The notification will include at least the following information:<br />

Name and phone number of the emergency coordinator.<br />

Name and address of the proposed TSF.<br />

Date, time, and type of the incident (spill, fire, etc.).<br />

Causes of the incident or emergency.<br />

Description of corrective actions taken to prevent recurrence of such incident or<br />

emergency.<br />

Name, identification, and quantity of chemicals involved in the incident.<br />

Report of damages to human health and the environment, including harm to others and<br />

the extent of damage outside the proposed TSF.<br />

An estimate of the amount of the chemicals, chemical wastes, and materials that require<br />

disposal as a result of the incident or emergency.<br />

A plan for disposal of the chemical wastes and materials resulting from the incident or<br />

emergency.<br />

Events that require implementation of the Contingency Plan will be documented in the proposed<br />

TSF operating record. Within 15 days of the accident, PMR will submit a written report<br />

describing the incident to the Administrator of NDEP (40 CFR 264.56(g)-(j)).<br />

5 EMERGENCY RESPONSE EQUIPMENT AND MATERIALS<br />

Emergency response equipment will be available in the immediate work area. The proposed TSF<br />

personnel will be trained in the response actions required for a release of hazardous material. The<br />

ready access to response materials is key to an effective response effort. All response activities<br />

will consider the potential migration of the hazardous waste and minimize the potential impact to<br />

human health and the environment.<br />

The equipment and materials to be used to respond to an incident involving hazardous waste are<br />

located in process areas. This equipment and materials include the following:<br />

<strong>Dry</strong> chemical fire extinguishers - extinguish chemical fires.<br />

Splash goggles - protect eyes.<br />

Butyl or neoprene gloves - protect hands.<br />
