Comments. A commenter recommended against the adoption <strong>of</strong> the alternative<br />

standard (CPT-4), unless we subsidized the cost <strong>of</strong> licensing CPT-4 as has been done for<br />

certain other code sets. Some commenters expressed c<strong>onc</strong>erns about the license<br />

requirements and one commenter stated that the license cost will likely be passed down<br />

from the EHR developer to the eligible pr<strong>of</strong>essional or eligible hospital. Some<br />

commenters believed that if we intended to keep this alternative standard, we should<br />

make it freely available.<br />

Response. We understand that most current EHR technology already includes the<br />

CPT-4 code-sets, and we believe that this indicates that the licensing costs are not<br />

prohibitive. Regardless, we have adopted an alternative standard to CPT-4, SNOMED-<br />

CT®, which is freely available.<br />

Comment. A commenter noted that the <strong>certification</strong> criterion references<br />

immunizations but the Medicare and Medicaid EHR Incentive Programs proposed rule<br />

did not include immunizations in the objective. The commenter suggested that we<br />

modify our <strong>certification</strong> criterion to match the proposed rule.<br />

Response. We have removed this term, consistent with the previous revisions we<br />

have made to other <strong>certification</strong> criteria above.<br />

Enable a user to create an electronic copy <strong>of</strong> the discharge<br />

instructions and procedures for a patient, in human readable<br />

format, at the time <strong>of</strong> discharge on electronic media or<br />

through some other electronic means.
