Descripti<strong>on</strong> This set of community systems measures is designed to track<br />

progress in creating improved service systems for all women and<br />

children, including those with special health needs. It is based <strong>on</strong><br />

the principles of Communities Can and is organized into five<br />

areas: 1) early identificati<strong>on</strong> and referral system; 2) primary and<br />

specialized health service network; 3) family satisfacti<strong>on</strong> and<br />

quality of care; 4) assessment, development, and coordinati<strong>on</strong> of<br />

primary, specialized, and related services; and 5) family<br />
