Introduction<br />

Welcome to Module III of the <strong>Radiation</strong> <strong>Oncologist</strong> <strong>Training</strong>. This book is your<br />

guide to unlocking the possibilities of MOSAIQ v1.20 application software, hereafter<br />

referred to as MOSAIQ. It is designed to be used during your instructor-led training,<br />

and as a handy desk reference any time you need a quick reminder how to do<br />

something in MOSAIQ. We recommend that you keep this book near your<br />

workstation so you can find it easily when needed.<br />

This training guide is not intended to replace the MOSAIQ documentation, which is<br />

contained in the MOSAIQ Help system. The MOSAIQ Help system provides indepth<br />

information on all available MOSAIQ features.<br />

<strong>Radiation</strong> <strong>Oncologist</strong> <strong>Training</strong> is an Intermediate course that covers how to use checklists,<br />

documents, and reports in MOSAIQ. This course assumes that you have basic<br />

Windows skills and have taken, and are familiar with, the concepts presented in<br />

MOSAIQ General Introduction <strong>Training</strong>. Topics presented in this course build upon the<br />

concepts learned in the General Introduction.<br />

In the first module, you learned how to use the MOSAIQ user interface, view the<br />

scheduling and code-capturing system, view patient information, and enter diagnosis<br />

information. In the second module, you learned how to enter course/Care Plan<br />

information, add order sets, and work with radiation prescriptions, as well as work<br />

with labs, vitals, and assessments. In addition, you generated and viewed orders.<br />

What’s In This Module<br />

In this module, you will work with Quality check items and documents. In addition,<br />

you will view the Report Navigator and print reports from the Report Navigator and<br />

the Diagnoses and Interventions window.<br />

This module will not teach you basics on using MOSAIQ, which are covered in the<br />

General Introduction.<br />

Note that when you are working in the practice database, all activities are enabled for<br />

the practice ID. In your clinical database, you may or may not have security rights to<br />

do all the activities that you will do in this module.

Document Conventions<br />

This document uses specific conventions, or rules, in the instructions to help you<br />

perform the exercises correctly. This section explains the text, mouse, and keyboard<br />

conventions used in this document.<br />

Text Conventions<br />

The instructions in this guide are formatted to make them easy to read and to<br />

recognize when you should perform a particular task. The following text conventions<br />

are used in the instructions:<br />

Bold Print Indicates menu or list items that you select.<br />

Example: Click File | Register New Patient….<br />

Example: In the Select Patient window, click Lamb,<br />

Mary.<br />

Italics Indicates words or phrases that appear on the computer<br />

screen, references to text or a number as itself, or new terms<br />

that are being defined.<br />

Highlighted<br />

Print<br />

Example: Position the insertion point to the left of p in<br />

patient.<br />

Example: Annotation is the process of adding customized<br />

notes to a Help topic.<br />

Indicates something you type.<br />

Mouse Conventions<br />

Example: Click .<br />

Notes, hints and warnings are preceded by unique symbols<br />

and are presented in a shaded box. Notes provide ancillary<br />

information, hints provide tips to help you use the system,<br />

and warnings provide important information on the<br />

consequences of performing a particular action.<br />

You can also display the eCHART Navigator window by pressing<br />

F5.<br />

You may find it helpful to print out illustrations from the Help<br />

reference tools to include with patient education materials.<br />

Once you change the status of an order to Voided, you cannot<br />

change the status again.<br />

The following terms are used to describe actions you perform with the mouse:<br />

Click Press and release the left mouse button.<br />

Double-click Click the left mouse button twice in rapid succession.<br />

Right-click Press and release the right mouse button.<br />

Drag Move the mouse while holding down the left mouse button.<br />

Highlight Drag the mouse pointer across data, causing the information<br />

to appear in reverse video.<br />

Keys that do not produce a character appear in all upper case bold print. These keys<br />

Lesson 9 Using Quality<br />

Checklists<br />

Objectives<br />

You will learn how to:<br />

• View Quality Checklist items<br />

• Add Quality Checklist items<br />

• Complete Quality Checklist items<br />

Refer to Quality Checklist Tutorial (LTG14001) for information on<br />

configuring the Quality Checklists and Quality Checklist Items.<br />

Quality Checklists are designed to help you track adherence to any type of procedure,<br />

including established clinical pathways, clinical trials, or any other type of quality<br />

assurance protocols used at your facility. You can track almost any activity at your<br />

facility for which you want to create accountability.<br />

Procedures such as simulations, follow-up protocols, or pre-treatment tasks, can all be<br />

tracked using Quality Checklists, then monitored on-line or in printed reports. Quality<br />

Checklists are especially useful to remind staff members which critical tasks must be<br />

accomplished, and when they must be completed. Quality Checklists are also useful<br />

for procedures that must have some type of verification recording that they were<br />

completed on time.<br />

In this lesson, you will work with quality checklist items. You will view, add, and<br />

complete quality checklist items.<br />

To select an existing patient, you must have Administrative |<br />

General | View rights. To admit a new patient, you must have<br />

Administrative | New Patient | Modify rights.<br />

To view the Quality Checklists, you must have Quality Checklist |<br />

View rights.<br />

All subsequent actions, such as adding an item, require Quality<br />

Checklist | Modify rights.<br />

To capture codes, you must have the following General Security<br />

rights: Administrative | Charge Capture | Modify rights.

Viewing the Quality Checklist Items<br />

You can view quality checklists for a particular patient or set of dates, as well as quality<br />

checklist items associated with a selected staff member or location. In the following<br />

exercises, you will view Quality Checklists by patient, daily, staff, and location, and<br />

view completed checklist items by staff. In addition, you will also use the various<br />

filters, such as the Due Date Range.<br />

Using the Due Date Range Filter<br />

The Due Date Range filter is available from all views. In this exercise, you will use the<br />

Due Date Range filter to display items whose due date falls within a certain date range.<br />

1. Click Tools | Quality Checklist | Daily….<br />

Displays the Daily Quality Checklist window for all patients, shown in Figure 1.<br />

2. Click next to the From field.<br />

Notes 3. Type the current year in the Year field.<br />

Specifies the year for which to display items.<br />

4. Type the current month in the Month field.<br />

Displays the calendar for the month and year you entered as shown in Figure 3.<br />

5. Click the first day of the month in the calendar, then click .<br />

Displays all items whose due dates fall on or after the date you selected. Any<br />

items with an earlier due date no longer display as shown in Figure 4.<br />

6. Type a date two days before the current date in the To field.<br />

Displays all items for the specified date range. All items before and after these<br />

dates do not display as shown in Figure 5.<br />

Viewing Patient Quality Checklists<br />

The Patient Quality Checklist enables you to view items associated with a specific<br />

patient. In this exercise, you will view a patient’s Quality Checklist.<br />

1. Open the chart for your lesson patient, then click Tools | Quality Checklist<br />

| Patient….<br />

Displays the Quality Checklist window for the currently selected patient, shown in<br />

Figure 6.<br />

If nothing displays, try changing the To date to a future date such<br />

as 12/31/2008 and/or changing the From date to a prior date<br />

such as 1/1/2008.<br />

The Patient Quality Checklist contains the quality checklist items that make up the<br />

quality checklist and consists of the following columns:<br />

Due Displays the due date for the item. The due date is either a<br />

fixed date you entered, or a calculated date based on the<br />

required date and a quality checklist group’s start date.<br />

Actual Displays the date that the item was actually completed. This<br />

day may or may not correspond to the due date listed in the<br />

first column.<br />

Procedure Displays the procedure or activity to perform.<br />

Device/Field Applies to Proton Therapy only.<br />

Req Displays the initials of the person or location requesting the<br />

item.<br />

Resp Displays the initials of the person or location responsible for<br />

performing the item.<br />

C/S Displays the initials of the person who actually completed or<br />

skipped the item. If the item has not been completed or<br />

skipped, this column is blank.<br />

MD Displays the initials of the attending physician for the patient.<br />

Status Displays the status of the item. If not completed or skipped,<br />

the status is blank. S displays if the item was skipped. C<br />

displays if the item was completed. ChC displays if the item<br />

was charged when completed.<br />

Note Displays whether the item has an associated note. Displays Y<br />

if a note exists or blank if there is no note.<br />

Comment Displays the any comments associated with the item.<br />

Notes Viewing Daily Checklists<br />

The Daily Checklist enables you to view items for a specific date range, which you set<br />

using the Due Date Range section fields. In this exercise, you will view a daily quality<br />

checklist.<br />

1. In the Patient Quality Checklist window, click next to the View field.<br />

Displays the View field list, shown in Figure 7.<br />

2. Click By Date.<br />

You can also click Tools | Quality Checklist, then click the<br />

checklist to display a checklist. For example, click Tools |<br />

Quality Checklist | Daily… to display the Daily Quality<br />

Checklist.<br />

The Daily Quality Checklist window contains the quality checklist items listed for<br />

the specified date(s).

Notes Viewing Staff Checklists<br />

The Staff Quality Checklist enables you to view items associated with a particular staff<br />

member. This staff member is the person responsible for the item. IMPAC<br />

recommends that you check your Staff Quality Checklist on a regular basis, such as<br />

every few hours. In this exercise, you will view a staff quality checklist.<br />

1. In a Quality Checklist window, click next to the View field, then click By<br />

Staff.<br />

Displays the Staff Quality Checklist window, shown in Figure 9.<br />

The Staff Quality Checklist window contains the quality checklist items for the<br />

staff member displayed in the Staff field. It does not display items for which the<br />

staff member may not have been responsible, but completed anyway. It also does<br />

not display items that the staff member may have requested.<br />

2. Click next to the Staff field.<br />

Displays the Staff Directory window, shown in Figure 10.<br />

Viewing Location Quality Checklists<br />

The Location Quality Checklist enables you to view the items for which a location is<br />

responsible. In this exercise, you will view a location quality checklist.<br />

1. In the Staff Quality Checklist window, click next to the View field, then click<br />

Adding Quality Checklist Items<br />

A quality checklist item may or may not be billable. A billable item generates a charge<br />

code when completed; non-billable items do not. You create and assign billable items<br />

the same way as non-billable items, except that you specify a code group or specific<br />

charge code to be charged to the patient for billable items. In the following exercises,<br />

you will add a non-billable item to the patient’s Quality Checklist with a note, as well<br />

add billable items with a charge code and a charge group.<br />

Adding a Non-Billable Item to the Patient’s Quality Checklist<br />

In this exercise, you will add a non-billable item to the patient’s Quality Checklist and<br />

include a note.<br />

1. In a Quality Checklist window, click .<br />

you have no way of knowing who is responsible when you view<br />

items from any Quality Checklist window, the staff members who<br />

should perform the procedures may not know they are<br />

responsible, and those staff members cannot resolve the items<br />

from the Staff Quality Checklist window.<br />

When you complete a billable item, MOSAIQ generates a charge code and charges<br />

the patient for the quality checklist item. In this exercise, you will add a billable quality<br />

checklist item associated with a specific charge code to the Quality Checklist.<br />

Completing Quality Checklist Items<br />

When you perform a quality checklist item, you mark it as completed. Completed<br />

items display the due date as well as the date it was marked as completed. If an item<br />

is billable, the Code Capture dialog box displays, which you can use to capture the<br />

charge. In the following exercises, you will complete, as well as skip, non-billable and<br />

billable quality checklist items.<br />

Completing a Non-Billable Item from a Quality Checklist Window<br />

In this exercise you will complete a non-billable quality checklist item from the Staff<br />

Quality Checklist window.<br />

1. In a Quality Checklist window, click a non-billable Quality Checklist item.<br />

Specifies the item to complete.<br />

read an attached note before completing the item. In this exercise, you will complete<br />

an item from the Quality Checklist Item dialog box.<br />

Specifies the item to complete and displays the Quality Checklist Item dialog box.<br />

2. In the Complete section, click Yes.<br />

Objectives<br />

You will learn how to:<br />

• Viewing documents<br />

• Editing documents<br />

MOSAIQ has three documentation needs: PhAST Note, eSCRIBE, and eSCAN.<br />

• eSCRIBE is an on-line transcription system that enables you to create or<br />

import patient documents. The resulting documents are available from<br />

eCHART, the MOSAIQ electronic charting system. You can also use<br />

eSCRIBE and MOSAIQ to manage your paper documents electronically.<br />

• eSCAN enables you to import PDF and other file types, such as GIFs and<br />

JPEGs. Typical documents include pathology reports and treatment plans.<br />

• PhAST Note (Physician Assisted Structured Template Note) enables you to<br />

develop a structured patient encounter note. You can select individual items<br />

as you assess a patient. When finished, MOSAIQ generates a narrative based<br />

on your selected findings. In addition, MOSAIQ suggests E & M code(s)<br />

based upon the document content. Note that you must purchase PhAST<br />

Note separately.<br />

In this lesson, you will work with eSCRIBE, eSCAN, and PhAST Note documents.<br />

You will view, edit, and approve documents.<br />

To run eSCRIBE, you must have Administrative | Import/Export |<br />

View rights. To import an eSCRIBE document, you must also have<br />

Modify rights.<br />

To view the Documents window and any documents, you must<br />

have Clinical | Documents | View rights. For pending or unfinished<br />

documents, you must also have Clinical | View Pending Docs |<br />

View rights.<br />

To change eSCRIBE or PhAST Note documents, you must have<br />

Clinical | Documents | Modify rights.<br />

To approve a document, you must have Clinical | Documents |<br />

Admin rights. To co-sign a document, you must also have Clinical<br />

Co-Sign Docs | Admin rights.

Using the Documents Window<br />

The Documents window is your main point of access to creating, editing, viewing,<br />

and printing documents. You can add, change, or delete documents from this<br />

window, as well as view, edit, print, and change document status. In the following<br />

exercises, you will use the different features of the Documents window.<br />

Using the View Field<br />

1. Open the chart of your lesson patient, then click eChart | Documents….<br />

Displays the Documents window, shown in Figure 51.<br />

The View field enables you to sort the documents by patient, dictating physician,<br />

and more. Depending on the View selected, certain columns, such as Dictating<br />

MD, Transcriptionist, or Dates / To, may or may not display.

3. Click next to the Dictating MD field, then double-click the Attending Physician<br />

for your lesson patient.<br />

Selects the dictating physician, closes the Staff Directory window, then returns to<br />

the Documents window, shown in Figure 53.<br />

Note that documents associated with other patients display in addition to your<br />

If a non-physician selected My Pending, the View field displays By Transcriptionist<br />

(another option in the View field) and the Status field displays Pending. A new field,<br />

Transcriptionist displays in the header. You can then select the non-physician in the<br />

Transcriptionist field to display the Pending documents for the staff member.<br />

Using the Status Field<br />

In addition to filtering using the View field, you can also use the Status field to filter the<br />

Documents window. In this exercise, you will filter the Documents window using the<br />

Status field.<br />

1. Click next the View field, then click By Patient.<br />

Specifies that you want to view only documents for the currently selected patient.<br />

Note that Pending displays in the Status field and that only Pending documents<br />

currently display, shown in Figure 55.<br />

Using the Date Range<br />

The Date Range filter enables you to display only those documents from a certain date<br />

range, based on the Encounter date of the document. By default, the Documents<br />

window displays the documents imported or created during the previous month. In<br />

this exercise, you will use the Date Range filter.<br />

1. In the Documents window, click in the Date Range section.<br />

Displays all documents regardless of Encounter date, shown in Figure 56.<br />

Viewing Documents<br />

You can view eSCRIBE, PhAST Note, and eSCAN documents from the Documents<br />

window, the Documents pane in the Chart View, or from the My Work List pane in the<br />

Home View. In the following exercises, you will view documents from the different<br />

windows and panes.<br />

Viewing Documents from the Documents Window<br />

1. Open the chart of your lesson patient, then click eChart | Documents….<br />

Displays the Documents window. The F column indicates the format of the<br />

document. For example, E indicates an eSCRIBE document, P indicates a PhAST<br />

Note document, and S indicates an eSCAN document.<br />

2. Click an eSCRIBE document, then click .<br />

Displays the eSCRIBE document. An example is shown in Figure 58.<br />

Notes The Document Editor enables you to view or change an unapproved eSCRIBE,<br />

PhAST Note. You can only view approved eSCAN documents. If the eSCRIBE<br />

or PhAST Note document is approved, you cannot change the document, but<br />

you can copy the information to a new document. You will edit documents in<br />

another exercise.<br />

From the Document Editor, you can use the upper buttons to move from page<br />

to page. The lower buttons enable you to edit, print, or change the status of the<br />

document, as well as view or change the encounter record for the document<br />

from the Encounter dialog box. You can also move from document to<br />

document, viewing documents that display in the Documents window based on<br />

your current filters (such as the My Pending view and/or a date range).<br />

3. Click , then click .<br />

Closes the Document Editor, then closes to the Documents window.<br />

Viewing Documents from the Documents Pane<br />

You can also view documents for a specific patient from their Chart View. The<br />

Documents pane enables you to either display the Documents window or view the<br />

document directly. In this exercise, you will view documents from the Documents pane<br />

on the Chart View.<br />

1. In the toolbar, click .<br />

The Documents pane displays the same information as in the Documents window.<br />

However, unlike the Documents window, you can decide what information<br />

displays and in what order.

Viewing Documents from My Work List Pane<br />

The My Work List pane enables you to keep track of all your tasks, including quality<br />

checklists, schedules, orders, and documents. In this exercise, you will view documents<br />

from the My Work List pane.<br />

1. In the toolbar, click , then click next to the Documents work list in the My<br />

Work List pane.<br />

Editing Documents<br />

You can edit eSCRIBE or PhAST Note documents that have not yet been approved.<br />

You cannot change the content of eSCAN documents, though you can add or<br />

remove pages from an eSCAN document. In the following exercises, you will edit an<br />

eSCRIBE, eSCAN, and PhAST Note document.<br />

Notes Editing an eSCRIBE Document<br />

You can edit an eSCRIBE document, depending on status. You cannot edit a<br />

document with a status of Approved. In this exercise, you will edit an eSCRIBE<br />

document.<br />

1. Click an eSCRIBE document, then click .<br />

Displays the eSCRIBE Document Editor, shown in Figure 64. If you selected an<br />

approved document, the Cannot Edit message box displays. If you clicked<br />

, the Encounter dialog box displays first.<br />

Note that the patient information, such as name, medical record ID, and date of<br />

birth automatically display. You may briefly see a message box that indicates the<br />

Word template is merging fields. Note also that the eSCRIBE toolbar, shown in<br />

You cannot change the content of an imported eSCAN document, but you can add<br />

Note that the Document Editor displays, but the PhAST Note document is<br />

structured differently. The PhAST Note document consists of headings and predefined<br />

selections, called findings. Although you can add free-form text, the bulk<br />

of the PhAST Note document is selected from headings and their findings. Once<br />

you have finished, the PhAST Note software generates a narrative based on the<br />

selected findings.<br />

The Text tab enables you to enter text or record dictation and attach the text or<br />

recording to a finding or heading within your PhAST Note. The Free Text area<br />

lets you use standard phrases, create your own Text Library, or add free-text<br />

specific to the current document. The Dictation area lets you record findings and<br />

save them as a .wav file. You can click a finding and dictate a recording into a<br />

Some findings take you automatically to the Form tab. Clicking selections in the<br />

Form tab enhances the finding with more information, such as severity and status.<br />

This information can increase the weight of the E&M code generated by the<br />

finding. You can select only one Prefix, Modifier, Result, or Status for a finding. You<br />

can use the Onset, Value, Duration, and Unit fields to further define when the<br />

finding began and how long it lasted.<br />

5. Click next to the chest pain finding.<br />

Displays the subfindings for the selected finding. Click to contract. A finding<br />

can have a status of Positive (abnormal) or Negative (normal). Click to<br />

Changing the Status of Documents<br />

Once you have completed the document, you can approve the document. In this<br />

exercise, you will approve a document.<br />

1. In the Documents window, select one of the documents you added.<br />

Specifies the document that you want to approve.<br />

2. Click .<br />

Displays the Record Status dialog box. Note that the Tiered Security Info section<br />

now displays. If the document requires a co-signer, the information displays in<br />

this section.<br />

3. In the New Status section, click Approved.<br />

Specifies that you want to approve the document.<br />

4. Type your Password, then click .<br />

Approves the document, closes the Record Status dialog box, then returns to the<br />

Documents window, shown in Figure 73.<br />

In this lesson, you will display the Report Navigator and run reports from the Report<br />

Navigator and from dialog boxes or windows such as the Diagnoses and<br />

To print a report you must have the following security rights:<br />

Administrative | General | View .<br />

Note that you must have Administrative | Charge Capture | View<br />

rights to print the Charges by Patient/Date report.

In the Report Navigator, you can display a brief description of each report, run a<br />

compiled report, or determine the window or dialog box with which the report is<br />

associated.<br />

Displaying the Report Navigator<br />

In this exercise, you will display the Report Navigator window.<br />

1. Click .<br />

Alternately, you can press F3 or click File | Print Reports…<br />

to display the Report Navigator window.<br />

The Report Navigator window has multiple folders and uses a “tree” structure<br />

similar to that of Windows Explorer. Each folder has a title and contains one or<br />

more related reports that you can display on the computer screen, print to a<br />

printer, or print to a file.<br />

Notes Using the Report Navigator Window<br />

The Report Navigator window has multiple folders, each with one or more reports<br />

that you can run. In this exercise, you will use the Report Navigator window to move<br />

through the folders and sub-folders.<br />

1. In the Report Navigator window, click next to the Code Capture folder.<br />

Displays the reports for that folder (or any subfolders) as shown in Figure 75.<br />

Note the inactive , which displays in the lower right-hand corner of<br />

the Description section. If a selected report is available directly from a dialog box<br />

or window elsewhere in MOSAIQ, this button becomes active. Clicking it opens<br />

the dialog box or window. From there, you can bring up the right-click Options<br />

menu to select the report(s). If this button is inactive after you select a report, the<br />

report is only available from the Report Navigator window.<br />

2. Click one of the reports.<br />

Note that is active, indicating that you can run the report from the<br />

Report Navigator window. is also active, indicating that you can also<br />

If your facility has MOSAIQ Medical Billing, you can also view and<br />

print reports from the Billing View.<br />

If you do not know which folder a report resides in or even the view (Billing or Main),<br />

you can use the Find view to search for it. In this exercise, you will search for a report<br />

from the Find view of the Report Navigator window.<br />

1. In the Find view of the Report Navigator window, click in the Find field, then<br />

type treatment.<br />

Indicates the criteria for which you want to search. The Find feature searches the<br />

titles of the reports, as well as their descriptions.<br />

2. Press ENTER.<br />

Displays any reports matching the criteria you entered as shown in Figure 78.

Once you have found the report, you can run it from the Report Navigator window.<br />

In this exercise, you will run a report.<br />

1. In the Main view of the Report Navigator window, select<br />

Diagnosis/Staging/Course from the Patient Chart folder, then click<br />

The Print dialog box enables you to display the report directly on the computer<br />

screen, print the report to the network or local printer, or print the report to a file<br />

(such as an Microsoft Excel file).<br />

3. In the Print Report section, click Printer.<br />

Specifies that you want to print the report to the printer. If you want to select a<br />

printer other than the default, or want to change the printer settings, click<br />

. For this exercise, accept the default settings.<br />

4. Type 2 in the Copies field.<br />

Specifies that you want two copies of the report printed. Your changes should<br />

look similar to the ones shown in Figure 81.<br />
