Sealed and competitive tenders in the prescribed form are invited for the supply of MEDICINES
& DIAGNOSTIC KITS required for conduct of infertility camps in Thrissur district in various panchayats
under the project infertility mission . Separate list of items required with specifications attached.
Envelope containing the tender should be super scribed “Tender for the supply of - MEDICINES
& DIAGNOSTIC KITS -Tender No 1 /2011-2012”.
PAC : Rs.6 lakhs
Price of original tender form : Rs. 200 + 4 % VAT
Envelope containing tender will have the following enclosures
1) Original catalogue/brochure of the quoted item.
2) Certificate of GMP for the products quoted in tender.
3) Rate of item should be quoted as basic price, rate of tax and total amount.
4) List of state/central govt. organizations to which these items were supplied.
5) Original tender form duly signed by authorized signatory.
6) Preliminary agreement in Kerala stamp paper worth 100.
7) EMD at 1 % of total cost quoted item by means of Demand Draft in favour of Deputy director
(AH), Thrissur.
Tender forms are available on all working days from the undersigned. Cheques, postal orders
and demand drafts are not accepted towards the cost of tender forms. Cost of tender forms is nonrefundable.
Tender forms are non transferable. The tender should be accompanied by EMD of 1 % by

full inclusive of all taxes and duties prevailing at the ruling rate. The undersigned reserves the

SPECIAL CONDITIONS
Tender no. 1/11-12
1. The medicine should be of latest standards and should have minimum 75% shelf life left under
normal storage condition at the time of supply.
2. The rates quoted should be as basic price, rate of tax and total price
3. All offers should be from ready stock in the original packing of the manufacturer.
4. Packing and containers of drugs shall be as prescribed in pharmacopeias or drugs and
cosmetics Rules 1945. Containers made of good plastics must be used for those items which
are to be supplied in plastic containers.
5. The Security Deposit furnished will be released only after satisfactory completion of the contract.
6. The rates should be for delivery F. O. R at any part of the district.
7. Ex-go down rates and F. O. R station of Despatch rates offered by the firms are not acceptable
and such tenders will be summarily rejected.
8. The quantities specified in the schedule are only approximate and they may vary according to
actual need.
9. The successful tenderers should complete all supplies with in the stipulated time stated in the
supply orders. No extension of supply period will be granted.
10. The non-availability of raw materials, price hike of raw materials, transportation problems etc will
not be accepted as reasons for non-supply or late supply.
11. The rates should be quoted in Indian Currency, and payment will be made in Indian Currency
only.
12. The undersigned reserves all rights to cancel the tender without assigning any reason thereof.
13. The only jurisdiction for any dispute regarding this purchase will be at Thrissur.
14. The photocopies of the documents produced along with the tender should be attested by a
Gazetted Officer or a Notary.
15. If any of the date scheduled for receipt and opening of tender is declared as holiday (local or
general), the receipt and opening will be shifted to consecutive working day at the same time.
16. Name and place to which the medicines are to be supplied:
Four Regional Artificial Insemination centres at Kodungallur,Cherpu,Chalakudy and
Kunnamkulam. however the tenderer should be ready to supply to any part of the district.
17. The undersigned reserves the right to delete, waive any of the above special Conditions or
cancel the whole tender.
18. The supply should be effected only by the original tenderer and the invoice should also be
furnished only by them.

Specifications
Item 1. High Performance Premium Portable Black and white Ultrasound Scanner with in- built battery
and power back up for Veterinary use.(One number)
System should incorporate facility for high resolution 2D, M-mode, 2B & 4B with state of the art Digital technology
system capable of performing Imaging applications of abdomen,obstetrics,gynaecology,small organ parts and
vascular system.
Multi beam parallel imaging with speckle reduction,tissue harmonic and cross beam imaging technologies.
Transducers with Broad bandwidth beam technology for extreme High resolution 2D imaging.

6. The rates should be for delivery F. O. R destination. Despatch rates offered by the firms are not
acceptable and such tenders will be summarily rejected.
7. The successful tenderers should complete all supplies within the stipulated time stated in the
supply orders. No extension of supply period will be granted.
