<strong>Investigation</strong><br />
First form teams of four from your community health clinical<br />
students<br />
Next read the <strong>Outbreak</strong> Response Packet on Web CT<br />
Plan a time to meet and conduct the investigation. You may choose a<br />
time at your convenience.<br />
<strong>Community</strong> members who will have the disease plan to meet to<br />
determine how the responses needed in the interview process. For<br />
example what restaurant, what day, what food is common to your<br />
experience.<br />
Investigators read the text to determine what questions are<br />
necessary to obtain the diagnostic information.<br />
All group members will go to the fast food restaurant as a team<br />
The community members will participate in the observation of the<br />
environment<br />
Document the experience<br />
Your group of 2 investigators has been contacted to investigate the report<br />
of two cases of diarrheal disease in a class of <strong>Community</strong> <strong>Health</strong> nursing<br />
students. Your job is to conduct a Foodborne Disease <strong>Outbreak</strong> case<br />
questionnaire.<br />
Divide your clinical group into two (2) investigators and (2) three members<br />
of the community.<br />
Organize: From your group choose a leader who will be responsible for the<br />
organization of the group and a member to document the assignment.<br />
Prepare: Prepare to investigate by reading:<br />
1.) Summary of Salmonella serotype Enteritis <strong>Outbreak</strong> Reported in 2002<br />
and<br />
2.) <strong>Investigation</strong> table that shows the etiologic agent, incubation period,<br />
clinical syndrome, and confirmation.

Interview: Two community members who will role play as individuals who<br />
now experience diarrheal disease. Use the standard foodborne disease<br />
outbreak case questionnaire.<br />
As a victim of the assigned etiologic agent role play all of the aspects of<br />
the clinical syndrome. For example, abdominal pain, bloody diarrhea,<br />
cramping, nausea etc. Check the time onset of symptoms compared to the<br />
time of exposure.<br />
Reveal this information only as you are interviewed. The investigators will<br />
need to question you carefully to obtain this information. Stay in role<br />
throughout the interview.<br />
Analyze: After the interview, investigators will review the information and<br />
investigate<br />
common food-related exposure,<br />
common environmental experience<br />
common chemical exposures, air-borne exposures, common<br />
stressors, and/or any other related information.<br />
The community members that are being interviewed are to choose a<br />
situation that involves a fast food restaurant in the local area and a<br />
meal that they would have chosen on a school day.<br />
Assume your role to reveal the symptoms or clues only with careful<br />
questioning.<br />
Interviews are to be done with only the community member and the<br />
investigator present. Indicate on the questionnaire both names for<br />
credit.<br />
Interview script with case study for role play is given to the<br />
individual community member and is not to be shared with the<br />
investigator except in responding to questions in interview.<br />
From the information gained in the interview the investigators will<br />
analyze the information.<br />
The investigators will go to the fast food restaurant to notice:<br />
• Overall cleanliness<br />
• Tables cleaned between meals<br />
• Employees neat and clean<br />
• Employees touching money do not handle food<br />
• Floor clean in eating area<br />
• Restroom clean

• People in restroom wash hands (If you can observe and provide<br />
privacy)<br />
• Trash contained neatly<br />
• Sanitation Scores noted if posted<br />
• Food handler certification for each employee (The last employee<br />
at a local restaurant stated that this was required for employees<br />
in Wichita Falls.) You will not be able to know this, however it<br />
would be relevant in an investigation<br />
• Food storage off floor in closed containers<br />
• Food temperature cold or hot as required<br />
• Buffet clean and protected without contamination of servers or<br />
people<br />
• People in environment appear healthy<br />
Determine the most probable diagnosis:<br />
• Meet as a group to determine any common elements found in the<br />
investigation.<br />
• Review the Table Etiologic Agent Chart to find the most probable<br />
disease agent.<br />
Communicate the findings:<br />
• Do not write or reveal the fast food establishment you<br />
visited to anyone.<br />
• Do not in your observation of the fast food establishment<br />
show the purpose of the visit.<br />
• Turn a one-page summary of the <strong>Investigation</strong> at the<br />
beginning of your lab next week. Be sure to list team<br />
members with roles and the final decision of the team.<br />
Meet next Thursday at 3 in Room 305 to debrief.
