Adults with developmental disabilities are highly dependent <strong>on</strong> public<br />

programs for meeting their l<strong>on</strong>g-term care needs. Most pers<strong>on</strong>s with<br />

developmental disabilities have mental retardati<strong>on</strong>, but others have severe,<br />

chr<strong>on</strong>ic disability resulting from cerebral palsy, epilepsy, or other life-l<strong>on</strong>g<br />

c<strong>on</strong>diti<strong>on</strong>s, except mental illness, that began before they were 22 years<br />

old. The populati<strong>on</strong> with developmental disabilities receives more than<br />

$13 billi<strong>on</strong> annually in public finding for l<strong>on</strong>g-term care, sec<strong>on</strong>d <strong>on</strong>ly to<br />

the elderly. More than 300,000 adults with developmental disabilities<br />

receive government l<strong>on</strong>g-term services financed primarily through<br />

<str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> and to a lesser extent through state and local programs.<br />

L<strong>on</strong>g-term care services can include supervisi<strong>on</strong> and assistance with<br />

everyday activities such as help in dressing, going to the bathroom,<br />

managing m<strong>on</strong>ey, and keeping out of danger. Pers<strong>on</strong>s with developmental<br />

disabilities have traditi<strong>on</strong>ally received their l<strong>on</strong>g-term care in instituti<strong>on</strong>al<br />

settings.<br />

Recently, states have begun to significantly expand the use of the <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g><br />

1915(c) home and community-based waiver, enacted by the C<strong>on</strong>gress in<br />

1981, to provide alternatives to instituti<strong>on</strong>al care for pers<strong>on</strong>s with<br />

developmental disabilities.' The waiver program has two advantages. First,<br />

it gives states a tool to c<strong>on</strong>trol costs by allowing them to limit the number<br />

of recipients served. In c<strong>on</strong>trast, states must serve all eligible individuals<br />

in the regular <str<strong>on</strong>g>Medicaid</str<strong>on</strong>g> program Sec<strong>on</strong>d, it permits states to meet the<br />

needs of many pers<strong>on</strong>s with developmental disabilities by offering them a<br />

broader range of services in less restrictive settings such as group or<br />
