Sealed and competitive tenders are invited in the prescribed form under two cover system from the
original manufacturers or authorized dealers for the supply of certain PHYSIOTHERAPY EQUIPMENTS for
the upgradation of physiotherapy unit of District Homoeo Hospital,Palakkad .Envelope containing the
tender should be superscribed “Tender for the supply of PHYSIOTHERAPY EQUIPMENTS “(TENDER NO
2/2012‐13)
FIRST COVER(Cover A ) Technical specifications
1.Original catalogue /brochure of the quoted item
2.List of state /central Govt organizations to which these items were supplied
3.After sale service and service network in Kerala
SECOND COVER (Cover B ) Financial,ENCLOSURES
1.Rate of item should be quoted as basic price ,rate of tax and total amount
2.Original tender form duly signed by authorized signatory .
3.Preliminary agreement in Kerala stamp paper worth Rs.100
4.EMD at 1 % of total cost of the quoted item by means of Demand draft drawn in favour of
“The Superintendent ,District Homoeo Hospital,Palakkad “.
Both covers (A & B) must be effectively sealed and put in another cover ,which is also sealed and
submitted .Cover A and B will be opened as per schedule given below.Cover B of those tenders who
satisfy the conditions in Cover A will only be opened.

Panchayath website lsgdkerala.in/palakkad/.Intending tenderors can collect the tender remitting the
cost of tender form from The Superintendent ,District Homoeo Hospital,Palakkad ,during office hours
or may send the exact amount towards the cost of tender form along with amount for postal charges
.Cheques ,postal orders and demand drafts are not accepted towards cost of form.Cost of tender form
is non refundable .Tender forms are non transferable.

Price of original tender form‐ Rs.200 + 4% VAT+ 1% cess.
Late tenders will not be accepted.The tenders will be opened in the presence of tenderors or their
representatives .The items must be delivered at District Homoeo Hospital,Palakkad.The items should
have minimum 1 yr warranty .Items should come with all standard fittings and spare accssories .The
supply should be completed with in 6 weeks from the date of placing supply order.
The rates should be for destination freight prepaid in full inclusive of all taxes and duties
prevailing at ruling rate .The undersigned reserves the right to accept or reject tenders without prior
notice

SPECIAL CONDITIONS

TENDER FOR PHYSIOTHERAPY EQUIPMENTS
1.The rates quoted should be inclusive of all taxes only .
2.The items supplied should have a minimum of year warranty .
3.The rates should be shown in Indian Rupee only.
4.The security deposit furnished by the firm will be released only after the completion of warranty
period. Security Deposit will be 5 % of the total cost.
5.The rates should be for delivery at District Homoeo Hospital ,Palakkad .
6.The successful tenderors should complete the supply of tems with in stipulated time as shown in
supply order.
7.Payment will be effected only after the successful delivery and installation .No advance payment will
be made.
8.The photocopies of the documents should be attested by gazette officer .
9.The supply should be from the new stock noting the original specification ,make of model etc.
10.If the date scheduled for opening of tenders is is declared a holiday ,the next working day will be
considered.
11.offers of conditional supply will not be considered .
12.Tenderors can quote for all items or in part also.
