PACE replicarl<strong>on</strong>.<br />

FRail older pers<strong>on</strong>s in Milwaukee County have a, to a Unique ptngPm which provbtu for dark l<strong>on</strong>gter<br />

health care needs. This propam is called Community <str<strong>on</strong>g>Care</str<strong>on</strong>g> for the Eldedly, or Ca; which replicates<br />

the PACE modcl, a managed cart system pi<strong>on</strong>ecrcd by On Lok System of San Francisco.<br />

CCE is avoluntary alernatve to nrsinghomecare for frai sc who wish torematni<strong>on</strong>d<strong>on</strong>pdeat.be<br />

program targets frail seniors whose physical andfor mental status make them ligIble for nurdng home<br />

placement CCE program participants attend an adult day health censtr and receive ci v e<br />

medical and social services while living in the Community.<br />

Community <str<strong>on</strong>g>Care</str<strong>on</strong>g> Organizati<strong>on</strong> (CCO), a private n<strong>on</strong>-profit arganizi<strong>on</strong> in Milwaukee, begun operatng<br />

CCE in 1990. CCE also operas WPP. a program for frail elderly pes<strong>on</strong>s and thoce with disabilities who<br />

would normally be placed in a nusing home or instituti<strong>on</strong>.<br />

A distinctive feature of CCE is the multi-disciplinary team that coordinates the health of each participan<br />

Members of CCE's multidisciplinary team work together to meet aR the needs of the individuaL Thc<br />

individual or family ecver needs to negotiate aervices with amitiple providmr and _acii<strong>on</strong>etv, the team<br />

takes care of everything. And becaus CCE caregivers have daily e<strong>on</strong>mct with enrollee they can detect<br />

any subtle changes in c<strong>on</strong>diti<strong>on</strong> and modify the individual's treatent plan acotrdingly.<br />

Many of the services provided as CCE are provided at the progam't tdult day health <strong>on</strong>tee Parlicip nts<br />

in the program generally atend a day health cenerelat ast <strong>on</strong>c per week In additi<strong>on</strong> to raiviag health<br />

and social services. CCE's 340 PACE participants enhance their social life bypjoining a wide varety of<br />

activities offered at the center.<br />

Services at CCE's adult day health care centers inchlde the following:<br />

* Lunch.<br />
