This inspecti<strong>on</strong> focuses <strong>on</strong> the internal adjustments facing State <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> agencies as they<br />
move toward managed care. It starts out by addressing how these adjustments are<br />
influenced by the extent of <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> managed care in a State. It then identifies the major<br />
organizati<strong>on</strong>al challenges faced by the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> agencies as they increase their<br />
commitment to managed care and addresses how they have been resp<strong>on</strong>ding to those<br />
challenges.<br />
In tackling this topic, we recognize that any generalizati<strong>on</strong>s about the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> program<br />
that cross State boundaries can be hazardous. An often expressed adage am<strong>on</strong>g <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />
directors is: 'If you have seen <strong>on</strong>e <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> program, you have seen <strong>on</strong>e <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />
program."' Yet in a broad sense, currents of change are apparent am<strong>on</strong>g the States and<br />
are helpful to highlight, even if they do not adequately reflect what is happening in any<br />
individual State. In our synthesis observati<strong>on</strong>s, we focus <strong>on</strong> those currents. To reflect<br />
some of the diversity that exists and to minimize the danger of our becoming too abstract,<br />
we offer examples of developments occurring in particular States.<br />
Our inquiry is based primarily <strong>on</strong> a review of the recent experiences of 10 State <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />
agencies that have made str<strong>on</strong>g commitments to <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> managed care. The States are<br />
diverse in terms of their locati<strong>on</strong>, size, and experience with managed care.' They are<br />
clearly am<strong>on</strong>g the top half of States in terms of the proporti<strong>on</strong> of their <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />
beneficiaries in managed care. For each of the States, we interviewed agency officials<br />
and reviewed pertinent documents. In three, we c<strong>on</strong>ducted in-depth visits that also<br />
included interviews with representatives of health plans, advocacy groups, State<br />
legislatures, other State agencies, and <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> enrollees.<br />
Our inquiry does not address the internal adjustments that the move toward managed care<br />
presents for other State agencies, such as those focusing <strong>on</strong> mental health, public health,<br />
mental retardati<strong>on</strong>, elder affairs, and social services. These adjustments can be significant<br />
since <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> often serves as a major source of funding for their service operati<strong>on</strong>s and<br />
since managed care can lead to major changes in these operati<strong>on</strong>s. The organizati<strong>on</strong>al<br />
effects in these other agencies warrant attenti<strong>on</strong>. But with the reverberati<strong>on</strong>s of managed<br />
care likely to be most immediate and c<strong>on</strong>sequential within in the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> agencies, we<br />
have decided to c<strong>on</strong>centrate this inquiry <strong>on</strong> them.<br />

issued by the President's Council <strong>on</strong> Integrity and Efficiency.<br />
