Annual Report Edition
News & Notes
Volume 4, No. 2 501 Westwood Blvd . Los Angeles, CA 90095-1605
Since 1978
We are pleased to present the second EH&S Annual Review. The Annual Review highlights the accomplishments of EH&S’
core programs and provides a snap-shot of the safety resources available to the campus community. In 2011, EH&S
programs have proven to be successful by improving compliance, decreasing the number of repeat incidents, reducing
regulatory fines, protecting employees and creating a culture of safety. These accomplishments reflect the strong commitment
of EH&S’ employees and the entire campus community toward creating a safe and healthy environment.
UC Center for Laboratory Safety
In 2011, the UC Center for Laboratory Safety was created
to improve the practice of laboratory safety through the
performance of scientific research and implementation
of best safety practices in the laboratory. The mission of
the Center is to:
Sponsor and support research in laboratory safety
Develop and translate applied best practices research
Facilitate implementation and optimization of
laboratory safety practices
Director’s Note
As our 2011 Annual Review illustrates, EH&S continues to
improve both productivity and the breadth and level of services
provided to the campus community. Our Research Safety
Program continues to make remarkable progress and is
approaching 100% compliance with our performance metrics.
Fewer non-compliant findings during PPE inspections is good
news and suggests greater awareness of the importance of
PPE in laboratories and a positive shift in our safety culture. As
part of our continued efforts, EH&S also increased exposure
assessments in labs, shops and offices. Inspections for food
facilities, pools and spas remain at record levels. All in all,
EH&S continues to provide essential services to the campus
and is looking for ways to improve even further. If you have
any suggestions or concerns, please contact the EH&S
Hotline, 310-825-9797. Thank you again for your continued
commitment to protecting the health and safety of our campus
community. - James Gibson, EH&S Director
2011 EH&S Highlights
Revised the UCLA Employee Safety
Handbook and distributed it to the
University’s Administrative Division.
93% of the 3,000 employees from
the Division have completed review
of the handbook.
Launched a “got safety concerns?”
campaign to promote a safe and
healthy environment by
encouraging the campus
to contact EH&S if they
have any questions or
safety concerns.
Distributed the new UCLA Chemical
Hygiene Plan to every Principle
Investigator (PI) on campus.
Developed an on-line standard
operating procedure (SOP) library
to provide guidance on the safe use
of chemicals and chemical groups. There are 121
SOPs in our library, which have received more than
3,000 views from across the globe.
Laboratory Compliance
Laboratory compliance inspections continue to be a
priority of our Research Safety Division. The 2011
inspection results – and the consistently
high results in the three previous years
– demonstrates EH&S’ commitment to
implementing safe laboratory practices.
Since 2008, inspections have increased
by 234% (Figure 1).
Laboratory Performance Metrics
Following inspections, Laboratory
Safety Officers timeline is to issue
lab inspection reports the next
business day. In 2011, 100% of
all initial lab inspection reports
were issued by the next business
day (Figure 2). Since 2009, lab
inspection reports continue to
maintain a high turnaround rate.
Accident Investigations
The Injury Prevention Division has significantly
increased investigations of preventable accidents
since 2009 (Figure 10).
• In 2009, 78 accident investigations out of 737
medical and indemnity injuries (11%)
• In 2010, 261 accident investigations out of
814 (32%)
• In 2011, 363 investigations were completed
out of 787 (46%)
Shop Safety Inspections
There are 73 general use machine shops on
campus, with 1,000 shop employees. The
Shop Safety Coordinator inspects all shops
and provides follow-up inspections (Figure
11). Additionally, the Shop Safety Program has
developed tools to provide consistent methods to
evaluate shop hazards, select appropriate PPE
and train employees in the selection, application
and use of PPE for
hazardous tasks. The
Hazard Assessment
Tool for Shops (HATS)
assesses hazards and
creates a written hazard
assessment report.
Since the introduction
of HATS in 2011, the
benefits have included:
Shop personnel identified
the PPE required for
job task performed and
purchased the most
effective PPE
Employees actively
participated in the PPE
selections program and
received specific training
on their responsibilities
