following such written notice; provided, however, that AT&T may provide written<br />

notice that such existing services may be Discontinued within fifteen (15) days<br />

following such notice, subject to the criteria described in Section 1.5.5.<br />

1.5.5 AT&T may take the action to Discontinue the provision of existing service upon<br />

fifteen (15) days from the day after AT&T provides written notice of such<br />

Discontinuance if (a) such notice is sent by certified mail or overnight delivery; (b)<br />

BHN has not paid all amounts due pursuant to a subject bill(s), or has not provided<br />

adequate security pursuant to a deposit request; and (c) either:<br />

(1) AT&T has sent the subject bill(s) to BHN within (7) business days of the bill<br />

date(s), verifiable by records maintained by AT&T:<br />

i. in paper or CDROM form via the United States Postal Service (USPS),<br />

or<br />

ii. in magnetic tape form via overnight delivery, or<br />

iii. via electronic transmission; or<br />

(2) AT&T has sent the subject bill(s) to BHN, using one of the media described in<br />

(1) above, more than thirty (30) days before notice to Discontinue service has<br />

been rendered.<br />

1.5.6 In the case of Discontinuance of services, all undisputed billed charges, as well as<br />

applicable disconnect charges, shall become due.<br />

1.5.7 BHN is solely responsible for notifying the End User of the Discontinuance of<br />

service. If, within seven (7) days after BHN’s services have been Discontinued,<br />

BHN pays, by wire transfer, automatic clearing house or cashier’s check, all past<br />

due undisputed charges, including late payment charges, outstanding security<br />

deposit request amounts if applicable and any applicable restoral charges as set<br />

forth in Section A4 of the GSST, then AT&T will reestablish service for BHN.<br />

1.5.7.1 Termination. If within seven (7) days after BHN’s service has been Discontinued<br />

and BHN has failed to pay all past due charges as described above, then BHN’s<br />

service will be Terminated.<br />

1.6 Notices. Notwithstanding anything to the contrary in this Agreement, all bills and<br />

notices regarding billing matters, disconnection of services for nonpayment of<br />

charges, and rejection of additional orders from BHN, shall be forwarded to the<br />

individual and/or address provided by BHN in establishment of its billing<br />

account(s) with AT&T, or to the individual and/or address subsequently provided<br />

by BHN as the contact for billing. All monthly bills and notices described in this<br />

Section shall be forwarded to the same individual and/or address; provided,<br />

however, upon written request from BHN to AT&T's billing organization, the<br />

notice of discontinuance of services purchased by BHN under this Agreement<br />

provided for in Section 1.5.4 of this Attachment shall be sent via certified mail to<br />
