We are pleased to present the second EH&S <strong>Annual</strong> Review. The <strong>Annual</strong> Review highlights the accomplishments of EH&S’<br />
core programs and provides a snap-shot of the safety resources available to the campus community. In <strong>2011</strong>, EH&S<br />
programs have proven to be successful by improving compliance, decreasing the number of repeat incidents, reducing<br />
regulatory fines, protecting employees and creating a culture of safety. These accomplishments reflect the strong commitment<br />
of EH&S’ employees and the entire campus community toward creating a safe and healthy environment.<br />
UC Center for Laboratory <strong>Safety</strong><br />
In <strong>2011</strong>, the UC Center for Laboratory <strong>Safety</strong> was created<br />
to improve the practice of laboratory safety through the<br />
performance of scientific research and implementation<br />
of best safety practices in the laboratory. The mission of<br />
the Center is to:<br />
Sponsor and support research in laboratory safety<br />
Develop and translate applied best practices research<br />
Facilitate implementation and optimization of<br />
laboratory safety practices<br />
Director’s Note<br />
As our <strong>2011</strong> <strong>Annual</strong> Review illustrates, EH&S continues to<br />
improve both productivity and the breadth and level of services<br />
provided to the campus community. Our Research <strong>Safety</strong><br />
Program continues to make remarkable progress and is<br />
approaching 100% compliance with our performance metrics.<br />
Fewer non-compliant findings during PPE inspections is good<br />
news and suggests greater awareness of the importance of<br />
PPE in laboratories and a positive shift in our safety culture. As<br />
part of our continued efforts, EH&S also increased exposure<br />
assessments in labs, shops and offices. Inspections for food<br />
facilities, pools and spas remain at record levels. All in all,<br />
EH&S continues to provide essential services to the campus<br />
and is looking for ways to improve even further. If you have<br />

Laboratory Compliance<br />
Laboratory compliance inspections continue to be a<br />
priority of our Research <strong>Safety</strong> Division. The <strong>2011</strong><br />
inspection results – and the consistently<br />
high results in the three previous years<br />
– demonstrates EH&S’ commitment to<br />
implementing safe laboratory practices.<br />
Since 2008, inspections have increased<br />
by 234% (Figure 1).<br />
Laboratory Performance Metrics<br />
Following inspections, Laboratory<br />
<strong>Safety</strong> Officers timeline is to issue<br />
lab inspection reports the next<br />
business day. In <strong>2011</strong>, 100% of<br />
all initial lab inspection reports<br />
were issued by the next business<br />
day (Figure 2). Since 2009, lab<br />
inspection reports continue to<br />
maintain a high turnaround rate.<br />

Unannounced PPE Compliance Inspections<br />
In 2010, EH&S launched unannounced PPE compliance<br />
inspections and since have reached 477 laboratory<br />
groups (approximately 1,455 lab rooms). The number of<br />
PPE related findings found in labs during PPE inspections<br />
has decreased by 58% since August 2010. Figure 3<br />

In 2010, EH&S implemented a lab safety training<br />
program for Principal Investigators/Laboratory<br />
Supervisors to ensure those individuals most directly<br />
responsible for the health and safety of laboratory<br />
researchers have knowledge of the fundamentals<br />
of laboratory safety, and fully understand their roles<br />
and responsibilities. Figure 4 represents the <strong>2011</strong><br />
completion rates.<br />

Pool and Spa Inspections<br />
During pool/spa inspections for regulatory compliance,<br />
data is collected pertaining to the following:<br />
• Pool water chemistry<br />
• Filtration and recirculation systems<br />
• Management and operations<br />
The number of pools/spas inspected have increased by<br />
219% since 2008 (Figure 5).<br />
Food and Temporary Event Inspections<br />
In <strong>2011</strong>, there were over 130 food<br />
facility and temporary event inspections<br />
conducted for the 49 campus food<br />
facilities, as well as the various temporary<br />
food events, yielding a 57% increase over<br />
2007 (Figure 6).<br />
Industrial Hygiene Assessments<br />
Taking a pro-active approach to exposure assessments<br />
and mold prevention, EH&S has dramatically increased<br />
the number of IH assessments, including 63 exposure<br />
assessments for laboratories. The following assessment<br />
increases are over 2009 (Figures 7 and 8).<br />

Hazardous Materials Minimization<br />
Minimizing hazardous materials on campus has been<br />
achieved by a continued effort to develop programs that<br />
focus on reducing waste and reusing our resources.<br />
• 149 lbs. of chemicals redistributed<br />
• 261 mercury thermometers exchanged<br />
• 176 tons of lab waste safely processed and shipped<br />
• 54% increase in special pick-up requests of lab waste<br />
during laboratory clean-outs (Figure 9)<br />

Accident Investigations<br />
The Injury Prevention Division has significantly<br />
increased investigations of preventable accidents<br />
since 2009 (Figure 10).<br />
• In 2009, 78 accident investigations out of 737<br />
medical and indemnity injuries (11%)<br />
• In 2010, 261 accident investigations out of<br />
814 (32%)<br />
• In <strong>2011</strong>, 363 investigations were completed<br />
out of 787 (46%)<br />
• 365% increase of investigations between<br />
2009 and <strong>2011</strong><br />
Shop <strong>Safety</strong> Inspections<br />
There are 73 general use machine shops on<br />
campus, with 1,000 shop employees. The<br />
Shop <strong>Safety</strong> Coordinator inspects all shops<br />
and provides follow-up inspections (Figure<br />
11). Additionally, the Shop <strong>Safety</strong> Program has<br />
developed tools to provide consistent methods to<br />
evaluate shop hazards, select appropriate PPE<br />
and train employees in the selection, application<br />
and use of PPE for<br />
hazardous tasks. The<br />

Ergonomic Evaluations<br />

The <strong>UCLA</strong> Ergonomics program is responsible for<br />
preventing and reducing workplace musculoskeletal<br />
disorders (MSD). EH&S ergonomists have maintained<br />
a high number of ergo evaluations over the last 4 years,<br />
which has resulted in recommendations that help<br />
reduce these injuries (Figure 12).<br />

Fire and Life <strong>Safety</strong> Inspections<br />
Fire and life safety inspections are a major and vital<br />
component of the Fire and Life <strong>Safety</strong> Division.<br />
As the University expands and new buildings<br />
are constructed and old<br />
buildings renovated, the<br />
number of required Title<br />
19 and Title 24 safety<br />
inspections increase. The<br />
Fire Division has maintained<br />
project schedules and inspections for over $2 billion<br />
dollars in new construction and remodel plans. The<br />
following reviews and inspections have dramatically<br />
increased over 2009:<br />

373% Title 19 inspections (Figure 15)<br />
329% Title 24 inspections (Figure 16)<br />
Evacuation Drills<br />
<strong>Annual</strong>, quarterly, and monthly evacuation drills are<br />
conducted for all the following buildings:<br />

Krieger Day Care<br />
Lab School (UES)<br />
Complex<br />
High-rise buildings<br />
An additional 41 drills were performed for various<br />
buildings. These drills ensure the proper evacuation<br />
of the campus community during an emergency.<br />
Fire <strong>Safety</strong> Training<br />
In <strong>2011</strong>, over 2,600<br />
individuals completed<br />
on-line fire safety training.<br />
The Fire and Life <strong>Safety</strong><br />
Division conducted<br />
hands-on fire extinguisher<br />
training for 551 students<br />
and staff (Figure 17).<br />

Developed an on-line Mandarin language version of<br />
the Lab <strong>Safety</strong> Fundamentals Course<br />
Additionally, EH&S developed lab specific orientation<br />
