When suppliers are unable to deliver ordinary and insured parcels, they will<br />

leave a <strong>com</strong>pleted PS Form 3849 in the customer’s mailbox. The item will be<br />

annotated with date and initial. Upon return to the office, the parcel will be<br />

placed in a location designated by the Administrative Official or designee.<br />

Carriers are required to mark up undeliverable mail and forward mail for<br />

customers who have filed change of address orders. The Administrative<br />

Official, or authorized representative, will instruct carriers in the proper<br />

procedure to be followed.<br />

Carriers should use the procedures established by the Administrative Official<br />

to record changes of address. Generally, all changes of address received by<br />

a carrier on the route are submitted to the Centralized Forwarding System<br />

office for scanning. Suppliers do not maintain records of new customer<br />

addresses.<br />

If a contract box numbering system is in use, the Administrative Official is<br />

responsible for assignment of contract box numbers. Street addressing or<br />

911 addressing is the preferred mode of addressing and is assigned by local<br />

government authority.<br />

44 Report of Exceptional <strong>Service</strong><br />

Suppliers must report and explain to the Administrative Official or designee<br />

daily any deviation from the scheduled line of travel, such as a detour or<br />

curtailment of service on the annotated section of PS Form 5399. The report<br />

must include the miles scheduled on that day and the miles actually traveled,<br />

shown to the nearest 1/10 of a mile.<br />
