appropriate to attempt to model the current situation.<br />

The <strong>in</strong>cremental cost of HemeSelect and Hemoccult II Sensa is high due to the<br />

high price of the first relative to the Hemoccult II and due to the <strong>in</strong>creased<br />

number of colonoscopies generated by both. With the implementation of a<br />

mass screen<strong>in</strong>g program <strong>with</strong> one of those tests the demand for these tests will<br />

<strong>in</strong>crease dramatically. This would offer the government the possibility to<br />

negotiate <strong>with</strong> the <strong>in</strong>dustry about the price of the tests. Lower prices will lower<br />

the expected budget impact and hence the average cost per CRC detected.<br />

8.2.3 Sensitivity analysis<br />

The confidence <strong>in</strong>tervals around the estimated costs of the different<br />

implementation scenarios are large due to the uncerta<strong>in</strong>ty <strong>in</strong> a number of<br />

modell<strong>in</strong>g parameters. The relative impact of the uncerta<strong>in</strong>ty <strong>in</strong> the different<br />

modell<strong>in</strong>g parameters on the estimated total cost of the screen<strong>in</strong>g program is<br />

illustrated by means of an <strong>in</strong>fluence diagram <strong>in</strong> figure 15 for the basel<strong>in</strong>e analysis<br />

apply<strong>in</strong>g a mail<strong>in</strong>g system. The lengths of the bars represent the relative<br />

importance of the variable for the results. They result from the 1000 Monte<br />

Carlo simulations ran on our model for the probabilistic sensitivity analysis.<br />

The by far most important uncerta<strong>in</strong>ty <strong>in</strong> all screen<strong>in</strong>g scenarios is participation.<br />

Uncerta<strong>in</strong>ty <strong>in</strong> the participation rate is the most decisive factor for the<br />

variability <strong>in</strong> the total cost estimate. In the mail<strong>in</strong>g system also mail<strong>in</strong>g costs,<br />

fixed campaign costs and compliance <strong>with</strong> colonoscopy after a positive FOBT<br />

<strong>in</strong>fluence the total cost estimate of mass screen<strong>in</strong>g for CRC. In the GP system<br />

the participation rate is the s<strong>in</strong>gle most important uncerta<strong>in</strong> factor <strong>in</strong> the model.<br />
