If you do not pick an MCO, the State will pick<br />

Once you pick an MCO or are assigned <strong>on</strong>e by the<br />

State, you can change to another MCO <strong>on</strong>e time<br />

during the first year. People who move to a new<br />

county will be able to choose a new MCO near them<br />

at any time. If you need to change your MCO, call<br />

1-800-977-7388.<br />

V Make an appointment with your MCO<br />

doctor or health care provider.<br />

V Tell your doctor about your health<br />

care history. Discuss the services that<br />

you got in the past. Your doctor can<br />

refer you to a specialist if needed.<br />

to get Pers<strong>on</strong>al <str<strong>on</strong>g>Care</str<strong>on</strong>g> and Medical Day<br />

<str<strong>on</strong>g>Care</str<strong>on</strong>g> if you have these services now.<br />

MCO <str<strong>on</strong>g>Special</str<strong>on</strong>g> Needs Coordinator.<br />

If you need a ride to your health visits because of a<br />

disability, call your local health department. The ph<strong>on</strong>e<br />

numbers are listed <strong>on</strong> the last page of this booklet.<br />
