DEFINTIONS AND EXPLANATIONS OF TERMS 

TYPE of facility is listed to provide some of the information that may 

be needed by readers. The terms used reflect either programmatic 

support (which may affect scope of services and/or target 

populations) or traditional descriptions of facilities. 

Community Health Centers (CHC) 

Community Health Centers are funded by a federal grant program 

under Section 330 of the Public Health Service Act and administered 

by the Health Resources and Services Administration (HRSA) of the 

US Department of Health and Human Services. Community Health 

Centers provide comprehensive preventive, primary medical care, 

dental, and mental health services. Not all services are provided onsite; 

some may be provided through contract and referral systems. 

Community Health Centers are governed by a volunteer board of 

directors representative of the communities they serve, with a 

majority of board members who are users of the health center. 

Community Health Centers must provide services to everyone, 

regardless of ability to pay. 

Community Health Center services are not free, but ensure that lack 

of income or insurance are not barriers to care. Patients who are 

not covered by insurance are eligible for discounted fees. Finally, 

Community Health Centers have a responsibility to focus upon the 

health concerns of their entire service area and not only the portion 

of the community that receives care in their clinics. Community 

Health Centers are responsible for working to increase access to 

healthcare within their service areas.

Community Health Aide Program 

The Community Health Aide (CHA) Program developed in the 1950s 

in response to a number of health concerns including the 

tuberculosis epidemic, high infant mortality, and high rate of 

injuries in rural Alaska. In 1968, the CHA Program received formal 

recognition and congressional funding. The long history of 

cooperation and coordination between the federal and state 

governments and the Native tribal health organizations has 

facilitated improved health status in rural Alaska. 

The CHA Program now consists of a network of approximately 500 

Community Health Aides/Practitioners (CHA/Ps) in over 170 rural 

Alaska villages. Community Health Aides/Practitioners work within 

the guidelines of the Alaska Community Health Aide/Practitioner 

Manual, 1998 Revised Edition, which outlines assessment and 

treatment protocols. There is an established referral relationship, 

which includes mid-level providers, physicians, regional hospitals, 

and the Alaska Native Medical Center. In addition, providers such as 

public health nurses, physicians, and dentists make visits to villages 

to see clients in collaboration with the CHA/Ps.

The Alaska Area Native Health Service has the responsibility for 

provision of medical and health related services to Indian Health 

Service beneficiaries residing in Alaska. These services are provided 

by tribal organizations within the Alaska Native Health Care System. 

The village based CHA/Ps are a vital link in the delivery system. 

Community Health Aides are selected by their communities to 

receive training. Training centers are located in Anchorage, Bethel, 

Nome, and Sitka. There are four sessions of CHA training, each of 

which lasts three to four weeks. Between sessions, the CHAs work in 

their clinics completing a skills list and practicum. Completion of the 

four session training curriculum and successful completion of a 

clinical skills preceptorship and examination, qualify the CHA as a 

Community Health Practitioner (CHP). CHA/Ps at any level of 

training may obtain certification by the Community Health Aide 

Program Certification Board. 

The Community Health Aide Program model is currently being used 

as a template to develop programs in the areas of dental care, 

behavioral health, and elder care.

Public Health Nursing Clinics 

State Government, Alaska Department of Health and Social 

Services, Division of Public Health, Section of Public Health Nursing 

operates public health centers and offices in twenty two 

communities throughout the state. These centers are staffed by 

public health nurses who provide community based service delivery 

for disease prevention, health promotion, and a variety of 

population-based services, as well as direct clinical preventive 


services to individuals and families. Public health nurses also 

provide services on an itinerant basis to more than 250 

communities. Services include: prevention and control of infectious 

diseases through immunizations, testing, treatment, contact tracing, 

education and outreach; prevention and control of injuries and 

chronic diseases through education and reduction of health risks, 

referral and linking to needed services; outreach to families and 

children at medical and social risk; pregnancy testing and promotion 

of early prenatal care; public health emergency preparedness and 

response; and participation in community partnerships to address 

issues of health concern. There are charges for some services, with 

sliding fee scale for patients with limited income. No one is refused 

service due to inability to pay. 

Local Government and Not-for-profit Public Health Nursing services 

are provided by the Municipality of Anchorage, Norton Sound 

Health Corporation, Maniilaq Health Services in Northwest Arctic 

Borough, and the North Slope Borough with grant support from the 

State of Alaska. 

Hospitals and Nursing Homes 

26 hospitals in Alaska provide a range of services including primarily 

inpatient care (medical, surgical, rehabilitation, long term acute 

care, psychiatric) and diverse outpatient services. More information

social services.

Not-for-profit refers to organization under section 501(c) of the 

internal revenue code. 

Government organizations refer to state and local government 

operated and military facilities. In some cases in Alaska, a local 

government owns the facility but the clinic or hospital is run by a 

not-for-profit organization. 

Private facilities are those owned and operated by individuals, 

partnerships or corporations without not-for-profit status.

Guide to the Frequency for Community Health Center Providers 

Community and village health centers operated by Alaska's tribal health organizations provide health services to remote communities that 

otherwise would not receive services. While these sites frequently have Community Health Aides and Practitioners in residence, other medical 

providers — including doctors, dentists, mid-levels, and behavioral health practitioners — visit the communities on an itinerant basis. The 

following tables list community health centers organized by managing organization. Each table shows the type of service provided at each 

health center, and the frequency. The frequency key is as follows: 

R=Resident: The provider lives in the community. 

S=Seasonal: In some cases communities will have providers only through the busier summer months. 

M=Monthly: On the average a provider visits each month. 

Q=Quarterly: On the average the provider visits four times a year at regular intervals. 

LQ=Less than Quarterly: On the average the provider visits less frequently that quarterly but more frequently than twice a year. 

Y=Yearly: On the average the provider makes annual visits. 

TY=Twice a year: On the average the provider will visit every six months. 

V=Vacant: There is usually a practitioner at this level but the position is currently vacant. 

AN=As Needed: A practitioner only visits the community when there is a specific need.
