Proposer must provide at least three (3)
references where the same or similar scope of
services were provided.
Exhibit 3: Prospective CONTRACTOR List of
Contracts
The listing must include all Public Entities
contracts for the last three (3) years. Use
additional sheets if necessary.
Exhibit 4: Prospective CONTRACTOR List of
Terminated Contracts
Listing must include contracts terminated within
the past three (3) years with a reason for
termination.

Complete Exhibit 20, Arbitration or Litigation
History of Required Forms, to provide listing of
any claims and/or threatened or pending
litigation, by the names of parties and case
numbers, as well as court location(s) involving
CONTRACTOR or any principals, officers, or
directors thereof, which has occurred within
five (5) years prior to submission date of
proposal. Failure to provide this information
on the submission date may result in
deductions in score.
