What Can Communities Do?<br />

It really does take an entire community to nurture, care for,<br />

and raise our children-especially those with chr<strong>on</strong>ic illness or<br />

disability. Many of the recommended changes in health care<br />

systems and clinical care can be addressed without<br />

government or agency edict. Often, all that is required is for<br />

professi<strong>on</strong>als, aware of the issue, to make the changes.<br />

The following recommendati<strong>on</strong>s address issues that are<br />

"external" to specific health care delivery systems and clinics<br />

yet are c<strong>on</strong>sidered essential for a community if children with<br />

chr<strong>on</strong>ic illness or disabilities and their families are to thrive.<br />

1. Separate agency-focused case management<br />

from independent family-centered care<br />

coordinati<strong>on</strong>.<br />

I i Families should have access to independent care<br />

coordinati<strong>on</strong> and family advocacy, that is familycentered,<br />

community-based and culturally sensitive.<br />

2. Develop easy access to informati<strong>on</strong>, support,<br />

referral and advocacy for both families and<br />

providers.<br />

U] Develop a teleph<strong>on</strong>e informati<strong>on</strong> service.<br />

C l Develop a computer-based informati<strong>on</strong> system.<br />

3. Improve communicati<strong>on</strong> between agencies<br />

and health systems.<br />

Fl Educate practiti<strong>on</strong>ers to plan for and resp<strong>on</strong>d to<br />

families' anxiety as systems change.<br />

U] Allow for thoughtful and seamless transiti<strong>on</strong><br />

when shifting between public and private funding<br />

systems.<br />

4. Understand that-health care is a market driven<br />

enterprise.<br />

U] Establish incentives for individual health care<br />

systems that reward creative methods of serving<br />

children with chr<strong>on</strong>ic illness or disabilities.<br />
