The genus comprises of some 2500 serovars that are defined by serological reactions recognising surface antigens including the Vi capsular antigen, the somatic O antigen and the often phase variable flagella H antigen and can be is divided into six subspecies. S. enterica subsp. enterica affects warm blooded animals including man whereas the other subspecies are very rarely associated with disease. Furthermore, certain serovars within subspecies I are associated with specific hosts. Host adaptation and the form of disease mediated by Salmonella is serovar specific. S. Typhi and Paratyphi cause severe systemic (typhoidal) disease in man. Other serovars such as S. Gallinarum are adapted to cause a systemic disease in poultry and rarely affects man. On the other hand, serovars such as S. Enteritidis and S. Typhimurium are promiscuous causing disease in man and animals. However, the mode of disease is largely gastro-intestinal (non-typhoidal). These two serovars cause the significant majority of human Salmonellosis. Within the EU, 100,000 cases were recorded in 1997 reducing to 73,000 in 2001. In the United States, estimates of 30,000 to 40,000 cases per year have been made. The epidemiology of non-typhoidal Salmonellosis in man over the past decades worldwide has been dominated by S. Typhimurium, S. Enteritidis, S. Newport, S, Heidelberg and S. Hadar. However, there are geographical variances with Hadar more pronounced in Europe than either Newport or Heidelberg. Why S. Typhimurium, particularly the multiresistant clones such DT104, and S. Enteritidis, notably phage type 4, emerged so dramatically in recent years are still unclear although hypotheses abound. Review Summary The rationale behind this project was considered important. Historical examination of salmonella data from humans and animals suggests that, despite the measures already put in place, a future epidemic in humans is likely to arise from infections in the animal population. Since eradication of salmonella from animals is unlikely to be feasible, it will 42
e important to identify an epidemic strain early, so that measures can be taken to reduce the size and cost of any resulting epidemic. It is also important to identify the factors that make a strain of salmonella epidemic, and also why such strains disappear in due course. This work helps maintain an important section of expertise at VLA and keeps the VLA at the forefront of scientific methodology in this area, particularly with regard to array work and developments in GIS. The potential benefits outweigh the risks associated with the development and application of new techniques. The panel felt that the work would yield valuable data when completed. 43
