There is a huge need to educate c<strong>on</strong>sumers about managed care.<br />

This educati<strong>on</strong> should address the unique needs of people livinq with HIV<br />

and the challenges posed by managing HIV care.<br />

In <strong>on</strong>der for people living with HIV to maximize the benefits of managed care, it is important that they<br />

understand how this system works. C<strong>on</strong>sumer educati<strong>on</strong> is needed to achieve the following five goals:<br />

* Create Informed benefidaries. It is necessary to educate c<strong>on</strong>sumers about the features of man-<br />

aged care and how best to interact with the system to get their needs met. This should cover basic<br />

c<strong>on</strong>cepts such as how to select a health plan, how to select a health care provider, when and how<br />

to seek out-of-network care, and how to grieve or complain about inadequate care or the denial of<br />

health care benefits.<br />

* Develop str<strong>on</strong>g advocates. This goal involves educating people living with HIV and their advocates<br />

about how the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> program works and how managed care programs work. Advocates must un-<br />

derstand basic facts about the structure of <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g>, induding how the waiver authority functi<strong>on</strong>s<br />

that allows states to create mandatory managed care programs. Advocates must also learn about the<br />

<str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> system in their state, induding identifying the key health care players such as federal, state,<br />

and local officials and legislators, as well as other advocates.<br />

a Clarify the distincti<strong>on</strong>s between managed care sad fee-for-service health care and am<strong>on</strong>g managed<br />

care plans. C<strong>on</strong>sumers must gain a better understanding of basic principles of our insurance<br />

system. Individuals must be provided informati<strong>on</strong> <strong>on</strong> how to functi<strong>on</strong> in a new <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> envir<strong>on</strong>-<br />

ment. A part of this indudes giving c<strong>on</strong>sumers informati<strong>on</strong> about how to navigate through the system<br />

when experiencing difficulties with a provider or a managed care organizati<strong>on</strong> as well as how to make<br />

choices when competing benefits are available from various health plans.<br />

* Improve the evaluati<strong>on</strong> and m<strong>on</strong>itoring of <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> managed care plans. Under the fee-for-<br />

service system, a key role for <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> advocates was to m<strong>on</strong>itor the federal oversight of state Medic-<br />

aid programs. lfc<strong>on</strong>sumers were not receiving the health care services they needed, if poor quality care<br />

was being provided, or if eligible individuals were being denied <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> benefits, pressuring the<br />

federal government to exercise its oversight authority <strong>on</strong> state <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g>-prgrams often led to<br />

improvements. In the managed care envir<strong>on</strong>ment understanding the relati<strong>on</strong>ship between the state<br />

<str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> program and numerous managed care organizati<strong>on</strong>s has become far more important in<br />

order to protect high quality care. People living with HIV need to understand how this new system<br />
