The transformati<strong>on</strong> to managed care may be the most important development since the<br />

rise of modern medicine and the use of insurance to pay for health care. This transformati<strong>on</strong> has<br />

not <strong>on</strong>ly changed the relati<strong>on</strong>ship between providers and patients but also has transformed<br />

<str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> from a health care financing program to <strong>on</strong>e of the nati<strong>on</strong>'s largest purchasers of<br />

private insurance. As a result of the move to <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> managed care, the Center for Health<br />

Policy Research undertook a nati<strong>on</strong>wide study of the managed care c<strong>on</strong>tracts between states and<br />

managed care plans.<br />

The study -- which is in its sec<strong>on</strong>d phase - has two purposes. The first is to present an<br />

overview of the structure and c<strong>on</strong>tent of the detailed service agreements which describe <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />

managed care arrangements, including what populati<strong>on</strong>s participate and how services are<br />

structured. The sec<strong>on</strong>d purpose is to identify key issues presented by <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g>'s shift from payor<br />

to purchaser of managed care.<br />

In a transformati<strong>on</strong> of this magnitude, the states face enormous challenges in their effort<br />

to c<strong>on</strong>struct integrated delivery systems for poor people and people who have special health care<br />

needs, an effort which has no parallel in the private insurance sector. Am<strong>on</strong>g the primary<br />

challenges is c<strong>on</strong>vincing private companies to enter the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> market which offers relatively<br />

low capitati<strong>on</strong> rates.<br />

While some states have l<strong>on</strong>gstanding experience with managed care and operate in<br />

mature markets, many states do not. 1The-nansformati<strong>on</strong> to <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> managed care is thus in its<br />

infancy and is a highly evoluti<strong>on</strong>ary process. Accordingly, states generally -- with some notable
