Evacuations necessary.<br />

Environmental media the material was released to, if any.<br />

Cleanup procedures taken.<br />

Description of equipment damage and evacuation status.<br />

Identities of other agencies to which spill reports will be made.<br />

Hazardous Materials Spill Response Contractor<br />

The emergency coordinator will determine if the incident requires the assistance of a hazardous<br />

waste spill response contractor. If the proposed TSF requires outside contractor assistance, the<br />

request will be made upon determination.<br />

7 REVIEW OF THE CONTINGENCY PLAN<br />

The Contingency Plan must be reviewed and amended if necessary whenever:<br />

Applicable regulations are revised.<br />

The Plan is modified to improve response.<br />

The proposed TSF changes in its design, construction, operation, maintenance, or other<br />

circumstances – in a way that materially increases the potential for incidents.<br />

The list of personnel changes.<br />

The list of emergency equipment changes.<br />

The amended Contingency Plan will be provided to the original agencies and outside support<br />

services in a timely fashion.<br />
