This report is the outcoome of a first time oDllaborati<strong>on</strong> of HealthPartners, a staff nodetl<br />

health maintenance organizati<strong>on</strong>r (forrnerly known as Group Health Inc.), through its<br />

supporting research organizati<strong>on</strong>, the Group Health Foundati<strong>on</strong>, the Center for<br />

Children with Chr<strong>on</strong>ic Ilness and Disability, a nati<strong>on</strong>al research and training center<br />

based at the University of Minnesota, and PACER Center, a parent training and<br />

informnati<strong>on</strong> center. And while this is an unprecedented collaborati<strong>on</strong>, more<br />

importantly. it represents a collaborati<strong>on</strong> of a wide variety of people an disciplines<br />

who share a omm<strong>on</strong>in visi<strong>on</strong>, providing family-centered care for children with<br />

disabilities and their families. As such, mniy rnew relati<strong>on</strong>ships have been built that<br />

will promnote the rare of children with chr<strong>on</strong>ic illness ard disabilities in Minnesota.<br />
