Health Care In<strong>format</strong>ion on the Web: Model and Menace<br />

Transcend<strong>in</strong>g traditional product l<strong>in</strong>es of foods, drugs, devices, and providers, today’s medical<br />

organizations “market” <strong>in</strong>tangibles such as risk-assessment/reduction, self-care and self-triage,<br />

plan/network enrollment, and, more generally, appropriate utilization of the professional resources they<br />

provide. Thus, health care offers its constituents specific benefits for purchas<strong>in</strong>g and utiliz<strong>in</strong>g their<br />

offer<strong>in</strong>gs <strong>in</strong> certa<strong>in</strong> ways.<br />

Ironically, an <strong>in</strong>creas<strong>in</strong>gly customer-centered <strong>in</strong>dustry must now control customer expectations<br />

and manage demand as never before. Thus, the importance of communicat<strong>in</strong>g specific differences<br />

between appropriate and <strong>in</strong>appropriate care-seek<strong>in</strong>g and lifestyle behaviors has never been greater.<br />

Behavioral changes are more likely to derive from personal experience than through abstract reason<strong>in</strong>g.<br />

The Web permits the <strong>in</strong>tegration of multimedia learn<strong>in</strong>g tools, updated <strong>in</strong><strong>format</strong>ion, a range of reputable<br />

sources and easy access to the histories of others. However, it often lacks a means through which to<br />

<strong>in</strong>tegrate an <strong>in</strong>dividual’s characteristics and experience <strong>in</strong>to Web-based health-related <strong>in</strong><strong>format</strong>ionseek<strong>in</strong>g.<br />

For example, few sites offer any of the follow<strong>in</strong>g: detailed, <strong>in</strong>teractive algorithms for evaluat<strong>in</strong>g<br />

providers and plans, comprehensive provider directories, simplified explanations of plan certificates,<br />

explanations for common diagnostic tests and procedures, differential diagnoses, or even reference values.<br />

Moreover, most summaries of diseases are simply text blocks, with few hypertext or other l<strong>in</strong>ks to relevant<br />

literature, topics or providers. The <strong>in</strong>tegration of forms and search eng<strong>in</strong>es is also fairly m<strong>in</strong>imal, given<br />

such basic uses as: plan and address changes, personalized benefits <strong>in</strong>quiries, <strong>in</strong>teractive maps,<br />

questionnaires, pilot tests, compla<strong>in</strong>ts, compliments, etc.<br />

Professionals <strong>in</strong> many fields, not only health care, are frequently chastised for their failure to offer<br />

customers a clearly-def<strong>in</strong>ed role <strong>in</strong> specify<strong>in</strong>g the product. The plumber says simply, “you need a new<br />

dra<strong>in</strong> pipe.” The physician states, “you need to start a beta blocker,” not necessarily address<strong>in</strong>g the<br />

consequences of each alternative.<br />

Basic concerns, such as clarify<strong>in</strong>g terse statements about diagnosis or treatment, or why certa<strong>in</strong><br />

questions are asked dur<strong>in</strong>g an exam<strong>in</strong>ation, rema<strong>in</strong> difficult to address under the hierarchial, parochial<br />

structure of most Web-based health <strong>in</strong><strong>format</strong>ion. Many sites restate <strong>in</strong><strong>format</strong>ion that can be found<br />

elsewhere, rather than l<strong>in</strong>k<strong>in</strong>g to other sites as appropriate and build<strong>in</strong>g a site based on their <strong>in</strong>dividual<br />

strengths. Content providers benefit when the highest quality, most creatively-presented Web-based<br />

<strong>in</strong><strong>format</strong>ion on every conceivable health topic is available readily to their patients and other constituents.<br />

Three overlapp<strong>in</strong>g viewpo<strong>in</strong>ts characterize Web-based health care <strong>in</strong><strong>format</strong>ion:<br />

Model: The Web facilitates <strong>in</strong><strong>format</strong>ion flow, <strong>in</strong>creases sophistication about health, and fosters<br />

realistic expectations and the management thereof. Over time, the Web and other media will successfully<br />

enhance self-responsibility, lifestyle changes, and health ma<strong>in</strong>tenance as opposed to symptom-focused,<br />

quick-fix care.<br />

Market<strong>in</strong>g Tool: The Web should be utilized as any other market<strong>in</strong>g tool. Thus, content providers<br />

on the Web should focus on specific messages, complement other market<strong>in</strong>g tactics, and mediate<br />

unfavorable messages whenever possible. To do otherwise cedes the Web to a random assortment of<br />

uncontrolled data.<br />

Menace: The Web is more a menace to appropriate care-seek<strong>in</strong>g than a medium for education or<br />

market<strong>in</strong>g. It allows low-quality messages to be accessed repeatedly, thus cont<strong>in</strong>ually lower<strong>in</strong>g the<br />

aggregate quality of health care <strong>in</strong><strong>format</strong>ion available. “Quackery” assumes an artificial importance s<strong>in</strong>ce<br />

Web users cannot differentiate sources of <strong>in</strong><strong>format</strong>ion and their credentials as easily as with other media.<br />

Thus, providers and other responsible parties should focus their efforts on traditional media <strong>in</strong> an attempt<br />

to counter and perhaps neutralize the Web's impact.<br />

Certa<strong>in</strong> characteristics of the Internet strongly <strong>in</strong>fluence the usage of health care <strong>in</strong><strong>format</strong>ion,<br />

such as:
