Selected <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> HEDIS measures related to children are<br />
abstracted al<strong>on</strong>g with a set of measures developed by the Quality<br />
Community <str<strong>on</strong>g>Managed</str<strong>on</strong>g> <str<strong>on</strong>g>Care</str<strong>on</strong>g> Project. The measures are organized<br />

Title Project ACCORD<br />
Minnesota Department of Health and Health Partners<br />
Descripti<strong>on</strong> The project was designed to create a public-private partnership<br />
between the state health department, HMOs, and key<br />
organizati<strong>on</strong>s within the state to facilitate statewide adopti<strong>on</strong> of<br />
practice guidelines. The special health c<strong>on</strong>diti<strong>on</strong> used is pediatric<br />
asthma. Success is measured in terms of the rate of children with<br />
severe asthma who are <strong>on</strong> anti-inflammatory medicati<strong>on</strong> and the<br />
rate of parents who report a written asthma plan. Implementati<strong>on</strong><br />
goals are to identify and correctly diagnose asthma in children,<br />
provide optimal asthma management through educati<strong>on</strong> and<br />
avoidance of triggers, and establish a comprehensive<br />
pharmacological treatment and asthma acti<strong>on</strong> plan.<br />
