cecropin<br />

An antibacterial protein derived from immunized cecropia<br />

moth pupae and also found in butterflies. Cecropin is a<br />

basic protein that induces prompt lysis of selected Gramnegative<br />

and Gram-positive bacteria.<br />

celiac disease<br />

An inflammatory immunological disease of the intestinal<br />

mucosa attributable to an immune response against gluten<br />

proteins found in selected cereals, including wheat. Refer to<br />

gluten-sensitive enteropathy.<br />

celiac sprue (gluten-sensitive enteropathy)<br />

Gluten-sensitive enteropathy resulting from hypersensitivity<br />

to cereal grain storage proteins, including gluten or its<br />

product gliadin, present in oats, wheat, and barley. It is characterized<br />

by villous atrophy and malabsorption in the small<br />

intestine. It occurs mostly in Caucasians and occasionally in<br />

African-Americans, but not in Asians. Individual patients<br />

may have the disease limited to the intestines or associated<br />

with dermatitis herpetiformis, a vesicular eruption of the<br />

skin. The mucosa of the small intestine shows the greatest<br />

reactivity in areas in contact with gluten-containing food.<br />

Antigliadin antibodies are formed, and lymphocytes and<br />

plasma cells appear in the lamina propria in association with<br />

villous atrophy. Gluten-sensitive enteropathy is associated<br />

with HLA-DR3, -DR7, and -DQ2, as well as with HLA-B8.<br />

Diagnosis is made by showing villous atrophy in a biopsy of<br />

the small intestine. Administering a gluten-free diet leads<br />

to resolution of the disease. Antigliadin antibodies are of<br />

the immunoglobulin A (IgA) class. Both T and B cell limbs<br />

of the immune response participate in the pathogenesis of<br />

this disease. Patients may also develop IgA or IgG reticulin<br />

antibodies. An α gliadin amino acid sequence shares homology<br />

with adenovirus 12Elb early protein. Patients with celiac<br />

disease develop immune reactivity for both gliadin and this<br />

viral peptide sequence. The disease occurs more frequently<br />

in individuals exposed earlier to adenovirus 12. Patients<br />

develop weight loss, diarrhea, anemia, petechiae, edema, and<br />

dermatitis herpetiformis, among other signs and symptoms.<br />

Lymphomas such as immunoblastic lymphoma may develop<br />

in 10 to 15% of untreated patients.<br />
