a new program that will begin July 2008<br />

and will handle 800 DCFS and Probation foster<br />

youth, approximately 100 per SPA. This program<br />

is for youth 16 or older where family reunification<br />

has failed for the purpose of obtaining life-long<br />

connections for these aging-out youth.<br />

Transition to Permanency<br />

This program is for youth 18-25 years who have<br />

had contact with the criminal justice system as a<br />

juvenile or an adult and are homeless or at<br />

imminent risk of becoming homeless. The program<br />

will provide financial assistance with rent, moving<br />

costs and appliance. The youth must be employed<br />

or willing to be employed, and the program will<br />

assist with finding the youth shelter until they are<br />

employed. This is the first project that both the<br />

adult and juvenile are working on together.<br />

Placement Community Transition Services This unit identifies youth eligible for Evidence-<br />

Based In-Home Services (FFT and MST) and<br />

assists in transitioning these youth out of Group<br />

Home care and back into the home of their parents.<br />

The PCTS Unit supervises these youth in<br />

community and works with the therapist to support<br />

the success of the intervention.<br />
